ただし、ロボット歩行トレーニングだけでは不十分な場合があります。
この研究では、ロボット歩行トレーニング単独の効果と、低周波または高周波の非侵襲的経脊髄電気刺激を組み合わせたロボット歩行トレーニングの効果を比較します。
運動不全の脊髄損傷者では、刺激ありまたは刺激なしの歩行訓練の 20 セッションの前後に、一連の神経機能の臨床的および電気的検査が実施されます。

脊髄損傷 (SCI) の患者は、運動機能障害を抱えており、その結果、社会的、個人的、および経済的にかなりのコストがかかります。
ロボット歩行トレーニングは、これらの個人の歩行能力を向上させる目的でよく使用されます。
研究者は最近、ロボットの歩行トレーニングが脊髄神経回路を再編成し、運動活動を改善し、運動不全の SCI を持つ人々の歩行能力の回復に大きく貢献することを報告しました。
ただし、アシストステッピング中の病的な筋緊張と異常な筋活動パターンは、ロボット歩行トレーニングの複数のセッションの後でも明らかでした。
したがって、運動訓練だけでは、脳と脊髄をつなぐ弱い神経シナプスを強化したり、脊髄の神経回路を完全に最適化するには不十分である可能性があります。
一方、脊髄刺激は、脊髄化した動物の軸索と樹状突起の発芽と可塑性を増加させます。
さらに、脊髄損傷者の経皮的脊髄刺激(ここでは経脊髄刺激と呼びます)は、重力が除去されたときにリズミカルな脚の筋肉活動を引き起こす可能性があります。
機能的神経可塑性の誘導と脚の運動機能の回復に関する基本的な知識のギャップがまだ存在する.
この研究の中心的な作業仮説は、BWS アシスト ステップ中に配信される経脊髄刺激が、脊髄の全体的な応答性を高め、運動出力を改善する緊張性興奮性入力を提供するというものです。
研究者は 3 つの具体的な目的に取り組みます: 低周波数 (0.3 Hz; 特定の目的 1) および高周波数 (0.3 Hz; 特定の目的 1) で BWS アシスト ステッピング中に経脊髄刺激が供給されると、運動不全 SCI を持つ人々の神経可塑性の誘導と脚の感覚運動機能の改善を確立します ( 30 Hz; 特定の目的 2)、および経脊髄刺激なしで BWS アシスト ステップ トレーニングを行った場合 (特定の目的 3)。
すべてのグループで、20 セッション後の結果は、最先端の神経生理学的方法で測定されます。
皮質脊髄回路の興奮性は、着座した被験者の経頭蓋磁気刺激運動誘発電位を介して測定されます(目的1A、2A、3A)。
ヒラメ筋の H 反射および前脛骨筋反射の興奮性パターンは、補助ステッピング中に測定されます (目的 1B、2B、3B)。
感覚運動機能は、歩行と筋力の標準化された臨床試験によって評価されます (目標 1C、2C、3C)。
さらに、調整された筋肉活性化のポリ筋電図解析が詳細に測定されます。
アシストステッピング中の 30 Hz での経脊髄刺激は、脚の運動機能を改善し、0.3 Hz と比較して足首の痙性をさらに減少させるという仮説が立てられています。
さらに、30 Hz での経脊髄刺激は、運動不全 SCI の患者の足踏み中に一般的に観察される異常な位相依存性ヒラメ筋 H 反射および屈筋反射変調を正常化すると仮定されています。
プロジェクトの仮説をテストするために、45 名の運動不全 SCI 患者が無作為に割り当てられ、BWS 補助ステッピング中に 0.3 または 30 Hz で 20 セッションの経脊髄刺激を受けるか、経脊髄刺激なしの BWS 補助ステッピング 20 セッションを受ける (1 グループあたり 15 被験者)。 .
この研究プロジェクトの結果は、脊髄研究の分野を大幅に進歩させ、治療の基準を変えるでしょう。なぜなら、人間の運動不完全SCI後の脚の運動機能を改善するための、斬新で効果的なリハビリテーション戦略の開発に大きな可能性があるからです。

- SCI は胸椎 12 椎骨の上にあります。
