이 연구의 목적은 TACE와 재조합 인간 아데노바이러스 5형 주사가 수술이나 국소 절제 요법을 받을 수 없는 진행성 간세포 암종(HCC) 환자의 결과를 개선하는지 여부를 결정하는 것입니다.

상세 설명
경동맥 화학색전술(TACE)은 현재 절제 불가능한 간세포 암종(HCC) 환자를 위한 전 세계적으로 완화 치료의 주류 중 하나입니다. 그러나 TACE로 치료받은 HCC 환자의 장기 결과는 일반적으로 좋지 않았습니다.
E1B 유전자 결실 아데노바이러스인 재조합 인간 아데노바이러스 5형은 상당한 항종양 활성을 갖는 것으로 알려져 있다.
또한, 재조합 인간 아데노바이러스 5형의 국소 주사는 항종양 요법(화학 요법 및 방사선 요법)의 효과를 높일 수 있다.
가설은 절제 불가능한 HCC 환자가 TACE와 조합된 재조합 인간 아데노바이러스 5형에서 이익을 얻을 수 있다는 것입니다.

포함 기준:
- 유럽 간 연구 협회(European Association for Study of the Liver) 기준에 따라 HCC로 새로 진단된 환자.
- BCLC 단계 A 또는 B
- Child-Pugh 클래스 A 또는 B(Child-Pugh 점수 7)
- ECOG 수행 상태 0
환자는 다음 기준을 모두 충족하는 종양 병변이 하나 이상 있어야 합니다.
- 병변은 RECIST 기준에 따라 적어도 한 차원에서 정확하게 측정될 수 있습니다.
- 병변은 이전에 수술, 방사선 요법, 고주파 절제, 경피적 에탄올 또는 아세트산 주사 또는 냉동 절제로 치료되지 않았습니다.
- 이전에 비표적 병변에 대한 국소치료(RFA, PEI, 냉동절제술, 수술, 절제술)를 받은 환자
- 국소 치료는 기준선 스캔 최소 4주 전에 완료되어야 합니다.
- 혈액학:절대 호중구 수(ANC) > 1 x 109/L, 혈소판 수 > 40 x 109/L, 헤모글로빈 > 9g/dL(이 수준을 유지하거나 초과하기 위해 수혈될 수 있음) 프로트롬빈 시간 국제 표준화 비율 < 1.5
- 생화학: 총 빌리루빈 < 2 mg/dL 혈청 크레아티닌 < 1.5 x 정상 상한
- 프로토콜을 이해하고 서면 동의서에 동의하고 서명하는 능력
제외 기준:
종양 인자
- 간외 전이의 존재
- 주로 침윤성 병변
- 양쪽 엽을 포함하는 광범위한 병변이 있는 미만성 종양 형태.
혈관 합병증
- 간동맥 혈전증, 또는
- 주요 문맥의 부분적 또는 완전한 혈전증, 또는
- 반대쪽 엽의 문맥 분지의 종양 침습, 또는
- 간정맥 종양 혈전 또는
- 션트 막힘에 잘 견디지 못하는 상당한 동맥문맥 션트
간 기능
- 진행성 간질환: 복수, 간성뇌증
- 연구 시작 전 30일 이내에 임상적으로 유의한 위장관 출혈이 있는 환자.
기타
- 혈액 투석 또는 복막 투석이 필요한 신부전
- 임산부 또는 수유부.
- 활성 패혈증 또는 출혈.
- 정맥 조영제에 대한 과민증.
- 환자는 HCC 표적 병변에 대한 이전 치료를 받았습니다.
심장 질환의 역사
- 울혈성 심부전 > NYHA 클래스 2; 활동성 관상 동맥 질환
- 베타 차단제 또는 디곡신 이외의 항 부정맥 치료가 필요한 심장 부정맥.
- 고혈압은 최적의 의학적 관리에도 불구하고 수축기 혈압 > 150mmHg 또는 확장기 혈압 > 90mmHg로 정의됩니다.
- 중증의 치유되지 않는 상처(2차 치유에 의한 상처 치유 포함), 급성 또는 치유되지 않는 궤양 또는 3개월 이내의 골절.
- 환자는 조사자의 의견에 따라 치료 및 연구 지침을 따를 수 없고/없거나 따를 의향이 없습니다.
- 환자의 연구 참여 또는 연구 결과 평가를 방해할 수 있는 약물 남용(현재), 심리적 또는 사회적 조건.
- 모든 활동성 임상적으로 심각한 감염(> 등급 2 NCI-CTCAE 버전 3.0)
- HIV 감염 또는 AIDS 관련 질병 또는 심각한 급성 또는 만성 질병(병력 기반)
