この第 II 相無作為化試験は、ベースライン肺機能の重度の障害を伴う局所進行非小細胞肺癌に対するナブパクリタキセルとネダプラチンを組み合わせたスプリットコース胸部放射線療法の有効性を決定することです。
患者は、ナブパクリタキセル (50mg/㎡) とネダプラチン (25mg/㎡) を毎週 4 サイクル、それぞれ 1 日間、またはナブパクリタキセル (30mg/㎡) とネダプラチン (10mg/㎡) を毎週 2 回、分割コースの胸部放射線療法は、1 か月の休憩を挟んで、1 番目と 2 番目のコースでそれぞれ 30 Gy/6 分割と 40 Gy/8 分割で実施されます。 主要エンドポイントは、無増悪生存期間です。これは、放射線療法の初日から疾患が進行する日までの経過時間です。 無増悪生存期間は、Kaplan-Meier 法を使用して計算されます。毒性は、CTCAE v. 4.0 に従って等級付けされます。

包含基準:
- NSCLCの病理学的確認。
- -患者は、固形腫瘍の反応評価基準(RECIST)基準に基づいて測定可能または評価可能な病変を持っています。
- 中等度から重度の閉塞性換気障害 (FEV1.0/FVC≤60%)、 または/および中等度から重度の拘束性換気障害(FEV1.0/FVC≤60%、TLC-SB(Act1/Pred)≤80%)、 または/および中等度から重度の拡散機能障害 (DLCOc/VA(Act1/Pred)≤60%)。
- EBUSによって確認された切除不能なフェーズIIIA(N2)およびIIIB肺癌。
- ラジカル照射の最小線量である60Gyを照射した場合、全肺V20>=35%。
- 東部共同腫瘍学グループ (ECOG) のパフォーマンス ステータス 0-1。
- 以前に化学療法で治療された、または治療を受けていない
- 以前の胸部放射線療法、免疫療法、または生物療法はありません
- ヘモグロビン≧10mg/dL、血小板≧100000/μL、絶対好中球数≧1500/μL
- -血清クレアチニンが正常上限(UNL)の1.25倍以下、またはクレアチニンクリアランスが60ml/分以上

- 正常範囲内の CB6

除外基準:
- -非黒色腫皮膚がんまたは子宮頸がんを除く、以前または最近の別の悪性腫瘍 in situ
- 化学療法の禁忌
- 悪性胸水または心嚢液。
- 妊娠中、授乳期、妊娠検査未実施の女性 初回投与14日前
- 避妊しなくても妊娠の可能性がある女性
- 出血傾向
- 30日以内の他の臨床試験で
- 薬物やアルコール依存症、エイズ患者
- 制御不能な発作または自己制御能力のない精神病患者

実験的:分割コース放射線療法
放射線療法は、同時統合ブースト (SIB) 強度変調放射線療法 (IMRT) を使用して行われます。 30Gy の線量と 5Gy の分割線量が、低 RT コースとして PTV-GTV に照射されました。 患者は、疾患の進行がなく、持続的な ≥G2 治療関連毒性がない場合に低ブースト療法を受ける資格がありました。 持続性 ≥G2 毒性のある患者については、低ブースト療法を受ける資格があるかどうかを判断するために 2 週間ごとに再評価が計画されました。 適応放射線治療計画を再策定するために、適格な患者全員が 4DCT シミュレーション スキャンを繰り返し受けました。 低ブーストの適応計画は、6 回に分けて 30Gy の線量 (1 回あたり 5 Gy) で残存腫瘍 (PTV-GTV-残存) に送達されました。 患者は、低 RT および低ブースト療法と同時に、ドセタキセル (25 mg/m2) およびシスプラチン (25 mg/m2) の毎週の注入を受けました。 意図された化学療法には、治療期間全体を通して 4 サイクルが含まれていました。 |
