谵妄是一种全身性脑功能障碍状态,表现为意识、注意力、认知和知觉的急性障碍。 它在很短的时间内(通常是几小时到几天)发展,并且在一天中往往会波动。 报告的心脏手术后谵妄发生率从 21% 到 47% 不等。 术后谵妄的发生对患者的预后有显着的不利影响,包括发病率增加、住院时间延长、医疗费用增加和死亡率增加。 它的发生还与长期认知能力下降和生活质量下降有关。

纳入标准:
计划接受心脏手术(CABG 和/或瓣膜置换手术)的 60 岁或以上患者
排除标准:
符合以下任一条件的患者将被排除在外:
- 拒绝参加;
- 精神分裂症、癫痫、帕金森综合征或严重痴呆的术前病史;
- 术前因视觉/听觉严重障碍或语言障碍而无法沟通;
- 脑损伤或神经外科手术史;
- 术前病态窦房结综合征、重度心动过缓(HR < 50 bpm)、二度或以上无起搏器的房室传导阻滞;
- 严重肝功能障碍(Child-Pugh C 级);
- 严重肾功能不全(需要肾脏替代治疗);
- 其他认为不适宜参加的情形。

干预药物(注射用盐酸右美托咪定)在麻醉诱导前至术后机械通气结束期间给药。

麻醉前,将注射用盐酸右美托咪定(200ug/2ml)用生理盐水稀释至50ml(右美托咪定终浓度4ug/ml)。 麻醉诱导前,将以 [0.9*kg] ml/h 的速度静脉输注 10 分钟(即右美托咪定 0.6 μg/kg,10 分钟),然后以一定速度连续输注[0.1*kg] 毫升/小时(即右美托咪定以 0.4 微克/千克/小时的速度)直到手术结束。 在手术结束时,输注速率将降低至[0.025*kg]ml/h(即右美托咪定以0.1ug/kg/h的速率)并持续至手术后机械通气结束。
