Onderzoek naar de effecten van oestradiol op de neurale en moleculaire respons op beloningen bij anhedonie en psychose met aanvang in de perimenopauzale periode

Gedetailleerde beschrijving
De overgang naar de menopauze (de "perimenopauze") wordt gekenmerkt door een verhoogd risico op het opnieuw ontstaan van depressie en psychose. Ons werk en dat van anderen heeft aangetoond dat een prominent symptoom van perimenopauzale-onset (PO) depressie anhedonie is, wat aanzienlijk bijdraagt aan angst en functionele beperkingen. Bovendien kan de incidentie van psychose bij vrouwen in deze periode toenemen. Dalende of lage oestradiolspiegels, vooral in de late perimenopauze, kunnen een rol spelen bij de pathogenese van PO-anhedonie en PO-psychose via effecten op mesolimbische hersenbeloningscircuits en dopamine (DA) neurotransmissie. Preklinisch bewijs heeft aangetoond dat oestradiol dopaminesystemen en beloningsgerelateerd gedrag moduleert en dat oestradiolontwenning verlies van dopaminerge functies veroorzaakt. Terwijl oestrogeentherapie voordelen heeft aangetoond bij het verminderen van stemming en psychotische symptomen bij vrouwen in de overgang, heeft geen enkele studie de neurale mechanismen die aan dergelijke effecten ten grondslag liggen, onderzocht in een transdiagnostische steekproef.
Dit project zal de effecten van oestradioltoediening op perimenopauzale anhedonie (PO) en psychose onderzoeken met behulp van gelijktijdige positronemissietomografie en functionele magnetische resonantiebeeldvorming (PET-MR). Voorlopige gegevens die hier worden gepresenteerd, tonen aan dat anhedonie geassocieerd is met verminderde striatale DA-afgifte aan beloningen met behulp van PET met de D2 / D3 DA-receptorantagonist [11C] raclopride; anhedonie en psychose worden gekenmerkt door veranderde striatale activering tot beloningen met behulp van fMRI; oestradiol beïnvloedt neurale reacties op beloningen bij PO-anhedonie en PO-psychose; en oestradiol verbetert PO-anhedonie en PO-psychose. Dit project stelt voor om deze onderzoekslijnen uit te breiden door gelijktijdige PET-MR te gebruiken om de effecten van transdermaal oestradiol, toegediend als een mechanistische sonde, op PO-anhedonie en PO-psychose te onderzoeken in een transdiagnostische steekproef van vrouwen die een dubbelblind placebo tussen groepen gebruiken. - gecontroleerd ontwerp. Dit monster wordt verrijkt voor anhedonie (d.w.z. op zijn minst milde anhedonie). Hoewel anhedonie en psychose dimensioneel zullen worden geanalyseerd, zal onze rekruterings- en stratificatiestrategie ervoor zorgen dat een reeks van symptoomernst (lichte tot matige of hoge PO-anhedonie; afwezige tot milde of matige PO-psychose) even evenwichtig en gerandomiseerd is voor elk experimentele groep (oestradiol of placebo). Onze centrale hypothesen zijn dat het mesolimbische DA-systeem is aangetast bij PO-anhedonie en psychose, dat toediening van oestradiol de neurale reacties op beloningen (gemeten met fMRI) en striatale DA-functioneren (gemeten met PET) zal normaliseren, en dat de mate van verandering in striatale functioneren zal worden geassocieerd met de mate van verandering in PO-anhedonie en PO-psychose.
Specifiek doel 1 (baseline-associaties tussen PO-anhedonie, PO-psychose en PET-MR): karakteriseer, bij baseline, associaties tussen PO-anhedonie en PO-psychose-symptoomernst en beloningsgerelateerde striatale activering gemeten met fMRI, en tonische en fasische striatale DA-activiteit gemeten met [11C]raclopride PET.
Specifiek doel 2 (estradioleffecten op PO-anhedonie en PET-MR): Bepaal de effecten van oestradiol (vs. placebo) op PO-anhedonie en veranderingen in PET-MR-statistieken met betrekking tot beloningsverwerking.
Specifiek doel 3 (estradioleffecten op PO-psychose en PET-MR): Bepaal de effecten van oestradiol (vs. placebo) op PO-psychose en veranderingen in PET-MR-statistieken met betrekking tot beloningsverwerking.
Dit project zal ons begrip van PO anhedonie en psychose en de werkingsmechanismen van het effect van oestradiol op PO anhedonie en psychose verbeteren. Dit onderzoek zal nieuwe mechanistische eindpunten opleveren om nieuwe PO-anhedonie- en psychosebehandelingen te evalueren die gericht zijn op het mesolimbische DA-systeem.

