選択的集学的膀胱保存的アプローチで治療された限局性筋層浸潤性膀胱癌患者を対象に、アテゾリズマブと同時分割放射線療法を併用するオープン多施設共同第 II 相試験。

研究デザイン:
膀胱の筋肉浸潤性尿路上皮癌の診断が組織学的に確認された患者を対象とした、アテゾリズマブと同時分割放射線療法のオープン多施設共同第 II 相試験。
膀胱温存療法には、以下の併用療法が含まれます。
試験治療:
治験薬(アテゾリズマブ/テセントリク®):
- 投与経路:静脈内注入。
- 治療期間:16週間。
- 投与量: 1.200 mg を 3 週間ごとに静脈内投与し、合計 6 回投与します。 アテゾリズマブは、静脈内注入用のアテゾリズマブ 1200 mg/20 mL バイアル (60 mg/mL) 溶液として提供されます。 希釈後、溶液 1 mL には約 4.4 mg のアテゾリズマブ (1,200 mg/270 mL) が含まれている必要があります。
外部ビーム放射線療法 (EBRT): 放射線
- 治療期間:6週間
- 線量: 60 Gy の放射線療法を 30 分割で 6 週間にわたって 2Gy/日で
調査対象母集団:
組織学的に膀胱の筋肉浸潤性尿路上皮がんの診断が確認された成人患者(18歳以上)で、臨床病期T2~T4aで、医学的理由、拒否、または患者の選択による根治的膀胱切除術の候補ではない。
合計39人の患者が研究に含まれると推定されています。
目的:
第一目的:
この研究の主な目的は、膀胱温存目的で治療された筋層浸潤性膀胱がん患者において、Miller and Payne 基準に従ってグレード 5 の奏効として定義される病理学的完全奏効の観点から、放射線療法と併用したアテゾリズマブの有効性を判断することです。
副次的な目的:
- 全生存期間 (OS) を評価します。
- 疾患特異的生存率 (DSS) を評価します。
- 無病生存率 (DFS) を評価します。
- 膀胱無傷無病生存率 (BIDFS) を評価します。
- 筋肉侵襲性および非筋肉侵襲性局所障害 (LF) の患者数を計算します。
- 転移を発症する患者のパーセンテージとして定義される遠隔転移率を決定する。
- 膀胱温存患者の割合を決定する。
- 即時または後期サルベージ膀胱切除術の割合を決定すること。
- アテゾリズマブと同時放射線療法の併用の安全性プロファイルと忍容性。
探索目的:
- アテゾリズマブと放射線療法で治療された膀胱癌患者における pCR、OS、DFS の観点から、PD-1 / PD-L1 および CD8 の発現の予測的役割を決定すること。
- IFN-γ、インターロイキン 6 (IL-6)、インターロイキン 18 (IL-18)、または ITAC (CXCL11 または IP-9 とも呼ばれる) のレベルと、アテゾリズマブと放射線療法による治療。
研究訪問と手順:
この調査には、次の訪問が含まれます。
- スクリーニング:研究治療を開始する前の28日以内。
- ベースライン訪問: サイクル 1 の初日
- 治療期間中の通院(16週間±7日):研究治療の最初の投与からアテゾリズマブの最後の投与または治療の中止まで。 治療は、アテゾリズマブ 1,200 mg を 3 週間ごとに 6 サイクル静脈内投与することで構成されます。
- 治療後のフォローアップ: アテゾリズマブの最後の投与から 1 ~ 2 か月後の生検と安全性の訪問、およびその後の 5 年間のフォローアップ期間が含まれます。

