この研究では、APBI(加速乳房部分照射)と呼ばれる補助放射線療法レジメンを使用します。
APBI は、早期の低リスク乳がんと考えられる女性が利用できる治療オプションです。
標準的な外部ビームベースの APBI 治療では、隔日で 5 回の治療が行われ、10 日間にわたって治療が行われます。
この研究のAPBIは修正されており、標準的な外部ビームベースのAPBI治療と比較して、毎日わずかに低い線量で、連続した治療日に1日1回の治療を5回行います。

これは、乳がん参加者における前向き介入コホート研究です。
この研究の目的は、医師が評価した化粧品の結果を主要な研究のエンドポイントとして追跡し、これらの結果を、良好なリスクの初期段階の乳癌を持つ女性が一連のアジュバント WBI を受けた過去の試験と比較することです。

- -書面によるインフォームドコンセントを提供する能力。
- -Eastern Cooperative Oncology Group(ECOG)のパフォーマンスステータスが3以下、またはカルノフスキーのパフォーマンススコアが30%以上。
- -フォローアップのためにサンフォードがんセンターに戻る意欲。
- 放射線療法は、日常ケアの一環として計画されています。
- 組織学的検査では、腫瘍は非浸潤性乳管癌(DCIS)または乳房の浸潤性乳管癌のいずれかでなければなりません。
- -患者は50歳以上でなければなりません。
- 単発性腫瘍の存在。
- 乳房の外科的治療は乳腺腫瘤摘出術であったに違いありません。 必須ではありませんが、腫瘍摘出ベッドの輪郭を描くために外科用クリップを配置することを強くお勧めします。
- 切除標本の最終マージンには、組織学的に腫瘍がないようにする必要があります (浸潤性腺癌の場合は 2 mm 以上、DCIS の場合は 3 mm 以上)。 マージンの再切除は許可されています。 肉眼的病変は単発性で、病理学的浸潤性腫瘍のサイズが 2.0 cm 以下、または病理学的 DCIS サイズが 2.5 cm 以下である必要があります。
- 浸潤性乳がんの患者は、センチネルリンパ節生検からなる腋窩病期診断を受ける必要があります。 センチネル リンパ節は、通常の H&E 評価と免疫組織化学染色の両方で、局所転移性疾患が陰性でなければなりません。 DCIS 患者には、腋窩病期分類は必要ありません。
- リンパ管腔浸潤の欠如。
- 腫瘍は、エストロゲンまたはプロゲステロン受容体の存在が陽性で、免疫組織化学染色で HER-2/neu 受容体が 0 または +1 でなければなりません。
- 対象の乳腺腫瘤摘出術の空洞は明確に描写されている必要があり、CT シミュレーション スキャンに基づいて、乳房全体の参照体積に対する目標の乳腺腫瘤摘出術の空洞の比率が 30% 以下である必要があります。
- -乳がんの診断を除く5年以上の平均余命(PIによって考慮される併存疾患)
- 出産の可能性のある女性は、CT シミュレーションの前に妊娠検査で陰性でなければならず、放射線治療コース中に効果的な避妊法を使用することに同意する必要があります。
除外基準:
- 妊娠中の女性、または妊娠の可能性のある女性で、適切な避妊をしたくない人。
- -核グレード3のDCISまたはノッティンガムグレード3の浸潤性乳管がん。
- グレードを問わない浸潤性小葉がん。
- 男性のセックス。
- -放射線療法による治療、および/または研究登録前に現在診断されている乳がんに対して投与されたホルモン療法
- -何らかの理由での以前の乳房または胸部放射線治療。
- -以前の浸潤性乳がんまたはDCISの病歴。 (手術のみで治療されたLCISの既往のある患者が対象です。)
- 乳頭のパジェット病。
- 同側または対側の腋窩、鎖骨上、鎖骨下、または内胸腺リンパ節の触知可能または X 線検査で疑われるもの。これらのリンパ節が腫瘍陰性であるという組織学的確認がない場合。
- コラーゲン血管疾患の患者;具体的には、SLE または強皮症です。
- 豊胸手術。 (インプラントを除去された患者が対象です。)
- -治験責任医師の意見では、患者が研究要件を満たすことを妨げる精神障害または中毒性障害またはその他の状態。

実験的:IMRT/VMAT ベースの乳房部分加速照射
患者は、新しい IMRT/VMAT ベースの加速部分乳房照射レジメンで治療されます。
|
乳腺腫瘤摘出ベッドは、5 分割で合計 26 Gy を受け取り、1 日 1 分割が送達されます。
理想的には、放射線療法のコースは連続した 5 日間の治療で完了します。
治療は、乳腺腫瘤摘出術または外科的マージンの再切除から 12 週間以内に開始し、10 暦日以内に完了する必要があります。

BCTOには22のアイテムが含まれており、3つの内部的に一貫したサブスケールに割り当てられます:1)機能状態、2)美的ステータス、3)乳房感受性ステータス。
患者は、治療された乳房と未処理の乳房の違いを評価する4点スケールでBCTOの各項目を評価するように指示されます(1 =差、4 =大きな差)。
各サブスケールのスコアは、そのサブスケールに属するすべてのアイテムの評価の平均です。
より高いスコアは、ステータスが低いことを反映しています(つまり、治療されたものと未処理の乳房の間に大きな違いがあります)。
また、この調査では、患者は乳がん治療と結果に満足していることも可能にします。