Inclusiecriteria:
- Verstrekken van ondertekend en gedateerd geïnformeerd toestemmingsformulier
- Verklaarde bereidheid om te voldoen aan alle onderzoeksprocedures, levensstijloverwegingen en beschikbaarheid voor de duur van het onderzoek
- 44-55 jaar oude niet-gemedicineerde perimenopauzale vrouwen met ≥ 2 overgeslagen menstruatiecycli, amenorroe ≥ 60 dagen, overeenkomend met de overgang in de late menopauze (Stages of Reproductive Aging Workshop (STRAW stadium -1).
- Anhedonie of psychosesymptomen die begonnen tijdens de periode van onregelmatige menstruatie.
- Clinician's Global Impression Scale-Severity score (CGI-S) > 3 om een klinisch gestoord monster te bevestigen.
- Anhedonie-inclusiecriteria en stratificatie: Alle deelnemers zullen Snaith-Hamilton Pleasure Scale (SHAPS)-scores > 20 hebben die overeenkomen met de NIMH Fast-Fail Trial for Mood and Anxiety Disorders, wat overeenkomt met anhedonie met een klinische beperking.
- Inclusiecriteria voor de ernst van de psychose en stratificatie: Deelnemers worden gestratificeerd volgens scores op de psychotische subschaal van de Brief Psychiatric Rating Scale (BPRS)
- Bereidheid om zich te houden aan het oestradiolregime
Uitsluitingscriteria:
- Zwangerschap; allergieën voor actieve of inactieve ingrediënten in de Climara®-pleister of Prometrium®.
- BMI < 18 of > 35 kg/m^2
- Een geschiedenis van chronische onregelmatige menstruatiecyclus, wat betekent > 1 jaar zonder menstruatie
- MR-contra-indicaties: Metaal in het lichaam, tandheelkundig werk anders dan vullingen of goud, tatoeages, metaalletsel, elk ander implantaat tenzij ze 100% plastic zijn.
- PET-tegenstrijdigheden: deelname aan >1 onderzoek in de afgelopen 12 maanden met ioniserende straling van meer dan 3 rem op het hele lichaam (bijv. PET, CT). Standaard beeldvorming is niet exclusief.
- Het gebruik van psychofarmaca of hormonale preparaten.
- Geschiedenis van psychiatrische ziekte gedurende de 2 jaar vóór het begin van de perimenopauze.
- Geschiedenis van chronische, terugkerende stemmings- of psychotische stoornissen (d.w.z. meer dan één niet-reproductieve stemmingsepisode voorafgaand aan de perimenopauzale indexepisode).
- Een geschiedenis van stemmingsepisodes waarvoor ziekenhuisopname nodig was.
- Huidige manie;
- Depressieve episode(n) binnen 2 jaar na inschrijving niet geassocieerd met de overgang naar de menopauze;
- Een geschiedenis van zelfmoordpogingen in het afgelopen jaar of huidige actieve zelfmoordgedachten met intentie en plan.
- Neurologische aandoeningen (bijv. geschiedenis van convulsies of TBI)
- Hersenstimulatiebehandeling in de afgelopen zes maanden.
- Endometriose;
- Eerstegraads familielid met premenopauzale borstkanker of borstkanker in beide borsten of meerdere familieleden (meer dan drie familieleden) met postmenopauzale borstkanker.
- Actueel medicatiegebruik (d.w.z. huidige psychofarmaca, huidige antihypertensiva, huidige statines, huidige hormonale preparaten of frequent gebruik van ontstekingsremmende middelen (> 10 keer/maand)). Er mogen vrouwen worden ingeschreven die medicijnen gebruiken zonder bekende stemmingseffecten (bijv. stabiele schildklierhormoonvervanging en incidenteel (< 5 keer/maand) gebruik van Ambien)*;
- Zwanger, borstvoeding gevend of proberen zwanger te worden;
- Laatste menstruatie meer dan 12 maanden voorafgaand aan inschrijving;
- Geschiedenis van niet-gediagnosticeerde vaginale bloedingen;
- Ongediagnosticeerde vergroting van de eierstokken;
- Polycysteus ovarium syndroom;
- Geschiedenis van borst- of eierstokkanker;
- Eerstegraads familielid met eierstokkanker;
- Abnormale bevinding in een borstonderzoek en/of mammogram van een provider;
- Bekende drager van BRCA1- of 2-mutatie;
- Porfyrie;
- Kwaadaardig melanoom;
- De ziekte van Hodgkin;
- Terugkerende migrainehoofdpijn die wordt voorafgegaan door aura;
- Galblaas- of pancreasaandoening**;
- Hart- of nierziekte**;

