Fase Ⅲ Proef van transkatheter-arteriële chemo-embolisatie (TACE) plus recombinant humaan adenovirus type 5-injectie voor inoperabel hepatocellulair carcinoom (HCC)
Het doel van deze studie is om te bepalen of TACE plus recombinant humaan adenovirus type 5-injectie de uitkomst zal verbeteren bij patiënten met gevorderd hepatocellulair carcinoom (HCC) die niet vatbaar zijn voor chirurgie of lokale ablatieve therapie.

Transarteriële chemo-embolisatie (TACE) is momenteel een van de steunpilaren van palliatieve behandelingen wereldwijd voor patiënten met inoperabel hepatocellulair carcinoom (HCC). De langetermijnresultaten waren echter over het algemeen slecht voor HCC-patiënten die met TACE werden behandeld.
Van recombinant humaan adenovirus type 5, een adenovirus met E1B-gen verwijderd, is bekend dat het een significante antitumoractiviteit heeft.
Bovendien kan lokale injectie van recombinant humaan adenovirus type 5 het effect van antitumortherapieën (chemotherapie en radiotherapie) versterken.
De hypothese is dat patiënten met inoperabel HCC baat kunnen hebben bij recombinant humaan adenovirus type 5 in combinatie met TACE.

Onderzoekers zoeken naar mensen die aan een bepaalde beschrijving voldoen, de zogenaamde geschiktheidscriteria. Enkele voorbeelden van deze criteria zijn iemands algemene gezondheidstoestand of eerdere behandelingen.

Beschrijving
Inclusiecriteria:
- Patiënten die onlangs zijn gediagnosticeerd als HCC volgens de criteria van de European Association for Study of the Liver.
- BCLC-stadium A of B
- Child-Pugh klasse A of B (Child-Pugh score 7)
- ECOG-prestatiestatus van 0
Patiënten moeten ten minste één tumorlaesie hebben die aan beide volgende criteria voldoet:
- De laesie kan nauwkeurig worden gemeten in ten minste één dimensie volgens de RECIST-criteria
- De laesie is niet eerder behandeld met chirurgie, radiotherapie, radiofrequente ablatie, percutane ethanol- of azijnzuurinjectie of cryoablatie.
- Patiënten die eerdere lokale therapiebehandelingen (RFA, PEI, cryoablatie, chirurgie, resectie) hebben ondergaan voor niet-doellaesies komen in aanmerking
- Lokale therapie moet ten minste 4 weken voorafgaand aan de nulmeting zijn voltooid.
- Hematologie: absoluut aantal neutrofielen (ANC) > 1 x 109/l, aantal bloedplaatjes > 40 x 109/l, hemoglobine > 9 g/dl (kan worden getransfundeerd om dit niveau te behouden of te overschrijden) Protrombinetijd internationaal genormaliseerde ratio < 1,5
- Biochemie: Totaal bilirubine < 2 mg/dL Serumcreatinine < 1,5 x de bovengrens van normaal
- Vaardigheid om het protocol te begrijpen en akkoord te gaan met en een schriftelijk geïnformeerd toestemmingsdocument te ondertekenen
Uitsluitingscriteria:
Tumor factoren
- Aanwezigheid van extrahepatische metastase
- Overwegend infiltratieve laesie
- Diffuse tumormorfologie met uitgebreide laesies waarbij beide kwabben betrokken zijn.
Vasculaire complicaties
- Trombose van de leverslagader, of
- Gedeeltelijke of volledige trombose van de hoofdpoortader, of
- Tumorinvasie van poorttak van contralaterale kwab, of
- Hepatische adertumor trombus, of
- Aanzienlijke arterioportale shunt die niet vatbaar is voor shuntblokkade
Lever functie
- Gevorderde leverziekte: ascites, hepatische encefalopathie
- Patiënten met klinisch significante gastro-intestinale bloedingen binnen 30 dagen voorafgaand aan deelname aan het onderzoek.
Anderen
- Nierfalen waarvoor hemo- of peritoneale dialyse nodig is
- Zwangere of zogende vrouwen.
- Actieve sepsis of bloeding.
- Overgevoeligheid voor intraveneuze contrastmiddelen.
- De patiënt is eerder behandeld voor HCC-doellaesie.
Geschiedenis van hartaandoeningen
- Congestief hartfalen > NYHA klasse 2; actieve coronaire hartziekte
- Hartritmestoornissen die andere anti-aritmica vereisen dan bètablokkers of digoxine.
- Hypertensie gedefinieerd als systolische bloeddruk > 150 mmHg of diastolische druk > 90 mmHg ondanks optimale medische behandeling.
- Ernstige niet-genezende wonden (inclusief wonden die secundair genezen), acute of niet-genezende zweren of botbreuken binnen 3 maanden.
- De patiënt kan en/of wil naar het oordeel van de onderzoeker de behandel- en studie-instructies niet opvolgen.
- Middelenmisbruik (actueel), psychologische of sociale omstandigheden die de deelname van de patiënt aan het onderzoek of de evaluatie van de onderzoeksresultaten kunnen belemmeren.
- Elke actieve klinisch ernstige infectie (> graad 2 NCI-CTCAE ver 3.0)
- HIV-infectie of AIDS-gerelateerde ziekte of ernstige acute of chronische ziekte (gebaseerd op medische geschiedenis)

Experimenteel: TACE plus adenovirus
Na identificatie van de doelslagader van HCC, recombinant humaan adenovirus type 5-injectie (15,0 * 1011 vp: 0,5 ml * 3)
wordt eerst geïnfundeerd via de doelslagader van de HCC-patiënt en gevolgd door chemotherapie (E-ADM 50 mg, Lobaplatin 50 mg, MMC 6 mg) en lipiodol-emulsie of/en polyvinylalcoholdeeltjes (afhankelijk van de tumorgrootte) Procedure: TACE (transcatheter arteriële chemo-embolisatie)
