기저선 폐 기능 장애가 있는 국소 진행성 NSCLC에 대해 Nab-paclitaxel과 Nedaplatin을 병용한 분할 과정 흉부 방사선 요법

이 2상 무작위배정 연구는 베이스라인 폐 기능의 심각한 장애가 있는 국소 진행성 비소세포 폐암에 대해 nab-paclitaxel과 nedaplatin을 병용한 분할 코스 흉부 방사선 요법의 효능을 결정하기 위한 것입니다.
환자들은 nab-paclitaxel(50mg/㎡)과 nedaplatin(25mg/㎡)을 각각 1일씩 4주기로 주당 투여받거나 nab-paclitaxel(30mg/㎡)과 nedaplatin(10mg/㎡)을 주 2회씩 동시에 투여받았다. 30 Gy/6 분할 및 40 Gy/8 분할의 분할 과정 흉부 방사선 치료가 1개월 휴식과 함께 첫 번째 과정과 두 번째 과정에서 각각 시행되었습니다. 1차 종료점은 방사선 치료 첫날부터 질병이 진행되는 날짜까지의 시간인 무진행 생존 기간입니다. 무진행 생존은 Kaplan-Meier 방법을 사용하여 계산됩니다. 독성은 CTCAE v. 4.0에 따라 등급이 매겨집니다.

포함 기준:
- NSCLC의 병리학적 확인.
- 환자는 고형 종양의 반응 평가 기준(RECIST) 기준에 따라 측정 가능하거나 평가 가능한 병변이 있습니다.
- 중등도에서 중증의 폐쇄성 환기 기능 장애(FEV1.0/FVC≤60%), 또는 / 및 중등도에서 중증의 제한적 환기 기능 장애(FEV1.0/FVC≤60%,TLC-SB(Act1/Pred)≤80%), 또는 / 및 중등도 내지 중증 확산 장애(DLCOc/VA(Act1/Pred)≤60%).
- EBUS로 확인된 절제 불가능한 IIIA기(N2) 및 IIIB기 폐암.
- 라디칼 조사의 최소 선량인 60Gy를 주면 전체 폐 V20>=35%.
- 동부 협력 종양학 그룹(ECOG) 수행 상태 0-1.
- 이전에 화학 요법 또는 치료 경험이 없는 치료를 받은 적이 있는 경우
- 이전 흉부 방사선 요법, 면역 요법 또는 생물 요법 없음
- 헤모글로빈≥10 mg/dL, 혈소판≥100000/μL, 절대 호중구 수≥1500/μL
- 혈청 크레아티닌 ≤1.25배 정상 상한치(UNL) 또는 크레아티닌 청소율≥60 ml/min
- 빌리루빈 ≤1.5배 UNL, AST(SGOT)≤2.5배 UNL ,ALT(SGPT)≤2.5배 UNL, 알칼리 포스파타제 ≤5배 UNL
- 정상 한계 내의 CB6
- 환자와 그 가족은 정보에 입각한 동의서에 서명했습니다.
제외 기준:
- 비흑색종 피부암 또는 자궁경부암을 제외한 이전 또는 최근의 또 다른 악성 종양
- 화학 요법에 대한 금기
- 악성 흉막 또는 심낭 삼출액.
- 임신, 수유기 또는 임신 테스트를 하지 않은 여성은 초회 투여 14일 전
- 피임을 하지 않아도 임신 가능성이 있는 여성
- 출혈 경향
- 30일 이내 다른 임상시험에서
- 마약이나 술에 중독된 AIDS 환자
- 자기 조절 능력이 없는 조절 불가능한 발작 또는 정신병 환자

실험적: 분할 코스 방사선 요법
방사선 치료는 동시 통합 부스트(SIB)-강도 변조 방사선 치료(IMRT)를 사용하여 전달됩니다. hypo-RT 과정으로 PTV-GTV에 5Gy의 분할 선량으로 30Gy의 선량이 전달되었습니다. 환자들은 질병 진행이 없고 지속적인 ≥G2 치료 관련 독성이 없을 때 하이포부스트를 받을 자격이 있었습니다. 지속적으로 ≥G2 독성을 보이는 환자의 경우, 하이포부스트를 받을 자격이 있는지 여부를 결정하기 위해 2주마다 재평가가 계획되었습니다. 모든 적격 환자는 적응형 방사선 치료 계획을 다시 작성하기 위해 반복적인 4DCT 시뮬레이션 스캔을 받았습니다. hypo-boost의 적응 계획은 6개 분할로 30Gy의 선량(분할당 5Gy)으로 잔여 종양(PTV-GTV-잔류)에 전달되었습니다. 환자들은 hypo-RT 및 hypo-boost 요법과 동시에 도세탁셀(25 mg/m2)과 시스플라틴(25 mg/m2)을 매주 주입 받았습니다. 의도된 화학요법에는 치료 과정 전반에 걸쳐 4주기가 포함되었습니다. |
