Activiteitsafhankelijke transspinale stimulatie voor herstel van loopvermogen na ruggenmergletsel
Robotische looptraining wordt vaak gebruikt met als doel het loopvermogen van mensen met een dwarslaesie te verbeteren.
Het is echter mogelijk dat robotische looptraining alleen niet voldoende is.
Deze studie zal de effecten van robotlooptraining alleen vergelijken met robotlooptraining in combinatie met laagfrequente of hoogfrequente niet-invasieve transspinale elektrische stimulatie.
Bij mensen met motorisch onvolledige dwarslaesie zal een reeks klinische en elektrische tests van de zenuwfunctie worden uitgevoerd voor en na 20 sessies looptraining met of zonder stimulatie.

Mensen met een dwarslaesie (SCI) hebben motorische stoornissen die resulteren in aanzienlijke sociale, persoonlijke en economische kosten.
Robotische looptraining wordt vaak gebruikt met als doel het loopvermogen van deze personen te verbeteren.
Onderzoekers meldden onlangs dat looptraining met robots de spinale neuronale circuits reorganiseert, de motorische activiteit verbetert en substantieel bijdraagt aan het herstel van het loopvermogen bij mensen met motorische onvolledige dwarslaesie.
Pathologische spiertonus en abnormale spieractivatiepatronen tijdens geassisteerde stappen waren echter nog steeds duidelijk na meerdere sessies robotlooptraining.
Locomotorische training alleen kan dus onvoldoende zijn om zwakke neuronale synapsen te versterken die de hersenen verbinden met het ruggenmerg of om spinale neurale circuits volledig te optimaliseren.
Aan de andere kant verhoogt stimulatie van het ruggenmerg het ontspruiten en de plasticiteit van axonen en dendrieten bij dieren met ruggengraat.
Bovendien kan transcutane stimulatie van het ruggenmerg (hier transspinale stimulatie genoemd) bij mensen met dwarslaesie ritmische beenspieractiviteit oproepen wanneer de zwaartekracht wordt geëlimineerd.
Er bestaat nog steeds een fundamentele kennislacune over de inductie van functionele neuroplasticiteit en het herstel van de beenmotorische functie na herhaalde thoracolumbale transspinale stimulatie tijdens door het lichaamsgewicht ondersteunde stappen bij mensen met een dwarslaesie.
De centrale werkhypothese in deze studie is dat transspinale stimulatie die wordt afgegeven tijdens BWS-geassisteerde stepping een tonische prikkelende input levert die de algehele responsiviteit van het ruggenmerg verhoogt en de motoroutput verbetert.
De onderzoekers zullen zich richten op 3 specifieke doelen: vaststellen van inductie van neuroplasticiteit en verbeteringen in de sensomotorische functie van de benen bij mensen met motorische onvolledige dwarslaesie wanneer transspinale stimulatie wordt toegediend tijdens BWS-geassisteerde stappen bij lage frequenties (0,3 Hz; specifiek doel 1) en bij hoge frequenties ( 30 Hz; Specifiek doel 2), en wanneer BWS-ondersteunde staptraining wordt gegeven zonder transspinale stimulatie (Specifiek doel 3).
In alle groepen worden de resultaten na 20 sessies gemeten via geavanceerde neurofysiologische methoden.
De exciteerbaarheid van het corticospinale circuit zal worden gemeten via transcraniële magnetische stimulatiemotor evoked potentials bij zittende proefpersonen (doelen 1A, 2A, 3A).
Soleus H-reflex en tibialis anterieure flexorreflex prikkelbaarheidspatronen zullen worden gemeten tijdens geassisteerde stappen (doelen 1B, 2B, 3B).
De sensomotorische functie zal worden geëvalueerd via gestandaardiseerde klinische tests van gang en kracht (doelstellingen 1C, 2C, 3C).
Daarnaast zal poly-elektromyografische analyse van gecoördineerde spieractivatie in detail worden gemeten.
Er wordt verondersteld dat transspinale stimulatie bij 30 Hz tijdens geassisteerde stappen de beenmotoriek verbetert en de spasticiteit van de enkel meer vermindert in vergelijking met 0,3 Hz.
Verder wordt verondersteld dat transspinale stimulatie bij 30 Hz de abnormale fase-afhankelijke soleus H-reflex- en flexorreflexmodulatie normaliseert die vaak wordt waargenomen tijdens het stappen bij mensen met motorische onvolledige dwarslaesie.
Om de projecthypothesen te testen, zullen 45 mensen met motorische onvolledige dwarslaesie willekeurig worden toegewezen om 20 sessies van transspinale stimulatie te ontvangen bij 0,3 of 30 Hz tijdens BWS-geassisteerde stappen of 20 sessies van BWS-geassisteerde stappen zonder transspinale stimulatie (15 proefpersonen per groep) .
De resultaten van dit onderzoeksproject zullen het gebied van het ruggenmergonderzoek aanzienlijk vooruit helpen en de zorgstandaard veranderen, omdat er een groot potentieel is voor de ontwikkeling van nieuwe en effectieve revalidatiestrategieën om de beenmotorische functie te verbeteren na motorische onvolledige dwarslaesie bij mensen.

Inclusiecriteria:
- Klinische diagnose van motorisch onvolledig ruggenmergletsel (SCI).
- SCI is boven thoracale 12 wervel.
- Afwezigheid van permanente enkelgewrichtscontracturen.
- SCI vond plaats 6 maanden vóór inschrijving voor het onderzoek.
Uitsluitingscriteria:
- Supraspinale laesies
- Neuropathieën van het perifere zenuwstelsel
- Degeneratieve neurologische aandoeningen van de wervelkolom of het ruggenmerg
- Motor compleet SCI
- Aanwezigheid van doorligwonden
- Urineweginfectie
- Neoplastische of vasculaire aandoeningen van de wervelkolom of het ruggenmerg
- Zwangere vrouwen of vrouwen die vermoeden dat ze zwanger zijn of kunnen worden.
- Mensen met cochleaire implantaten, pacemakers en geïmplanteerde stimulatoren
- Mensen met een voorgeschiedenis van toevallen
- Mensen met een geïmplanteerde baclofenpomp

Plasticiteit van corticale en corticospinale neuronale circuits

Neurofysiologische tests die de corticale en corticospinale prikkelbaarheid onderzoeken, zullen voor en na de ingreep worden gemeten.
Transcraniële magnetische stimulatie (TMS) met één puls zal worden gebruikt om de rekruteringscurve van door motor opgewekte potentialen samen te stellen, en TMS met gepaarde puls zal worden gebruikt om veranderingen in corticale remmende en faciliterende neuronale circuits te onderzoeken.

Neurofysiologische tests die de exciteerbaarheid van de spinale reflex onderzoeken, zullen voor en na elke interventie worden gemeten door stimulatie van de achterste tibiale en surale zenuwen tijdens Lokomat-geassisteerde stepping die de amplitudemodulatie van de soleus H-reflex en tibialis anterieure flexorreflex weergeven.
