慢性神経障害性疼痛および薬剤耐性疼痛治療における反復磁気刺激と直流刺激の評価
神経因性疼痛の有病率は、一般集団で 6.9% に上昇し、脊髄の病変を有する患者では 58% 以上に達する可能性があります。 この病理学的状態は、特に現在利用可能な治療法の有効性が限られているため、主要な健康問題のままです。 薬理学的アプローチによって、患者の 30 ~ 40% だけが痛みの 50% 以上を軽減します。
失敗した場合、薬物治療またはこれらの治療に加えて、運動皮質の刺激は 1990 年代の初めから坪川によって提案された治療経路ですが、神経障害の薬剤耐性疼痛管理のための場所を見つけたのは 10 年後のことです。 運動皮質刺激の鎮痛効果の神経生理学的メカニズムはまだほとんど理解されていません。 この刺激は、埋め込まれた電極を使用して慢性的かつ侵襲的な方法で実現できます。 このプロセスにより、この技術に反応する可能性のある患者にとって信頼できる臨床選択基準を特定する可能性がなくても、手術を受けた人の約半数が永続的に軽減されます。
最近、反復経頭蓋磁気刺激 (rTMS) と直流経頭蓋磁気刺激 (tDCS) の 2 つの電気生理学的非侵襲的技術が開発され、運動皮質の鎮痛刺激を得ることができます。
この研究の主な目的は、慢性的な薬剤耐性神経生理学的疼痛におけるtDCSの鎮痛効果の重要性を、運動皮質の非侵襲的刺激の参照方法のおかげで得られるものと比較することであり、その鎮痛効果はrTMSによってすでに検証されています文献データです。

包含基準:
- 国家社会保障に関係する人。
- 署名された自由な同意を与えた患者。
- 18 歳から 80 歳までの患者、男性または女性、慢性神経因性疼痛の保因者 (1 年以上 ; EVN > 2 S0 週中) 薬剤耐性、片側性、少なくとも上半身および/または半顔面を含み、その鎮痛剤治療は少なくとも1か月から安定しています。
- 薬剤耐性のある患者は、対象のアルゴロジスト (疼痛薬の専門家) に、非薬理学的な代替治療法を提案し、手術前の検査を開始する可能性を検討させます。
除外基準:
- 薬物中毒、片頭痛、てんかんの病歴。
- 強磁性の頭蓋内装置または刺激装置(ペースメーカー、大脳基底核の刺激、迷走神経刺激装置)の存在。
- 少なくとも 1 か月前からの新しい鎮痛治療の導入。
- 妊娠中、出産中、または授乳中の女性。 妊娠可能年齢の患者に対するすべての試験中、効果的な避妊が行われていないこと。
- 司法または行政の決定により自由を奪われた者、法的保護下にある者。
- MRIへの反論。
