超低カロリーケトジェニックダイエットが微生物叢、脂肪組織、免疫調節に及ぼす影響: メタボリックシンドローム患者を対象としたパイロット研究
VLCKDは、代謝のいくつかの側面に影響を与える食事であることが示されており、2型糖尿病、過体重、慢性炎症、脂肪肝の予防と治療に役立つことが証明されています。
このため、このパイロット研究の目的は、同時代にDM2、肥満、非アルコール性脂肪肝疾患(NAFLD)を患っている患者群に対するVLCKDの潜在的な影響を調査し、その結果を地中海食に基づくイポカロリー食と比較することである。原則とイタリアの LARN (SINU 2014)。
この研究では、人体計測データ、血液化学的パラメータおよびホルモンパラメータ、アンケート、便微生物叢とオミクス、血液微小胞、尿検査、機器検査(DXA、BIVA、エコグラフィー)、生検および機能検査など、相互に関連するいくつかの結果を考慮します。
40人の被験者が評価され、20人(VLCKD)と20人(MedDiet)の2つのグループに分けられます。

- BMI 30-40 mg/m2
- NAFLD
- DM2 薬物治療(メトホルミン、SGLT2 阻害剤、GLP-1 類似体、DPPIV 阻害剤、基礎インスリン)および HbA1c > 7 および < 10 %。
除外基準:
- 遺伝的または内分泌学的原因による続発性肥満。
- eGFR < 45 mL/min/1.73m2 の腎疾患 またはマクロアルブミン尿または結石症
- インスリン基礎+ボーラスまたはHbA1c% >10.0%
- その他の種類のDM
- 即下垂体症または副腎不全
- 抗生物質の使用は初回来院前 3 か月未満である

20 人の患者が内分泌科から募集され、同じ来院頻度と薬剤を維持し、2 つのグループのいずれかにランダムに割り当てられることを受け入れます。 包含基準:

患者は、専門の栄養士から VLCKD 食に関する正確な指導とパンフレットを受け、Labotaoire Therascience から提供される適切な量のサプリメント (4 食または 5 食) を自由に受け取ります。
サプリメントには、(合計で)600 ~ 800 kcal、主に脂肪、体重あたり 1.2 ~ 1.5 g のタンパク質、非常に少量の炭水化物が含まれています(

患者は、専門の栄養士から、低カロリーの地中海スタイルの食事に関する正確な指導とパンフレット (LARN 2014) を受け取ります。
患者はグラムと食べる食品についての指示に従います。
カロリーは、エネルギー必要量(間接熱量測定 * 身体活動スコアによって測定)から約マイナス 400 ~ 500 kcal になります。
