せん妄は、意識、注意、認知、知覚の急性障害によって現れる全体的な脳機能障害の状態です。 それは短期間(通常は数時間から数日)で発症し、1 日の中で変動する傾向があります。 報告されている心臓手術後のせん妄の発生率は 21% から 47% でした。 術後せん妄の発生は、罹患率の増加、入院期間の延長、医療費の増加、死亡率の上昇など、患者の転帰に重大な悪影響を及ぼします。 その発生は、長期的な認知機能の低下や生活の質の低下にも関連しています。
POD の発生につながる正確な病因はまだ不明であり、手術後の炎症、痛み、睡眠不足などのメカニズムが関与している可能性があります。 さらに、麻酔管理も何らかの影響を及ぼした可能性があることが示されています。 研究により、(バイスペクトルインデックスモニタリングの指導の下で)不必要な深い麻酔を回避すると、PODの発生率が減少することがわかりました。 理論的には、適切な麻酔深度を維持しながら麻酔薬の必要量を減らす対策、および炎症を軽減し、術後の鎮痛と睡眠の質を改善する対策により、POD の発生率は減少します。
デクスメデトミジンは、呼吸抑制を最小限に抑えながら、不安緩解、鎮静、適度な鎮痛を提供する、選択性の高いα-2 アドレナリン受容体アゴニストです。 研究では、デクスメデトミジンを補助麻酔薬として使用すると、麻酔中のオピオイド鎮痛薬やその他の鎮静薬の必要量が大幅に減少することが示されました。 私たちのグループの最近の研究では、手術後最初の夜に低用量デクスメデトミジン (0.2 μg/kg/h) を連続注入すると、主観的な睡眠の質が大幅に改善されることがわかりました。 2件のランダム化対照試験では、従来の鎮静剤(ミダゾラムやプロポフォール)や鎮痛剤(モルヒネなど)と比較した場合、心臓手術後の患者におけるデクスメデトミジンの使用はせん妄のリスク低下と関連していることが判明した。 しかし、従来の鎮静剤/鎮痛剤自体がせん妄のリスクを高めるため、デクスメデトミジンがせん妄を予防するのか、それともせん妄の発生を増加させないだけなのかについては依然として疑問が残っています。 さらに、動物実験では、デクスメデトミジンがエンドトキシンによって誘発される炎症の程度を抑制することが示されました。

心臓手術(CABGや弁置換術)を受ける予定の60歳以上の患者さん
除外基準:
以下の基準のいずれかを満たす患者は除外されます。
- 参加を拒否する。
- -統合失調症、てんかん、パーキンソン症候群、または重度の認知症の術前病歴;
- 重度の視覚/聴覚機能障害または言語障害により、術前にコミュニケーションができない。
- 脳損傷または脳神経外科の病歴;
- 術前の洞不全症候群、重度の徐脈(心拍数 < 50 bpm)、ペースメーカーなしの第 2 度以上の房室ブロック。
- 重度の肝機能障害 (Child-Pugh クラス C);
- 重度の腎機能障害(腎代替療法が必要)。
- その他参加に不適当と判断される条件。

実験的:デックスグループ
介入薬(注射用塩酸デクスメデトミジン)は、麻酔導入前から術後の人工呼吸終了までの期間投与します。
|
麻酔前に、注射用塩酸デクスメデトミジン(200μg/2ml)を生理食塩水で50mlに希釈する(デクスメデトミジンの最終濃度4μg/ml)。 麻酔導入前に、負荷用量を [0.9*kg] ml/h の速度で 10 分間静脈内注入し (つまり、10 分間でデクスメデトミジン 0.6 μg/kg)、その後一定の速度で持続注入します。手術終了まで [0.1*kg] ml/h (つまり、デクスメデトミジンを 0.4 μg/kg/h の速度で投与)。 手術の終わりに、注入速度は[0.025 * kg] ml/h (つまり、デクスメデトミジンの速度は0.1 ug/kg/h) に減少し、手術後の人工呼吸が終了するまで継続されます。
