アデノ扁桃摘出術は、米国で年間 50 万回以上行われており、小児の全身麻酔下で行われる最も一般的な手術です。 手術の大部分は、感染症やその他の適応症ではなく、呼吸の閉塞に対して行われます。
率直な閉塞性睡眠時無呼吸症候群 (OSA) の子供の 7 か月間の神経認知、行動、および健康上の結果を改善する上でのアデノ扁桃摘出術 (AT) の役割は、小児アデノ扁桃摘出術試験 (CHAT) で最近取り上げられました。 この厳格な多施設無作為化対照試験の結果は、注意深い待機と比較してアデノ扁桃摘出術により、行動、生活の質、睡眠呼吸障害 (SDB) 症状、および睡眠ポリグラフのパラメーターが改善されたことを示す非常に重要なデータを提供しました。

以下の基準をすべて満たしていると定義される軽度の睡眠呼吸障害 (MSDB) の診断:
- -ほとんどの夜、少なくとも週に3晩発生し、少なくとも3か月間存在している習慣的ないびきの介護者の報告(平均して週に3日以上発生するか、睡眠時間の半分以上)および
- 閉塞性無呼吸指数(OAI)が 1/時間未満、無呼吸低呼吸指数(AHI)が 3/時間以下、酸素飽和度(SpO2)の低下が 90% 未満で、閉塞性イベントに関連していないことを確認する中央スコアの睡眠ポリグラフ(PSG)。 PSG。
- -標準化されたスケール0〜4に基づく扁桃肥大≧2。
- -耳鼻咽喉科医(ENT)の評価によりATの候補であると見なされます(つまり、粘膜下口蓋裂などの手術の禁忌となる技術的な問題はありません)。
- AT の主な適応症は、夜間の閉塞性症状です (つまり、再発性の感染症やその他の適応症ではありません)。
除外基準:
- 部分扁桃摘出術を含む扁桃摘出術の既往

- 身長または体重のいずれかが年齢と性別の 5 パーセンタイル未満であると定義されます。
