この研究の主な目的は、ターゲットを絞った速度と注意力のトレーニング プログラム (INHANCE) の影響を評価して、健康な高齢者の認知を改善することです。 -放出断層撮影 (PET)。
治験責任医師は、処理速度と注意力を訓練するコンピューター化されたプログラムである INHANCE を使用した治療グループで構成される二重盲検、平行アーム、プラセボ対照、無作為化臨床試験デザインを採用します。 92人の健康な高齢者。
約 108 人の参加者は、92 人の参加者 (20% の離脱後) の成功を確実にするために同意されます。 その後、参加者はスクリーニング (V0) 評価を完了して、適格性を判断します。 参加後、参加者はベースライン (V1) 評価、PET イメージング、および構造 MRI スキャンを完了します。その後、参加者は INHANCE トレーニングまたはコンピューター ゲームのいずれかに無作為に割り付けられ、10 週間の介入期間中、約 35 時間のプログラム使用に従事します。 介入期間 (~ 5 週間) の約半分で、参加者は中間 (V2) 評価を完了し、介入後の評価と比較します。 10週間の介入に続いて、参加者は介入後(V3)評価とPETイメージングを完了して、認知機能の変化を評価します。 その後、参加者は割り当てられたプログラムの使用を 3 か月間停止し、プログラムをさらに使用しない場合の認知機能の変化の持続性を評価するために、フォローアップ (V4) 研究終了評価に戻ります。

- -潜在的な参加者は、研究スクリーニングの時点で65歳以上でなければなりません。
- 潜在的な参加者は、モントリオール認知評価 (MoCA) で 23 以上のスコアを達成する必要があります。

除外基準:
- -潜在的な参加者は、スクリーニング時に主要または軽度の神経認知障害の既存の診断を受けるべきではありません。
- -コロンビア自殺重症度評価尺度(C-SSRS)によって測定された、特定の計画と意図を伴う積極的な自殺念慮または同意から2か月以内の自殺関連行動を持つ潜在的な参加者
- 潜在的な参加者は、老年うつ病尺度 (GDS) スコアが 10 を超えてはなりません。
- -潜在的な参加者は、同意日から5年以内に、Posit Scienceによって製造されたコンピューターベースの認知トレーニングプログラムで治療されてはなりません。

- 潜在的な参加者は、薬物乱用/依存障害、コリン作動性機能を妨げる薬物、進行中の化学療法またはその他の癌治療を含む、研究活動への参加を妨げると見なされる医学的疾患および/または認知に影響を与える可能性のある不安定および/または未治療の状態にあるべきではありません。
- 現在の薬物乱用(アルコールおよび/または違法薬物を含む)による中毒の兆候を示す潜在的な参加者は除外されます。

Change Mean in EXAMINER Executive Composite Score

The EXAMINER (Executive Abilities: Measures and Instruments for Neurobehavioral Evaluation and Research) is a computer-based battery of executive function tests yielding a single Executive Composite score across three subtests (Flanker, Anti-Saccades, Set-Shifting).
The Executive Composite is a latent trait estimate using Empirical Bayes scoring from item-response theory and is therefore unitless and unbounded.
For reference, validation studies report mean scores of 0.70 in healthy adults, 0.29 in patients with mild cognitive impairment, and -0.20 in patients with Alzheimer's disease.
This outcome reports change in Executive Composite Score from baseline, calculated as the score at 3 months minus baseline and the score at 6 months minus baseline.
Higher values indicate better executive functioning.

This is a computerized assessment that serves as a positive control for task learning.
Raw scores are defined as the exposure duration at approximately 80% criterion accuracy in milliseconds accuracy in identification of central and peripheral targets.
The best possible raw score is 32 milliseconds, and the worst possible score is 3162 milliseconds.
Change was calculated as the score at 3 months minus the score at baseline, and as the score at 6 months minus the score at baseline.
Lower values in milliseconds reflect better Useful Field of View performance.

Change Mean Target Frequency Level in Tonic and Phasic Alertness (TAPAT) Assessment

This is a computerized assessment that serves as a positive control for task learning.
A single target frequency level required for users to achieve at least 80% accuracy in target and foil identification for a pre-specified target image presented among similar foil images.
Target frequency level is recorded as an integer ranging from 1 to 7, where 1 corresponded to the easiest condition (40% target frequency among foils) and 7 represented the most challenging condition (10% target frequency among foils).
Change was calculated as the score at 3 months minus the score at baseline, and as the score at 6 months minus the score at baseline.
Higher scores indicate better performance.