- Eerstegraads familielid met een voorgeschiedenis van een hartaanval of beroerte;

Allergie voor pinda's
alle gerapporteerde voorgeschreven medicijnen zullen worden beoordeeld en goedgekeurd door een onderzoeksarts voorafgaand aan de inschrijving van een deelnemer;
- deelnemers krijgen de gelegenheid om deze aandoeningen te beschrijven in de online screeningsenquête. Gerapporteerde aandoeningen die acuut van aard en/of goedaardig zijn, zullen worden beoordeeld door een onderzoeksarts en uitsluitingen zullen per geval worden beslist. Alle chronische aandoeningen zullen uitgesloten zijn. Voor degenen waarbij wordt aangenomen dat een uitsluiting niet van toepassing is, worden primaire analyses niet beïnvloed, maar worden verkennende analyses uitgevoerd met uitsluiting van deze personen

Karakteriseer beloningsgerelateerde striatale activering gemeten door fMRI met behulp van de Monetary Incentive Delay (MID) -taak om bloed-zuurstofniveau-afhankelijke (BOLD) reacties op te wekken.
Tijdens de MID-taak reageren de deelnemers op "winnende" proeven door zo snel mogelijk op een knop op een knoppenkastje in de MRI te drukken wanneer ze een doelwit zien.
Reactiviteit wordt gemeten door de verandering van de deelnemer in bloed-zuurstofniveau-afhankelijke (BOLD) respons op winnende proeven versus niet-winnende proeven te onderzoeken.
Vervolgens wordt de reactiviteit tussen de twee groepen vergeleken.

Bepaal de effecten van oestradiol (vs.
placebo) op PO-psychosesymptomen met behulp van de Brief Psychiatric Rating Scale (BPRS). De BPRS is een 18-item door een arts beoordeelde psychosemaat die de ernst van psychotische symptomen onderzoekt.
Personen met een psychotische depressie hebben typisch BPRS-positieve symptoomsubschaalscores van 7-11, en een score van 6 onderscheidt psychotische MDD het beste van niet-psychotische MDD.

We zullen basislijn niet-verplaatsbaar bindingspotentieel (BPND) en verandering in BPND na taakaanvang (ΔBPND%) gebruiken om verschillen tussen groepen te bepalen in termen van de basislijn (tonische) toestand en de activeringstoestand (fasische).