- 患者は18歳以上でなければなりません。
- 患者は、膀胱の筋肉浸潤性尿路上皮癌の組織学的に確認された診断を受けており、臨床病期 T2-4a N0 M0 であり、医学的理由、拒否、または患者の選択による根治的膀胱切除術の候補ではありません。
- シスプラチンベースの化学療法による治療を拒否する患者、またはシスプラチンベースの治療による治療が適切でない患者。
- -患者はECOGパフォーマンスステータス0〜2を持っている必要があります。
- 患者は、絶対好中球数によって定義される適切な骨髄機能を持っている必要があります > 1.500 / mm3;血小板 >100.000/mm3 および HB ≥ 9g/dl。
- 患者は、計算されたクレアチニンクリアランス> 15ml /分で定義されるように、十分な腎機能と肝機能を備えている必要があります。
- 総ビリルビン、SGOT (AST) および/または SGPT (ALT) が正常上限の 2.5 倍未満。
- 国際正規化比率(INR)またはプロトロンビン時間(PT):参加者が抗凝固療法を受けていない限り、≤1.5 X ULN(PTまたはPTTが抗凝固剤の使用目的の治療範囲内にある限り)。
- -活性化部分トロンボプラスチン時間(aPTT):参加者が抗凝固療法を受けていない限り、≤1.5 X ULN(PTまたはPTTが抗凝固剤の使用目的の治療範囲内にある限り)。
- -出産の可能性のある女性参加者は、患者を登録する前の72時間以内に尿または血清妊娠が陰性でなければなりません。 尿検査が陽性または陰性と確認できない場合は、血清妊娠検査が必要になります。
- 出産の可能性のある女性参加者は、避妊の2つの方法を喜んで使用するか、外科的に無菌であるか、研究の過程で異性愛活動を控える必要があります。 出産の可能性のある参加者は、外科的に不妊手術を受けていないか、1年以上月経がない人です。
- 男性参加者は、試験治療の初回投与から開始して、試験治療の最終投与の120日後まで、適切な避妊方法を使用することに同意する必要があります(1)。
- 関連する分子研究には、パラフィン包埋腫瘍サンプルが利用可能でなければなりません。
除外基準:
- -膀胱への放射線療法、全身化学療法または免疫チェックポイント阻害剤による以前の治療。 -筋層非浸潤性膀胱がんに対する以前の膀胱内BCG治療は許可されています。
- 所属リンパ節の存在または疾患の転移拡大。
- -他の実験的薬物による同時治療(研究登録前の30日以内)または他の抗がん療法。
- -以前に治療された皮膚の基底細胞癌、非筋層浸潤性膀胱癌、偶発的な前立腺癌以外の過去5年以内の以前の悪性腫瘍の病歴 -男性のステージT1a高分化前立腺癌(グリーソン= 3 + 3、PSA <5)および子宮頸部の上皮内癌。
- -腫瘍関連の中等度/重度の水腎症の証拠。
- -治癒的化学放射線療法を排除する広範囲または多巣性膀胱上皮内がん(CIS)。
- 治癒的治療に適さない巨大な T3/T4a 腫瘍 (すなわち、 > どの寸法でも 5 cm)。 腫瘍対策は、CT スキャンを介してポスト TUR を行う必要があります。
- 制御不能な重篤な感染症の患者。
- 活動性 BT (結核菌) の既知の病歴があります。
- -活動性の非感染性肺炎の既知の病歴または証拠があります。
- 白斑以外の自己免疫疾患、I型糖尿病、ホルモン補充を必要とする残存甲状腺機能低下症、全身治療を必要としない乾癬、または外部トリガーがなければ再発しないと予想される状態。
- -急性または慢性感染を示すB型肝炎ウイルス表面抗原(HBVsAg)またはC型肝炎ウイルスリボ核酸抗体(HCV-Ab)の陽性検査。

- -適切なインフォームドコンセントまたはプロトコルの遵守を妨げる一般的な医学的または心理的状態。
(1) 効果的な避妊の許容される方法:
- -治験治療開始前の14日間から治験治療終了後5ヶ月までの完全な禁欲。
- -女性の不妊手術(子宮摘出術を伴うまたは伴わない外科的両側卵巣摘出術を受けた)、または試験治療を受ける少なくとも6週間前の卵管結紮。
- -男性の不妊手術(スクリーニングの少なくとも6か月前)。 研究の女性被験者の場合、精管切除された男性パートナーがその被験者の唯一のパートナーでなければなりません。
- 文書化された故障が 1% 未満の子宮内器具またはシステム。
