Denne randomiserte fase II-studien skal bestemme effekten av delt-kurs thorax strålebehandling kombinert med nab-paclitaxel pluss nedaplatin for lokal avansert ikke-småcellet lungekreft med alvorlig svekkelse av baseline lungefunksjon.
Pasientene fikk fire sykluser med ukentlig nab-paclitaxel (50 mg/㎡) og nedaplatin (25 mg/㎡), hver av 1 dags varighet, eller nab-paclitaxel (30 mg/㎡) og nedaplatin (10 mg/㎡) to ganger hver uke, samtidig med delt kur thorax strålebehandling av 30 Gy/6 fraksjoner og 40 Gy/8 fraksjoner administrert i henholdsvis første og andre kur med en måneds pause. Det primære endepunktet er progresjonsfri overlevelse, som er tiden som går fra første dag med strålebehandling til datoen da sykdommen utvikler seg. Progresjonsfri overlevelse vil bli beregnet ved hjelp av Kaplan-Meier-metoden. Toksisiteter vil bli gradert i henhold til CTCAE v. 4.0.

Inklusjonskriterier:
- Patologisk bekreftelse av NSCLC.
- Pasienter har målbare eller evaluerbare lesjoner basert på kriteriene for responsevaluering i solide svulster (RECIST).
- Moderat til alvorlig obstruktiv ventilasjonsdysfunksjon (FEV1.0/FVC≤60 %), eller / og moderat til alvorlig restriktiv ventilasjonsdysfunksjon (FEV1.0/FVC≤60%,TLC-SB(Act1/Pred)≤80%), eller / og moderat til alvorlig diffusjonsdysfunksjon (DLCOc/VA(Act1/Pred)≤60%).
- Ikke-opererbar fase IIIA(N2) og IIIB lungekreft bekreftet av EBUS.
- Hellunge V20>=35 % når man gir 60Gy som er minimumsdosen radikal bestråling.
- Eastern Cooperative Oncology Group (ECOG) ytelsesstatus 0-1.
- Tidligere behandlet med kjemoterapi eller behandlingsnaiv
- Ingen tidligere strålebehandling, immunterapi eller bioterapi
- Hemoglobin≥10 mg/dL, blodplater≥100 000/μL, absolutt nøytrofiltall ≥1500/μL
- Serumkreatinin ≤1,25 ganger øvre normalgrense (UNL), eller kreatininclearance ≥60 ml/min.
- Bilirubin ≤1,5 ganger UNL, AST(SGOT)≤2,5 ganger UNL,ALT(SGPT)≤2,5 ganger UNL,alkalisk fosfatase ≤5 ganger UNL

Ekskluderingskriterier:
- Tidligere eller nylig annen malignitet, bortsett fra ikke-melanom hudkreft eller livmorhalskreft in situ
- Kontraindikasjon for kjemoterapi

Eksperimentell: strålebehandling med delt kurs
Stråleterapien leveres ved bruk av simultan integrert boost (SIB)-intensitetsmodulert strålebehandling (IMRT). Dosen på 30Gy med en fraksjonsdose på 5Gy ble levert til PTV-GTV som hypo-RT-kurs. Pasienter var kvalifisert til å motta hypo-boost når det ikke var sykdomsprogresjon og ingen vedvarende ≥G2 behandlingsrelaterte toksisiteter. For pasienter med vedvarende ≥G2-toksisitet, ble det planlagt en re-evaluering hver 2. uke for å avgjøre om de var kvalifisert til å motta hypoboost. Alle kvalifiserte pasienter gjennomgikk en gjentatt 4DCT-simuleringsskanning for å omformulere planen for adaptiv strålebehandling. Den adaptive planen for hypo-boosten ble levert til den gjenværende svulsten (PTV-GTV-rest) i en dose på 30Gy i 6 fraksjoner (5 Gy per fraksjon).
