この研究では、青少年の睡眠障害と自殺リスクのマーカーを減らすように設計された多面的な介入である TAILOR の有効性をテストします。 TAILOR介入には、認知行動療法、動機付け面接、最近自殺念慮のある青年に対する音声またはビデオ通話ベースの支援を含む、成人の睡眠および/または思春期の非睡眠行動に対処するための3つの異なる経験に基づく行動変容アプローチが組み込まれます。しかし自殺未遂歴なし。
仮説:
- 青少年と保護者/法定後見人 (P/LG) の両方の報告によると、TAILOR 介入を受けた青年は、生後 2 か月と 4 か月の強化通常ケア (EUC) 状態の青年と比較して、睡眠が改善されます。
- TAILOR の介入により、生後 4 か月の EUC 状態と比較して、自殺念慮が大幅に減少します。
探索的二次的目的:
- (a) TAILOR 介入への反応が、性別、人種/民族、投薬状況、または不眠症の種類 (例: 眠りにつくことと眠り続けることの困難) によって異なるかどうか、および (b) TAILOR が睡眠と自殺以外の他の健康リスク行動領域に影響を与えるかどうかを判断します。観念。

同意の時点で年齢が 12 歳 0 か月から 17 歳 6 か月で、睡眠障害 (過去 2 週間以内) および自殺念慮 (過去 90 日以内) のある 190 人の若者が、いずれかのテーラーに無作為に割り付けられます。 (n=95) または EUC (n=95) に。 自殺念慮とは、自殺について考えたり、いつ、どこで、どのように自殺するかを熟考することを指します。 そのため、自殺念慮から脅迫、未遂、自殺に至るまでの自殺行動の重症度の範囲において、自殺念慮は近位にあると考えられています。 したがって、TAILOR の主な目標は、自殺念慮を排除するか、少なくとも最小限に抑えることです。

参加者の半分は、この研究の実験群に無作為に割り付けられます。
治験担当医師は、各家族に 2 か月間で約 4 回電話をかけます。

その後、TAILOR 介入が行われます。
TAILOR には、既存の睡眠問題と睡眠習慣の評価、不眠症に対する認知行動療法 (CBT) 戦略に関する教育が含まれ、コミュニケーション スタイルとして動機付け面接 (MI) が使用されます。

最初の TAILOR セッションでは、思春期の若者と親/法的保護者の両方の視点から既存の睡眠の問題を評価し、CBT 戦略を試すことで締めくくります。
