この研究は、ブプレノルフィンによるオフィスベースのオピオイド治療(OBOT)のコンテキスト内で心理社会的治療を提供するためのベストプラクティスを知らせるのに役立ちます. 段階的アプローチを使用して、ブプレノルフィンプロバイダーによる投薬管理と、患者のニーズに応じたCRSおよびCBTへのリンクを含む、補助的な心理社会的治療の提供への適応的アプローチを開発および評価します。 提案の具体的な目的は以下のとおりです。
目的 1: 学際的なチームと協力して、CRS および CBT 介入のための特定のプロトコルを開発し、行動基準に基づいてアルゴリズムを確立して、各介入をいつ実装する必要があるかを決定します。 この目的の一環として、各介入を提供するための標準的な手順を開発し、介入者 [つまり、CRS と認定専門カウンセラー (LPC)] をトレーニングして、忠実にそれらを提供します。
目的 2: 適応介入の有効性を TAU と比較して評価するために、2 群無作為化試験を実施します。 検査される結果には、尿検査で確認されたオピオイドの使用、ブプレノルフィンベースのOBOTの保持、生活の質、および研究後1年間の心理社会的機能が含まれます。
目的 3: 介入の質的評価を実施し、その普及のための戦略計画を作成します。 適応介入の有用性と受容性を判断するために、クリニックのスタッフと関連する利害関係者とのフォーカス グループを実施します。 さらに、持続可能性を高め、他のオフィスベースのブプレノルフィンプログラムによる採用を促進するためのメカニズムを特定するための専門家ラウンドテーブルを開催して、戦略計画の策定を通知します。

包含基準:
- -過去4週間以内にFQHCでOUDのオフィスベースのブプレノルフィン治療を開始しました;
- 医療提供者によって決定された入院レベルのケアを必要としない;と
- 有効な連絡先情報とインフォームド コンセントを提供できること。
除外基準:
- 患者は 18 歳未満です。
- -より集中的な在宅治療の必要性を示す併存精神障害
- -患者はインフォームドコンセントを提供できません。
ベースラインで酔っている、認知障害がある、または精神医学的に不安定な個人は含まれません。ただし、失格条件が緩和された場合は、後で含めることができます。

介入なし:通常通りの治療 (TAU)
TAU 症状に割り当てられた参加者は、ブプレノルフィン投薬管理の予約がスケジュールされ、FQHC で OBOT と、通常 FQHC で提供される補助的な心理社会的治療を受けます。
チームは、その後のMAT訪問中に、部位によって多少の違いはありますが、頻度を減らしながら患者との面会を継続します。
MAT 来院のスケジュールには通常、導入週に 3 回の来院、患者が安定するまでは週に 1 ~ 2 回の来院、その後は毎月の来院が含まれます。
行動保健臨床医は患者をサポートし、UDS の結果について話し合い、回復と断酒への調整に関する戦略的問題解決を支援し、患者の MAT への取り組みを監視します。

適応的介入条件に割り当てられた参加者は、TAU について上記で説明したクリニックのプロトコルに従って、ブプレノルフィン投薬管理の予約がスケジュールされます。
この症状の参加者が受ける補助的な心理社会的治療は、(1) 行動健康専門家による CBT および/または (2) 認定回復専門家によるピアサポートです。
積極的な介入期間は治験登録後 3 か月となります。
参加者は、積極的な介入期間の後も引き続き TAU を受け取ります。

この状態の参加者が受ける補助的な心理社会的治療は、(1) 問題行動の専門家が提供する CBT および/または (2) 認定された回復の専門家が提供するピアサポートです。
積極的な介入期間は、研究登録後3か月に及びます。
参加者は、積極的な介入期間の後も TAU を受け取り続けます。
