코골이에 대한 소아 Adenotonsillectomy 시험 (PATS)

Adenotonsillectomies는 미국에서 연간 500,000 회 이상 수행되며 어린이의 전신 마취하에 수행되는 가장 일반적인 수술입니다. 대부분의 수술은 감염이나 다른 징후가 아닌 호흡 곤란에 대해 수행됩니다.
솔직한 폐쇄성 수면 무호흡증(OSA)이 있는 어린이의 7개월 신경인지, 행동 및 건강 결과를 개선하는 데 있어 아데노편도 절제술(AT)의 역할은 최근 소아기 선편도 절제술 시험(CHAT)에서 다루어졌습니다. 이 엄격한 다기관 무작위 통제 시험의 결과는 조심스럽게 기다리는 것과 비교하여 아데노편도 절제술이 행동, 삶의 질, 수면 장애 호흡(SDB) 증상 및 수면다원검사 매개변수를 개선한다는 것을 나타내는 매우 중요한 데이터를 제공했습니다.

- 간접흡연, 수면 부족, 사회경제적 지위, 가족 기능과 같은 조절 영향을 평가합니다.
- Childhood Adenotonsillectomy Trial(CHAT)에서 실현 가능하지 않은 장기(12개월) 결과를 조사합니다.

포함 기준:
다음 기준을 모두 충족하는 것으로 정의되는 경미한 수면 장애 호흡(MSDB)의 진단:
- 주당 최소 3일 밤 대부분의 밤에 발생하고 최소 3개월 동안 지속되는 습관성 코골이에 대한 간병인 보고(평균 발생 > 주당 3일 또는 수면 시간의 절반 이상) 및
- 폐쇄성 사건과 관련하여 폐쇄성 무호흡 지수(OAI) <1/시간 및 무호흡-저호흡 지수(AHI) ≤3/시간 및 무산소 포화도(SpO2) 불포화도 < 90%를 확인하는 중앙 점수 수면다원검사(PSG), PSG.
- 0-4의 표준화된 척도를 기준으로 편도선 비대 ≥2.
- 이비인후과 의사(ENT) 평가에 의해 AT 후보로 간주됨(즉, 점막하 구개열과 같은 수술에 대한 금기 사항이 될 기술적 문제 없음)
- AT의 주요 적응증은 야간 폐쇄 증상(즉, 재발성 감염 또는 다른 적응증이 아님)입니다.
제외 기준:
- 부분 편도선 절제술을 포함한 이전 편도선 절제술
- American Academy of Otolaryngology-Head and Neck Surgery Clinical Practice Guidelines(즉, 작년에 연간 7회 이상, 지난 2년 동안 연간 5회 이상 또는 3회 이상)에 따라 즉각적인 편도선 절제술(AT)이 필요한 재발성 편도염 지난 3년 동안 /년.)
- 심한 비만(체질량 지수(BMI) z-점수 ≥3).
- 신장 또는 체중이 연령 및 성별에 대해 5번째 백분위수 미만인 것으로 정의되는 성장 실패.
참여를 방해하거나 다음을 포함하되 이에 국한되지 않는 연구의 주요 변수를 혼란스럽게 할 수 있는 심각한 만성 건강 상태:
- 낭포성 섬유증, 선천성 심장병과 같은 중증 심폐 질환.
- 출혈 장애
- 겸상 적혈구 질환
- 약물 치료가 필요한 간질
- 비지속성 심실성 빈맥, 심방 세동, 2도 방실 차단, 지속성 서맥 또는 지속성 빈맥을 포함하여 PSG에서 유의한 심장 부정맥이 확인되었습니다.
- 당뇨병, 기면증, 잘 조절되지 않는 천식과 같은 기타 심각한 만성 건강 문제.
- 기도, 인지 또는 행동에 영향을 줄 수 있는 알려진 유전적, 두개안면, 신경학적 또는 정신 질환
- 향정신성 약물(주의력 결핍 과잉 행동 장애, 수면제, 항고혈압제, 인슐린을 포함한 혈당 강하제, 항경련제, 항응고제 또는 성장 호르몬을 위한 약물 제외)의 현재 사용.
- 자폐 스펙트럼 장애의 진단.
- 지적 장애가 있거나 모든 학과목에 대해 독립적인 교실에 배정됨.
- 심각한 발달 장애의 병력 또는 적응형 행동 평가 시스템(ABAS-3) 점수 ≤60.
- 올해 안에 해당 지역에서 이사할 계획인 어린이/간병인.
- 위탁 보호 아동.
- 신경 행동 측정을 완료할 만큼 영어나 스페인어를 잘 구사하지 못하는 어린이/간병인.
