상세 설명
폐경으로의 전환("폐경기 주변기")은 우울증과 정신병의 새로운 발병 위험이 증가하는 것이 특징입니다. 우리와 다른 사람들의 작업은 폐경기 발병(PO) 우울증의 눈에 띄는 증상이 무쾌감증이며 고통과 기능 장애에 크게 기여한다는 것을 보여주었습니다. 또한 이 기간 동안 여성의 정신병 발병률이 증가할 수 있습니다. 에스트라디올의 감소 또는 낮은 수준, 특히 후기 폐경기에서 중뇌 변연계 뇌 보상 회로 및 도파민(DA) 신경 전달에 대한 효과를 통해 PO 무쾌감증 및 PO 정신병의 발병에 역할을 할 수 있습니다. 전임상 증거는 에스트라디올이 도파민 시스템과 보상 관련 행동을 조절하고 에스트라디올 철수가 도파민 기능의 손실을 유발한다는 것을 확립했습니다. 에스트로겐 요법이 폐경기 여성의 기분과 정신병 증상을 줄이는 데 도움이 되는 것으로 나타났지만, 트랜스진단 샘플에서 그러한 효과의 근간이 되는 신경 메커니즘을 조사한 연구는 없습니다.

- 44-55세의 약을 사용하지 않고 월경 주기를 건너뛰는 주기가 2회 이상이고 무월경이 60일 이상이며 후기 폐경 전환에 해당하는 폐경기 주변기 여성(생식 노화 워크숍 단계(STRAW 단계 -1)
- 불규칙한 월경 기간 동안 시작된 무쾌감증 또는 정신병 증상.
- 임상적으로 손상된 샘플을 확인하기 위해 Clinician's Global Impression Scale-Severity score(CGI-S) > 3.
- 무쾌감증 심각도 포함 기준 및 계층화: 모든 참가자는 임상적으로 무쾌감증에 해당하는 기분 및 불안 장애에 대한 NIMH Fast-Fail 시험과 일치하는 SHAPS(Snaith-Hamilton Pleasure Scale) 점수 > 20을 갖게 됩니다.
- 정신병 중증도 포함 기준 및 계층화: 참가자는 간략한 정신과 평가 척도(BPRS)의 정신병 하위 척도 점수에 따라 계층화됩니다.
- 에스트라디올 요법을 고수하려는 의지
제외 기준:
- 임신; Climara® 패치 또는 Prometrium®의 활성 또는 비활성 성분에 대한 알레르기.
- BMI < 18 또는 > 35kg/m^2
- 만성 월경 주기 불규칙의 병력, 월경 없이 > 1년을 의미
- MR 금기 사항: 신체의 금속, 충전재 또는 금 이외의 치과 작업, 문신, 금속 손상, 100% 플라스틱이 아닌 기타 임플란트.
- PET 모순: 지난 12개월 동안 전신에 3rem을 초과하는 전리 방사선(예: PET, CT)을 포함하는 1개 이상의 연구에 참여했습니다. 치료 영상의 표준은 배타적이지 않습니다.
