大うつ病は、妊娠中の女性の最大 10% に影響を与える深刻な状態であり、発育中の胎児に深刻な影響を与えます。 しかし、現在の治療法は、中等度から重度のうつ病の女性にとって理想的とは言えません。 心理療法だけでは効果がないか、症状が改善するまでに数か月かかるため、その間、胎児はうつ病になりやすくなります。 抗うつ薬は効果的ですが、薬にさらされる恐れがあるため、標準的な薬理学的治療の拒否率が高い. 妊娠中のうつ病が発達中の胎児と子供に及ぼす非常に悪い影響は、タイムリーで革新的な治療法を評価する必要があることを示しています。
経頭蓋直流電流刺激法 (tDCS) は、うつ病の非薬物療法であり、体や脳の他の部分に影響を与えることなく、背外側前頭前皮質 (個人が落ち込んでいるときに異常に機能する脳の一部) を直接刺激することができます。 そのため、胎児をうつ病の薬物治療の影響にさらしたくない妊婦にとって理想的な治療法です。 妊娠していない成人のうつ病に効果的であることが示されており、3週間の治療コースで急速に改善が見られ、標準的な心理療法のほぼ3倍の速さです. 重大な悪影響は知られておらず、胎児への理論上のリスクもありません。
この調査研究では、妊娠中のうつ病の治療オプションとしての tDCS の実現可能性、受容性、コンプライアンスを測定します。 さらに、この研究では、妊娠中の中等度から重度のうつ病を患っている女性の治療直後の抑うつ症状の軽減に対するtDCSの効果を調査します。
この多施設パイロット無作為対照試験では、妊娠中の中等度から重度のうつ病の成人女性が、1 年間にわたって 1 つの病院の産科グループと 2 つの専門の周産期メンタルヘルス クリニックから募集されます。 女性は、SSRI (選択的セロトニン再取り込み阻害剤) または SNRI (選択的セロトニン-ノルエピネフリン再取り込み阻害剤) の投薬を開始または継続するように提案されましたが、使用を拒否しました。 すべての参加者は、試験中、それぞれの臨床プログラムから引き続き臨床ケアを受けます。 このケアには、アクティブな tDCS 治療フェーズが完了する前に開始される心理療法の介入が含まれる場合がありますが (クリニックの心理療法の待機リストが少ない場合)、精神療法の治療の最初の 3 週間以内に改善が見られることは期待できません。 そのため、これは、女性から非薬理学的標準治療を奪うことなく、新しい治療法の有効性を評価する理想的な機会です。
インフォームド コンセント手順に続いて、参加者は tDCS または偽対照条件 (1:1) に無作為に割り付けられ、オンサイト治療は週 5 日、3 週間にわたって 30 分間のセッションで行われます。 介入は、アクティブな 2mA 経頭蓋直流刺激 (tDCS) です。 直流電流は、生理食塩水に浸した一対のスポンジ電極 (接触面積 5 x 7cm) で転送され、特別に開発されたバッテリー駆動の定電流刺激装置によって供給されます。 電極は、EEG (脳波) 配置の国際システムに従って、F3 と F4 の上に配置されます。 偽刺激は、同じ刺激パラメーターを使用して、実際の治療部位に投与されますが、電流は 30 秒後にオフになります。
女性はベースラインでインタビューを受け、その後、治療中、出産まで4週間ごと、産後4週間および12週間で追跡され、抑うつ症状、妊娠、出産、新生児および乳児の転帰を測定できるようになります。 ベースラインと治療のインタビューは直接行われますが、治療後と出産後のインタビューは、参加者の好みに応じて、直接または電話で提供されます。

包含基準:
- 18歳以上の妊婦
- 登録時の妊娠12週以上
- 最初の治療来院時の妊娠32週以下(妊娠中にすべての治療が行われる可能性を高めるため)
- 大うつ病性障害の診断で、精神病的特徴のない中等度から重度の大うつ病エピソードがある (Mini-International Neuropsychiatric Interview、MINI で確認)。
- 外来精神科治療に安全です(研究PIによる評価)。
- 抗うつ薬の使用を申し出たが断った
- 治療に同意できる
- 研究説明が理解でき、英語での問診票を記入できる
除外基準:
- -過去6か月間のアルコールおよび/または物質の使用または依存のDSM-V履歴
- -付随する主要で不安定な医学的または神経学的疾患または発作の病歴
- 現在カルバマゼピンを服用している(これは陽極tDCSの効果を妨げる可能性があります)、
- -現在の妊娠における主要な合併症および/または既知の胎児の異常 研究チームによって決定された
- 現在の妊娠で出産前にトロントを離れることを計画しています。
