研究治疗:
研究产品(atezolizumab/Tecentriq ®):
- 给药途径:静脉滴注。
- 治疗持续时间:16周。
- 剂量:每 3 周静脉注射 1.200 毫克,共 6 剂。 Atezolizumab 以 atezolizumab 1200 mg/20 mL 小瓶 (60 mg/mL) 溶液供应用于静脉输注。 稀释后,1 mL 溶液应含有约 4.4 mg atezolizumab(1,200 mg/270 mL)。

描述
纳入标准:
- 患者必须年满 18 岁。
- 患者经组织学确诊为膀胱肌肉浸润性尿路上皮癌,临床分期为 T2-4a N0 M0,由于医学原因、拒绝或患者选择不适合根治性膀胱切除术。
- 拒绝接受以顺铂为基础的化疗或不适合接受以顺铂为基础的治疗的患者。
- 患者的 ECOG 体能状态必须为 0 至 2。
- 患者必须具有足够的骨髓功能,定义为中性粒细胞绝对计数 >1.500/mm3;血小板 >100.000/mm3 和 HB ≥ 9g/dl。
- 患者必须具有足够的肾功能和肝功能,如计算的肌酐清除率 >15ml/min 所定义。
- 总胆红素、SGOT (AST) 和/或 SGPT (ALT) < 正常上限的 2.5 倍。

排除标准:
- 既往接受过膀胱放疗、全身化疗或免疫检查点抑制剂治疗。 允许先前对非肌肉浸润性膀胱癌进行膀胱内 BCG 治疗。
- 存在区域淋巴结或疾病的转移性扩展。
- 与其他实验药物(研究开始前 30 天内)或其他抗癌疗法同时治疗。
- 除了先前治疗过的皮肤基底细胞癌、非肌肉浸润性膀胱癌、偶然的前列腺癌外,在过去 5 年内既往恶性肿瘤病史 T1a 期男性高分化前列腺癌(Gleason = 3+3,PSA <5)和宫颈原位癌。
- 肿瘤相关的中度/重度肾积水的证据,除非有支架或肾造口术以保持肾功能。
- 广泛性或多灶性膀胱原位癌 (CIS) 无法进行根治性放化疗。
- 大块 T3/T4a 肿瘤不适于根治性治疗(即 > 5 厘米)。 必须通过 CT 扫描在 TUR 后进行肿瘤测量。
- 严重不受控制的感染患者。
- 有已知的活动性 BT(结核杆菌)病史。

- 已知的人类免疫缺陷病毒 (HIV) 检测阳性史或已知的获得性免疫缺陷综合症 (AIDS)。
- 患有需要在研究药物给药后 14 天内使用皮质类固醇(相当于 > 10 mg/天泼尼松)或其他免疫抑制药物进行全身治疗的病症的受试者。
- 有生育能力的妇女不愿节制或使用有效的节育方法。
- 一般的医疗或心理状况会妨碍适当的知情同意或遵守协议。
(1)可接受的有效避孕方法:
- 研究治疗开始前 14 天至研究治疗完成后 5 个月内完全禁欲。
- 女性绝育(在接受研究治疗前至少 6 周进行过双侧卵巢切除术或子宫切除术)或输卵管结扎术。
- 男性绝育(筛选前至少 6 个月)。 对于研究中的女性受试者,输精管切除术的男性伴侣应该是该受试者的唯一伴侣。
