この研究は、12か月にわたる無作為化、単盲検、前向き、分割口対照臨床試験として設計されています。 この研究の被験者は、UCSF 歯学部から募集されます。 UCSFの博士前期、大学院小児歯科、および大学院歯科矯正クリニックからの対象基準を満たす患者には、研究について説明があり、参加するよう招待されます。
中程度または高いう蝕リスクを持つ若い患者は、研究に参加するよう招待されます。 これまで治療されておらず、虫歯がなく、密閉されていない一対の 2 本の大臼歯が必要です。 患者の利き側(右または左)の臼歯は、試験群または対照群のいずれかにランダムに割り当てられ、対側の非利き側にはもう一方の治療が行われます。 試験歯はCO2レーザー照射とフッ化物ニスで治療されます。対照の歯にはフッ化物ニス治療のみが行われます。
ICDAS II、白色および青色光デジタル写真 (SOPROLIFE)、および DIAGNOdent 評価を使用したベースラインの視覚検査が、治療前に歯科医によって行われます。
患者は 6 か月後と 12 か月後のフォローアップ検査のために再度受けるように求められ、その時点で、最初にベースライン検査を完了した歯科医によってすべての検査方法による視覚検査が実施されます。 各参加者に対する研究のエンドポイントは、ICDAS 評価 (ICDAS コード 3 - 「象牙質関与の臨床的視覚的徴候を伴わない局所的なエナメル質の破壊」) または 12 か月の検査によって対照歯または試験歯のいずれかに重大な脱灰があることが判明したときとなります。 、 いずれか早い方。

包含基準:
- 6歳以上、全般的に健康状態が良好
- CAMBRA によれば、被験者は中等度または高度のう蝕リスクを持っています
- 少なくとも1つの顎に密閉されていない臼歯が少なくとも1対あり、シーラントが必要である
- ICDAS コード 0、1、2 の歯。深い溝と亀裂があり、探索者に解剖学的スティックを提供します。

- 口腔衛生状態が極度に悪いという証拠を示す

実験的:レーザーとフッ素
スプリットマウスデザインでは、口の片側の大臼歯に介入 CO2 9.3 μm 短パルスレーザー治療と咬合裂領域のフッ化物ニス (実験側) が施されます。

CO2 - 9.3μmの短パルスレーザーが咬合面のエナメル質表面に照射されます。
これにより結晶の組成と構造が変化し、酸による溶解に対する歯科用ミネラルの耐性が高まり、12か月にわたるフッ化物単独療法と比較して、生きている歯の咬合面の虫歯をよりよく予防する効果があります。

口が分割されたデザインでは、顎のこの反対側がコントロールとして機能します。
