包含基準:
- -CTまたはMRI基準によって臨床的に確認された脳転移。 頭蓋外転移性疾患の証拠がある場合は、病変が病理学的に確認され(除外された組織型についてはセクション 4.2.5 を参照)、ユタ大学またはハンツマンがん病院の病理学者によって再検討されることが望ましいです。施設外。
- 以前の脳手術は許可されていますが、手術台にある病変はSRS治療の実験的投与を受けるために選択されません. 患者が病変の切除のために開頭術を受けた場合、SRS は手術後 4 ~ 6 週間で提供する必要があります。 4-6 開頭術後のウィンドウ外で SRS を実行することを目標とする患者の登録は、PI の裁量に委ねられています。
- 患者は合計 1 ~ 5 個の未治療の脳転移を持っている必要があります。
- -コホート1a(拡大を含む)またはコホート1aへの登録を計画している患者の場合:CTまたはMRI測定による同等の腫瘍直径≤20 mm 試験での転移病変の相談/スクリーニング時。 -標準治療のSRS投与で治療されたすべての病変のCTまたはMRI測定による等価腫瘍直径</= 20 mm。
- コホート 2 または 3 への登録を計画している患者の場合: CT または MRI 測定による等価腫瘍直径 ≤ 40 mm 試験での転移病変の相談/スクリーニング時。 -標準治療のSRS投与で治療されたすべての病変のCTまたはMRI測定による等価腫瘍直径</= 40 mm。 プロトコル バージョン 9 の時点で、コホート 2 および 3 は永久に発生が停止されています。
- すべての転移性病変は、互いに最も近接している病変の周辺端から測定して、最低 3 cm 離す必要があります。 複数の病変が存在し、すべてが互いに 3 cm 以上離れていない場合、患者は不適格と見なされます。
- BRAF阻害剤を服用している患者(少なくとも7日間)には適切なウォッシュアウトが必要ですが、以前の全身療法は許可されています。
- 現在、積極的な全身がん治療を受けている対象については、担当の腫瘍内科医に相談して、SRS の前に適切なウォッシュアウト (適切な場合) 期間を確保する必要があります。
- 患者は 18 歳以上でなければなりません。
- カルノフスキー パフォーマンス ステータス (KPS) ≥ 60。
- -インフォームドコンセントを提供でき、連邦および機関のガイドラインに準拠した承認済みの同意書に署名している。
- -出産の可能性のある女性は、研究登録から10日以内に妊娠検査で陰性でなければならず、放射線を受けている間および放射線後3か月間、許容される避妊方法を使用することに同意する必要があります。 非出産の可能性のある女性は、外科的に無菌であるか、閉経後 1 年以上経過している場合に含まれる場合があります。

除外基準:
- -以前の全脳/部分脳照射。
- -プロトコル適格性の相談/スクリーニング時のMRI画像での等価直径が40 mmを超える脳病変。

- 脳幹に位置する脳病変。
- 軟髄膜疾患のX線または細胞学的証拠
- 小細胞癌、胚細胞腫瘍、リンパ腫、白血病、または多発性骨髄腫を含む放射線感受性の組織型を伴う原発性病変
- 妊娠中または授乳中の出産の可能性のある女性
- -サイズ基準により、登録時に満員であり、12週間のDLT期間に達していないコホートに登録される複数の病変を有する患者。

実験的:定位放射線治療
|

Maximum Tolerated Dose of stereotactic radiosurgery

To determine the maximum tolerated dose (MTD) of stereotactic radiosurgery (SRS) treatment for patients in the Escalation Cohort (with 1 to 5 brain metastases, who have not received prior whole brain radiotherapy) and patients in the Expansion Cohort (with up to 5 metastases per patient). Maximum Tolerated Dose of stereotactic radiosurgery will be evaluated throughout the treatment period which is expected to last 12-16 weeks per patient Patients will be evaluated for any grade 3 or greater toxicities attributed to the lesion treated with the escalated dose. |
