当社の入院睡眠検査プログラムの一環として、高リスク心不全患者は、睡眠検査技師が設定したタイプIII睡眠検査(日本光電®)を受けます。 当社の入院患者向け睡眠サービスには、患者と協力して CPAP 教育を行い、入院後の睡眠ケアを設定して支援する臨床看護師もいます。 滴定研究が必要な場合、通常、退院後 1 ~ 2 週間後に手配されます。 すべての入院患者の睡眠研究は 3 人の登録睡眠ポリグラフ検査技師によって採点され、私がこれらの研究をレビューして最終決定します (入院患者サービスを主導する主治医としての現在の臨床フローは、この臨床試験では強化されます)。 研究コーディネーターと私は、REI>5 で Epic® のスクリーニングごとに適格性を満たす患者にアプローチし、睡眠研究の結果について話し合い、適格性を確認し、患者を参加に招待し、興味がある場合は書面によるインフォームドコンセントを取得します。 同意: 同意書では、すべての手順の性質を一般向けの用語で説明します。 参加は任意であることが強調されるだろう。 研究コーディネーターとPIが質問にお答えします。 すべての活動は現地の IRB および HIPPA ガイドラインに準拠します。
すべての参加者は、退院前に同じホームケア医療機関によって提供される 2 つのメーカー (Philips Respironics ® または ResMed ®) から Auto-CPAP を取得します。 PAP 圧力は、快適さに応じて 5 ~ 15 cmH2O、加温加湿、および呼気圧力解放に設定されます。 モデムは PAP デバイスに接続され、遵守データをリモートで抽出します (ワイヤレス機能がない場合は SD カード)。 すべての患者は退院前にPAP療法に関する標準化された20分間の教育セッションを受け、臨床看護師から毎晩睡眠中にデバイスを使用するようアドバイスされます。 患者には在宅医療用のマスクも装着される。 参加者がマスクや圧力不耐性の問題を抱えている場合は、当社のリサーチ コーディネーターに直接電話番号が提供されます。 すべてのベースラインアンケートは、REDcap リンクを介して患者の電子メールに送信されるか、退院前および来院中に紙の形式で記入されます (電子的な質問が不可能な場合は紙の形式で)。 コーディネーターは、退院前にすべてのアンケートが完了していることを確認し、フォローアップのアンケートに記入するように患者に指示します。 患者は、TIME と標準治療に対して 1:1 でランダム化されます (REDCap アルゴリズム)。 ブロックはランダムなサイズであり、研究者には盲検化されます。 研究チームや参加者に介入が分からないようにすることは現実的ではないが、標準治療グループも睡眠衛生教育を受け、プラセボ効果に対処するためにある程度の介入が行われていると認識されるようにする。 睡眠薬を使用したフォローアップ訪問中(TIMEグループは2週間と2ヶ月、標準治療グループは2ヶ月)、これが当院で提供される標準的な臨床ケアであるため、最初の15分間はOSAと両グループの問題のトラブルシューティングに焦点を当てます。睡眠障害センター。 この 15 分間の後に、介入グループでは 30 分間の ME、対照グループでは 10 分間の睡眠衛生教育が行われます。 結果は、両グループの 3 か月後と 6 か月後に測定されます。 どちらのグループも生後 4 か月後に追加電話を受けます。 介入グループは 30 分間のブースター ME セッションを受け、対照グループは 10 分間の睡眠衛生を受けます。

- ADHFの診断を認める
- OSA診断(REI≧5)
- 未治療でPAP療法に同意できる
- 参加することに同意します。
除外基準:
- 以前または現在の PAP 療法
- インフォームドコンセントを提供できない
- 中等度の低酸素状態(室内空気の酸素飽和度が 87% 未満、または一晩の睡眠研究中に 2L 以上の O2 が必要)
- 血行動態が不安定(収縮期血圧、SBP<90 mmHg、心拍数>120bpm)
- 中枢性優勢無呼吸 (中枢性無呼吸指数 > 無呼吸指数の 50%) 認知症/認知機能障害
