更年期への移行(「閉経周辺期」)は、うつ病や精神病の新たな発症のリスクが高まることを特徴としています。 私たちの仕事と他の人の仕事は、閉経周辺期発症 (PO) うつ病の顕著な症状が無快感症であり、苦痛と機能障害に大きく寄与していることを示しています。 さらに、女性の精神病の発生率は、この期間中に増加する可能性があります. 特に更年期後期におけるエストラジオールの減少または低レベルは、中脳辺縁系の脳報酬回路およびドーパミン (DA) 神経伝達への影響を介して、PO 無快感症および PO 精神病の病因に関与している可能性があります。 エストラジオールがドーパミンシステムと報酬関連の行動を調節し、エストラジオールの離脱がドーパミン作動性機能の喪失を引き起こすという前臨床的証拠が確立されています。 エストロゲン療法は、更年期前後の女性の気分や精神病の症状を軽減する効果があることが示されていますが、トランス診断サンプルにおけるそのような効果の根底にある神経メカニズムを調べた研究はありません。

- -すべての研究手順、ライフスタイルの考慮事項、および研究期間中の利用可能性を遵守する意思を表明した
- 月経周期が 2 回以上スキップされ、60 日以上の無月経があり、閉経後期の移行期に相当する 44 ~ 55 歳の薬を服用していない閉経周辺期の女性 (Stages of Reproductive Aging Workshop (STRAW stage -1))。
- 月経不順の期間中に始まった無快感症または精神病の症状。
- 臨床医のグローバル印象スケール - 重症度スコア (CGI-S) > 3 臨床的に障害のあるサンプルを確認します。
- 無快感症の重症度の選択基準と層別化: すべての参加者は、臨床的に損なわれている無快感症に対応する、気分および不安障害の NIMH Fast-Fail 試験と一致する Snaith-Hamilton Pleasure Scale (SHAPS) スコア > 20 を持ちます。
