この研究の目的は、再発のない持続的なステロイド/経腸栄養剤のない1年間の寛解を維持するための毎週の皮下投与メトトレキサートの有効性を以下と比較することです。
- 低リスクの小児クローン病における毎日の経口アザチオプリン/6メルカプトプリン
- ハイリスク小児クローン病におけるアダリムマブの皮下投与

詳細な説明
この無作為化比較試験では、PIBDNet (小児炎症性腸疾患ネットワーク) は、患者を積極的な疾患進行の 2 つのリスクグループに分類することにより、次の治療戦略を比較することを目的としています。侵襲性疾患のリスクが低い小児および高リスク群におけるメトトレキサートとアダリムマブの有効性。
PIBDNet は、低リスク層のクローン病の寛解維持に関してメトトレキサートがアザチオプリン/6 メルカプトプリンよりも優れており、高リスク層ではアダリムマブがメトトレキサートよりも優れているという仮説を立てています。
さらに、補助研究では、アダリムマブ治療を受けた患者を組み入れた場合 (TOP-Down) と、免疫調節療法の失敗によりアダリムマブに切り替えた患者 (STEP-Up) を分析する予定です。

- 確立された基準(37、38)を使用して診断された新たに発症したクローン病の6〜17歳の子供、ステロイドベースまたは経腸栄養ベースの導入療法が必要
- 初診時wPCDAI>40またはCRP>診断時上限2倍
- すべての wPCDAI スコア (0-120) が含まれる可能性があります (寛解期の患者および活動性疾患の患者)
- B2および/またはB3疾患行動の有無にかかわらず、管腔活動性クローン病(B1)
- 5-ASA および誘導体への初期曝露は許容されます
- 抗生物質への曝露は許容されます
次の基準のいずれかが存在する場合、患者は無作為化の前に高リスク群に割り当てられます。
- 複合瘻孔性肛門周囲疾患
- -汎腸管疾患の表現型(パリ分類ごとのL4bを伴うL3または十二指腸、胃または食道に深い潰瘍を伴うL3として定義される(HP(ヘリコバクターピロリ)またはNSAID関連ではない))
- CDに関連する可能性が高い重度の成長障害(高さのZスコアが-2未満または2パーセンタイル以上)
- -重大な低アルブミン血症(<30g / l)、C反応性タンパク質(CRP)の上昇(正常範囲の少なくとも2倍)、またはステロイドまたは排他的経腸栄養による3週間の最適化導入療法にもかかわらずwPCDAI>12.5
- B2、B3、または B2B3 の疾患行動
- -全体の累積疾患範囲が60cm以上
- 通知および署名済みの同意
除外基準:
- -CRPが上限の2倍を超える場合を除き、初期診断時にwPCDAIが42.5未満の患者
- ステロイドまたは経腸栄養による寛解導入療法なし
- -IBD(炎症性腸疾患)による以前の治療-このプロトコルで詳述されている導入療法以外の関連薬(5-ASAを除く)。
- -思春期患者(男の子と女の子の両方)の研究期間中の妊娠または避妊薬の使用の拒否 各研究訪問で絶対禁欲(性的活動なし)が確認されない限り。 研究を通して陽性の妊娠検査は、研究からの患者の迅速な撤退を引き起こします。
- 授乳中の母親
- 外科的治療(ドレナージ、シートン留置)が必要な肛門周囲瘻孔症の小児
- -チオプリンメチルトランスフェラーゼのホモ接合体、またはチオプリンメチルトランスフェラーゼ活性が赤血球6 nmol/h/ml未満または6MTG(6メチルチオグアニン/g Hb/h)が9nmol未満の患者。
- 排膿されておらず制御されていない膿瘍/痰の証拠
- -試験で使用される薬物に対する禁忌(不耐性/過敏症またはいずれかの治験薬に対するアレルギーを含む(チオプリン、メトトレキサートまたはアダリムマブ))
- 現在または以前の悪性腫瘍
- 重篤な併存症(腎不全、肝炎、呼吸不全など)が薬物療法や転帰パラメーターの解釈を妨げたり、患者が治験を終了する可能性を低くしたりする可能性があります。
- 結核菌による感染症
- 中等度から重度の心不全 (NYHA クラス III/IV)
- 経口抗凝固療法、抗マラリア療法
- -生ワクチンへの曝露(黄熱病を含む)が含まれる前に3週間未満

|
メトトレキサートの皮下投与は、週 1 回 15mg/m2 体表面積 (19, 30)、最大用量は 25mg/週。
オダンセトロン (ゾフラン) の前投薬 (注射の 1 時間前に 4 ~ 8 mg) が推奨され、葉酸置換 (15 mg po、メトトレキサート注射の 3 日後、小児用)
