継続的な抗凝固療法の必要性は、RRT モダリティの潜在的な欠点を表しています。
急性腎障害 (AKI) に関する KDIGO 2012 ガイドラインは、クエン酸塩の禁忌がない患者における継続的 RRT (CRRT) の第一選択抗凝固療法として RCA を採用することを提案しています。
クエン酸は SLED の抗凝固剤としても導入されており、主に拡散性の長期断続的なモダリティのコンテキストでの使用は、ACD-A などの濃縮クエン酸溶液を使用する場合に体外回路を維持するための簡単で安全な方法であることが示されています。
低リン血症は RRT の既知の問題であり、標準的な RRT 溶液が使用されている場合、特に高用量の RRT が投与される連続的/長期の間欠的なモダリティで、最大 50 ~ 80% の症例で報告されています。
この臨床環境では、関連する低リン血症に関連する合併症があるため、重症患者では RRT 関連のリン酸塩枯渇を避ける必要があります。
