Deze studie zal helpen bij het informeren van de beste praktijken voor het bieden van psychosociale behandeling binnen de context van kantoorgebaseerde opioïdenbehandeling (OBOT) met buprenorfine. We zullen een gefaseerde aanpak gebruiken om een adaptieve aanpak te ontwikkelen en te evalueren voor het aanbieden van aanvullende psychosociale behandeling, inclusief medicatiebeheer door een leverancier van buprenorfine en koppeling met CRS's en CBT, afhankelijk van de behoeften van de patiënt. De specifieke doelstellingen van het voorstel worden hieronder vermeld.
Doel 1: Werk samen met ons multidisciplinaire team om specifieke protocollen voor de CRS- en CBT-interventies te ontwikkelen en algoritmen op te stellen op basis van gedragscriteria om te bepalen wanneer elke interventie moet worden geïmplementeerd. Als onderdeel van dit doel zullen we ook standaardprocedures ontwikkelen voor het leveren van elke interventie en interventionisten trainen [d.w.z. CRS's en Licensed Professional Counselors (LPC's)] om ze met getrouwheid uit te voeren.
Doel 2: Voer een gerandomiseerd onderzoek met twee groepen uit om de doeltreffendheid van de adaptieve interventie ten opzichte van TAU te evalueren. Uitkomsten die moeten worden onderzocht, zijn onder meer door urineonderzoek bevestigd opioïdengebruik, behoud van op buprenorfine gebaseerde OBOT, kwaliteit van leven en psychosociaal functioneren tot een jaar na aanvang van de studie.
Doel 3: Voer een kwalitatieve evaluatie uit van de interventie en ontwikkel een strategisch plan voor de verspreiding ervan. We zullen focusgroepen houden met kliniekpersoneel en relevante belanghebbenden om het nut en de aanvaardbaarheid van de adaptieve interventie te bepalen. Daarnaast zullen we een rondetafelgesprek met deskundigen houden om mechanismen te identificeren voor het vergroten van de duurzaamheid en het verbeteren van de acceptatie door andere op kantoren gebaseerde buprenorfine-programma's om de ontwikkeling van het strategisch plan te informeren.
Doel 4: Een trainingsprogramma opzetten in klinisch onderzoek voor allochtone studenten. Acht niet-gegradueerde studenten van een historisch minderheidsinstelling voor hoger onderwijs zullen worden geselecteerd om deel te nemen aan stages van 9 maanden waarin ze uitgebreide en pragmatische training zullen krijgen in het volledige scala van klinisch onderzoek, van studieontwerp tot disseminatie.

Beschrijving
Inclusiecriteria:
- In de afgelopen 4 weken een behandeling met buprenorfine op kantoor voor OUD zijn gestart bij de FQHC;
- Geen intramuraal zorgniveau vereisen zoals bepaald door de zorgverlener; En
- In staat zijn om geldige contactgegevens en geïnformeerde toestemming te verstrekken.
Uitsluitingscriteria:
- Patiënt is jonger dan 18 jaar;
- Een comorbide psychiatrische stoornis die wijst op de behoefte aan intensievere residentiële behandeling
- Patiënt kan geen geïnformeerde toestemming geven.
Individuen die dronken, cognitief gehandicapt of psychiatrisch onstabiel zijn bij aanvang zullen niet worden opgenomen; ze kunnen later echter worden opgenomen als de diskwalificerende voorwaarde afneemt.

Geen tussenkomst: Behandeling zoals gebruikelijk (TAU)
Deelnemers die aan de TAU-conditie zijn toegewezen, zullen worden ingepland voor afspraken over het medicatiebeheer met buprenorfine en zullen OBOT krijgen bij de FQHC en aanvullende psychosociale behandeling zoals doorgaans wordt geboden bij de FQHC.
Het team zal de patiënt blijven ontmoeten tijdens daaropvolgende MAT-bezoeken, met een afnemende frequentie, met enkele kleine locatiespecifieke variaties.
Het schema van MAT-bezoeken omvat over het algemeen drie kliniekbezoeken tijdens de week van introductie, 1-2 bezoeken per week totdat de patiënt gestabiliseerd is, en daarna maandelijks.
Gedragsgeneeskundigen bieden ondersteuning aan de patiënt, bespreken UDS-resultaten, helpen bij het strategisch oplossen van problemen rond herstel en aanpassing aan nuchterheid, en houden toezicht op de betrokkenheid van de patiënt bij MAT.

Experimenteel: Adaptieve interventie
Deelnemers die zijn toegewezen aan de adaptieve interventieconditie zullen worden ingepland voor afspraken over medicatiebeheer met buprenorfine volgens het hierboven beschreven kliniekprotocol voor TAU.
De aanvullende psychosociale behandeling die deelnemers aan deze aandoening krijgen, bestaat uit (1) cognitieve gedragstherapie, gegeven door specialisten op het gebied van de gedragsgezondheid, en/of (2) collegiale ondersteuning, geleverd door een gecertificeerde herstelspecialist.
De actieve interventieperiode zal drie maanden na aanvang van de studie beslaan.
Deelnemers blijven TAU ontvangen na de actieve interventieperiode.

Deelnemers die zijn toegewezen aan de adaptieve interventieconditie zullen worden ingepland voor afspraken over het beheer van buprenorfine-medicatie volgens het kliniekprotocol dat hierboven is beschreven voor TAU.
De aanvullende psychosociale behandeling die deelnemers aan deze aandoening krijgen, is (1) CGT geleverd door gedragsspecialisten en/of (2) collegiale ondersteuning geleverd door gecertificeerde herstelspecialist.
De actieve interventieperiode beslaat 3 maanden na aanvang van de studie.
Deelnemers blijven TAU ontvangen na de actieve interventieperiode.

Kwaliteit van leven wordt gemeten met behulp van de Short Form-36 (SF-36).
De SF-36 is een zelfrapportage-inventaris die acht dimensies van fysieke en mentale gezondheidsgerelateerde kwaliteit van leven beoordeelt.
Het is aangetoond dat de SF-36 een hoge betrouwbaarheid en validiteit heeft.

Multidimensionale probleemernst, een secundaire uitkomstmaat, zal worden gemeten met behulp van de Addiction Severity Index-Lite (ASI-Lite).
De ASI-Lite is een betrouwbare en geldige multidimensionale beoordeling die samengestelde scores geeft die de huidige ernst van het probleem weergeven op medisch, werk-, alcohol-, drugs-, juridisch, familie/sociaal en psychiatrisch gebied.
