Vereenvoudigde regionale citraat-antistollingsprotocollen voor CVVH, CVVHDF en SLED gericht op de preventie van RRT-gerelateerde hypofosfatemie en optimalisatie van de zuur-base-balans: een pilotstudie
Het doel van de studie is: 1) Om het optreden van zuur-base-veranderingen en de incidentie van hypofosfatemie te evalueren tijdens verschillende modaliteiten van Renal Replacement Terapy (RRT) bij ernstig zieke patiënten [CVVH, CVVHDF en SLED (Sustained Low-Efficiency Dialysis) ] door gebruik te maken van een vereenvoudigd Regionaal Citraat Anticoagulatie (RCA) protocol in combinatie met het gebruik van een fosfaathoudende oplossing als dialysaat en/of vervangingsvloeistof; 2) Optimalisatie van de infusiesnelheid van verschillende oplossingen, inclusief citraat, om een geschikte elektrolyt- en buffervoorraad te verkrijgen.
Het uiteindelijke doel van deze aanpak is om de behoefte aan frequente controle van de zuur-basestatus en elektrolyten (met speciale aandacht voor geïoniseerd calciumgehalte) te verminderen en om de noodzaak van frequente aanpassingen van RCA-RRT-parameters (infusiesnelheid van verschillende oplossingen, suppletie van elektrolyten tijdens RRT).
Deze aanpak zou het mogelijk kunnen maken om antistollingsprotocollen met citraat te vereenvoudigen, om potentiële zorgen die een bredere verspreiding van RCA in de dagelijkse praktijk belemmeren, te minimaliseren.

De noodzaak van continue antistolling vertegenwoordigt een mogelijk nadeel van RRT-modaliteiten.
KDIGO 2012-richtlijnen over acuut nierletsel (AKI) suggereren de goedkeuring van RCA als eerste keus antistollingsmodaliteit voor continue RRT (CRRT) bij patiënten zonder contra-indicaties voor citraat.
Citraat is ook geïntroduceerd als antistollingsmiddel voor SLED en het is aangetoond dat het gebruik ervan in de context van een voornamelijk diffusieve langdurige intermitterende modaliteit een gemakkelijke en veilige methode is om het extracorporale circuit in stand te houden wanneer geconcentreerde citraatoplossingen, zoals ACD-A, worden gebruikt.
Hypofosfatemie is een bekend probleem van RRT en is gemeld in tot 50-80% van de gevallen wanneer standaard RRT-oplossingen worden gebruikt, vooral bij continue/langdurige intermitterende modaliteiten wanneer hoge RRT-doses worden toegediend.
RRT-gerelateerde fosfaatdepletie moet worden vermeden bij ernstig zieke patiënten vanwege de relevante hypofosfatemie-geassocieerde complicaties in deze klinische setting.
Het gebruik van fosfaathoudende CRRT-oplossingen zou de incidentie van hypofosfatemie kunnen verminderen en de behoefte aan parenterale fosforsuppletie tot een minimum kunnen beperken. Het gebruik van een in de handel verkrijgbare fosfaathoudende CRRT-oplossing, in de setting van RCA-RRT, kan het mogelijk maken om de dubbele doelstelling van RRT-geïnduceerde hypofosfatemie minimaliseren en zorgen voor een voldoende levensduur van het circuit, waarbij ook elektrolyt- en zuur-base-verstoringen worden vermeden.
