背景: 乳がんは、世界中の女性とシンガポールで最も一般的ながんです。 病気そのものに加えて、手術を受けることも患者にとってストレスの多い出来事です。 それらの多くは、身体的および心理社会的な問題を抱えています。 さまざまな心理社会的介入が開発されており、モバイル技術を使用した介入により、患者/生存者の健康転帰を改善しています。 ただし、このグループの患者向けの包括的な周術期ソリューションを開発したスマートフォンベースのソリューションはありません。
目的: この研究の目的は、(1) 乳がん手術 (iCareBreast) を受ける女性のための革新的なケア改善スマートフォン ベースの周術期ソリューションを開発することです。 (2) 参加者の自己効力感 (主要な結果)、不安と抑うつ、痛みと疲労、生活の質、および周術期ケアの満足度に関する iCareBreast の有効性を調べます。 (3) iCareBreast の長所と短所に関する参加者の認識を調査します。
リサーチクエスチョン:

仮説 対照群の参加者と比較して、定期的なケアと iCareBreast を受けた介入群の参加者は、
- より高いレベルの自己効力感、
- より低いレベルの不安とうつ病、
- より低いレベルの痛みと疲労、
- より高いレベルの生活の質、および
- 周術期ケアの満足度が向上します。
方法: 2 つのグループの事前および事後テストの無作為化比較試験が実施されます。 乳がんと診断され、乳房手術が必要な合計112人の適格な女性が、シンガポールの公立三次病院の乳房部門から募集されます。 参加者は、介入グループ (病院と iCareBreast によって提供される定期的なケアを受ける) または対照グループ (病院によって提供される定期的なケアのみを受ける) のいずれかにランダムに割り当てられます。 自己効力感、不安と抑うつ、痛みと疲労、生活の質、および周術期ケアの満足度の結果は、2 つの時点 (ベースラインと事後テスト) で、関連する有効で信頼できる手段によって測定されます。 介入グループの約 15 人の参加者は、iCareBreast に関する意見を調査するために、半構造化インタビューに参加するよう招待されます。 定量データはSPSSで分析し、定性データは含有量分析で分析します。
研究の意義:この研究は、乳がん手術を受ける女性が病気に関する教育、身体的、心理的、社会的サポートを受けるためのプラットフォームを提供するだけでなく、医療従事者と患者の間の相互作用を可能にするiCareBreastを開発します。 この研究は、参加者の前述の健康転帰を改善する iCareBreast の予備的効果を生成し、患者ケアを改善するための将来の臨床診療の変更を導くために使用されます。 モバイルヘルスは、患者の関与を強化し、医療提供者が回復の進行状況を監視できるようにするための低コストのコミュニケーションツールとして有望です. この研究は、他の外科的ニーズを持つ患者に同様のソリューションを実装するためのさらなる研究の必要性も通知します.

- 精神疾患または認知機能障害に苦しんでいる;
- 前年のアルコールまたは薬物乱用;
- 医療記録から特定された不安障害およびその他の気分障害;と
- 過去6か月以内に死別期間にあった。

実験的:介入グループ: iCareBreast と定期的なケア
介入グループの参加者は、病院が提供する定期的なケア (対照グループと同じ) に加えて、i) 手術前の教育と指示を提供する iCareBreast モバイル アプリを受け取ります。 ii) 手術後の教育、指示、および回復計画。 iii) ポジティブな心理的サポート; iv) 社会的支援。
合計介入期間は 29 日間(手術前 14 日、手術日、手術後 14 日)です。

ICareBreast BuddyCare モバイル アプリは、患者向けのインタラクティブな手術準備および回復アプリです。
これは、患者中心のケア調整および患者エンゲージメント モバイル アプリであり、患者の周術期の旅をサポートします。
このアプリは、手術の 2 週間前から手術の 2 週間後まで、周術期の患者への日々の情報を提供します。
一方、医療専門家は患者のアプリの使用状況を監視できます。
つまり、iCareBreast アプリは、効率的でユーザーフレンドリーなインタラクティブなタイムラインを介して、手術関連の手順と治療に関するすべての情報を患者に提供します。

回答の範囲は、0 (問題なし) から 3 (問題のレベルが高い) までです。
合計スコアは 0 ~ 21 の範囲で、スコアが高いほど不安や抑うつが強いことを示します。

評価は合計され、5 つのサブスケール (全身疲労、身体的疲労、精神的疲労、精神的疲労、活力) のスコアが得られ、スコアが高いほど症状が悪化していることを示します。

参加者の生活の質は、欧州研究治療機構-QOL アンケートおよび乳がん固有モジュール (EORTC QLQ-BR23) によって測定されます。
この形式ではリッカート尺度を使用し、1 (「まったくない」) から 4 (「非常にある」) までのスコアを付けます。
EORTC QLQ-BR23 は、身体イメージ、性的機能、性的楽しみ、将来の見通し、全身療法の副作用、乳房の症状、腕の症状、脱毛による動揺を評価するための 23 の質問で構成される乳房固有のモジュールです。
