Split-cursus thoracale radiotherapie gecombineerd met Nab-paclitaxel plus nedaplatine voor lokale geavanceerde NSCLC met verminderde longfunctie bij baseline

Deze gerandomiseerde fase II-studie is bedoeld om de werkzaamheid te bepalen van thoracale radiotherapie met twee kuren in combinatie met nab-paclitaxel plus nedaplatine voor lokaal gevorderde niet-kleincellige longkanker met ernstige verslechtering van de longfunctie bij aanvang.
Patiënten kregen vier wekelijkse cycli van nab-paclitaxel (50 mg/㎡) en nedaplatine (25 mg/㎡), elk met een duur van 1 dag, of tweemaal per week nab-paclitaxel (30 mg/㎡) en nedaplatine (10 mg/㎡), gelijktijdig met split-kuur thoracale radiotherapie van 30 Gy/6 fracties en 40 Gy/8 fracties toegediend in respectievelijk de eerste en tweede kuur, met een pauze van een maand. Het primaire eindpunt is progressievrije overleving, de tijd die verstrijkt vanaf de eerste dag van radiotherapie tot de datum waarop de ziekte voortschrijdt. Progressievrije overleving zal worden berekend met behulp van de Kaplan-Meier-methode. Toxiciteiten zullen worden beoordeeld volgens CTCAE v. 4.0.

Beschrijving
Inclusiecriteria:
- Pathologische bevestiging van NSCLC.
- Patiënten hebben meetbare of evalueerbare laesies op basis van de criteria voor responsevaluatie bij solide tumoren (RECIST).
- Matige tot ernstige obstructieve beademingsdisfunctie (FEV1.0/FVC≤60%), of / en matige tot ernstige restrictieve ventilatoire disfunctie (FEV1.0/FVC≤60%, TLC-SB (Act1/Pred)≤80%), of/en matige tot ernstige diffusiedisfunctie (DLCOc/VA(Act1/Pred)≤60%).
- Inoperabele fase IIIA(N2) en IIIB longkanker bevestigd door EBUS.
- Hele long V20>=35% bij toediening van 60Gy, de minimale dosis radicaalbestraling.
- Eastern Cooperative Oncology Group (ECOG) prestatiestatus 0-1.
- Eerder behandeld met chemotherapie of niet eerder behandeld
- Geen eerdere thoraxradiotherapie, immunotherapie of biotherapie
- Hemoglobine≥10 mg/dL, bloedplaatjes≥100.000/μL, absoluut aantal neutrofielen ≥1500/μL
- Serumcreatinine ≤1,25 maal de bovengrens van de normaalwaarde (UNL), of creatinineklaring ≥60 ml/min
- Bilirubine ≤1,5 keer UNL, AST (SGOT) ≤2,5 keer UNL, ALT (SGPT) ≤2,5 keer UNL, alkalische fosfatase ≤5 keer UNL
- CB6 binnen normale grenzen

Uitsluitingscriteria:
- Vorige of recente andere maligniteit, behalve niet-melanome huidkanker of baarmoederhalskanker in situ
- Contra-indicatie voor chemotherapie
- Kwaadaardige pleurale of pericardiale effusie.
- Zwangere vrouwen, lactatieperiode of geen zwangerschapstest 14 dagen voor de eerste dosis
- Vrouwen die de kans hebben op zwangerschap zonder anticonceptie
- Neiging tot bloedingen
- In andere klinische onderzoeken binnen 30 dagen
- Verslaafd aan drugs of alcohol, AIDS-patiënten
- Oncontroleerbare aanvallen of psychotische patiënten zonder zelfbeheersing
- Ernstige allergie of eigenaardigheid
- Niet geschikt voor dit onderzoek, oordeelden de onderzoekers

Experimenteel: gesplitste radiotherapie
De radiotherapie wordt toegediend met behulp van simultane geïntegreerde boost (SIB)-intensiteitsgemoduleerde radiotherapie (IMRT). De dosis van 30Gy met een fractiedosis van 5Gy werd als hypo-RT-kuur aan PTV-GTV toegediend. Patiënten kwamen in aanmerking voor de hypo-boost als er geen ziekteprogressie was en geen aanhoudende ≥G2-behandelingsgerelateerde toxiciteiten. Voor patiënten met aanhoudende ≥G2-toxiciteiten werd elke twee weken een herevaluatie gepland om te bepalen of zij in aanmerking kwamen voor het ontvangen van de hypoboost. Alle in aanmerking komende patiënten ondergingen een herhaalde 4DCT-simulatiescan om het adaptieve bestralingstherapieplan opnieuw te formuleren. Het adaptieve plan van de hypo-boost werd toegediend aan de resterende tumor (PTV-GTV-residu) in een dosis van 30Gy in 6 fracties (5 Gy per fractie).
