SCI에서 활동 의존적 경척추 자극

로봇 보행 훈련은 척수 손상이 있는 개인의 보행 능력을 향상시키기 위한 목적으로 자주 사용됩니다.
그러나 로봇 보행 훈련만으로는 충분하지 않을 수 있습니다.
이 연구는 로봇 보행 훈련 단독과 저주파 또는 고주파 비침습 경척추 전기 자극과 결합된 로봇 보행 훈련의 효과를 비교할 것입니다.
운동 불완전 척수손상 환자의 경우, 자극 유무에 관계없이 20회의 보행 훈련 전후에 신경 기능에 대한 일련의 임상 및 전기 검사를 실시합니다.

척수 손상(SCI) 환자는 상당한 사회적, 개인적, 경제적 비용을 초래하는 운동 기능 장애가 있습니다.
로봇 보행 훈련은 종종 이러한 개인의 보행 능력을 향상시키기 위해 사용됩니다.
연구자들은 최근 로봇 보행 훈련이 척추 신경 회로를 재구성하고, 운동 활동을 개선하며, 운동 불완전 척수손상 환자의 보행 능력 회복에 실질적으로 기여한다고 보고했습니다.
그러나 로봇 보행 훈련의 여러 세션 후에도 보조 보행 중 병리학적 근육 긴장도 및 비정상적인 근육 활성화 패턴이 여전히 분명했습니다.
따라서 운동 훈련만으로는 뇌와 척수를 연결하는 약한 신경 시냅스를 강화하거나 척추 신경 회로를 완전히 최적화하기에는 불충분할 수 있습니다.
한편, 척수 자극은 척수화 동물에서 축삭과 수상돌기의 발아와 가소성을 증가시킵니다.
또한 SCI 환자의 경피적 척수 자극(여기서는 경척수 자극이라고 함)은 중력이 제거될 때 리드미컬한 다리 근육 활동을 유발할 수 있습니다.
SCI 환자의 체중 지원(BWS) 지원 스테핑 동안 반복적인 흉요추 경척추 자극 후 기능적 신경가소성 유도 및 다리 운동 기능 회복에 대한 근본적인 지식 격차가 여전히 존재합니다.
이 연구의 중심 작업 가설은 BWS 보조 스테핑 중에 전달된 경척추 자극이 척수의 전반적인 반응성을 증가시키고 운동 출력을 향상시키는 긴장성 흥분성 입력을 제공한다는 것입니다.
연구자들은 3가지 특정 목표를 다룰 것입니다: 저주파수(0.3Hz; 특정 목표 1) 및 고주파수에서 BWS 보조 스테핑 동안 경척추 자극이 전달될 때 운동 불완전 척수손상 환자의 신경가소성 유도 및 다리 감각 운동 기능 개선( 30Hz, 특정 목표 2), 그리고 BWS 보조 스텝 트레이닝이 경척추 자극 없이 시행되는 경우(특정 목표 3).
모든 그룹에서 20회 세션 후 결과는 최첨단 신경생리학적 방법을 통해 측정됩니다.

보조 보행 중 30Hz에서 경척추 자극이 0.3Hz에 비해 다리 운동 기능을 개선하고 발목 경직을 더 감소시킨다는 가설이 있습니다.
또한 30Hz에서의 경척수 자극은 운동 불완전 SCI를 가진 사람들이 걷는 동안 일반적으로 관찰되는 비정상적인 위상 의존 가자미근 H-반사 및 굴근 반사 변조를 정상화한다는 가설을 세웁니다.
프로젝트 가설을 테스트하기 위해, 운동 불완전 SCI를 가진 45명의 사람들이 BWS 보조 스테핑 동안 0.3 또는 30Hz에서 경척수 자극의 20회 세션 또는 경척수 자극 없이 BWS 보조 스테핑의 20회 세션을 받도록 무작위로 배정됩니다(그룹당 15명의 피험자). .
