過活動膀胱 (OAB) は、他の病状がないにもかかわらず、頻尿、尿意切迫感、失禁、および夜間頻尿を特徴とします。 AUA ガイドラインでは、行動修正や薬物療法による治療が失敗した難治性 OAB 患者に対して、仙骨神経調節 (SNM) の試みを推奨しています。 SNM 設定は通常、患者とデバイス担当者の間の話し合いに基づいて選択されます。 症状改善のための最適な SNM 刺激プログラムを評価する厳密なデータは不足しています。
これは、OABのためにSNMを受けている患者における周期的仙骨神経調節(SNM)と持続的仙骨神経調節(SNM)を比較する、前向きのランダム化対照多施設試験となる。
難治性OABと診断され、SNMの治験(ステージIのリード/一時的バッテリー留置またはオフィス末梢神経評価のいずれか)を予定している18歳以上の患者は、クリニック訪問中に治験担当者から、または電話で問い合わせを受け、彼らはプログラミング学習に参加することに興味を持っています。
参加に関心のある患者は書面によるインフォームドコンセントを提出します。 インフォームドコンセントが得られた後、ベースライン評価が実行されます。 これには、病歴、3 日間の排尿日記 (排尿エピソード数、夜間頻尿エピソード数、切迫性失禁エピソード数/24 時間)、尿検査、尿培養 (指示がある場合)、排尿後の残存物、およびアンケートを使用した症状評価が含まれます。
トライアルに成功し、永久的な SNM 配置に適格または興味がある人のみがこのトライアルに参加できます。 ランダム化は、ステージ 2/完全永久移植後、神経調節剤が回復室でプログラムされているときに行われます。 患者は、連続刺激または周期刺激のいずれかにランダムに割り当てられます。
その後、術後 1 か月、6 か月、12 か月後に患者の臨床評価とアンケートが繰り返されます。

- 永続的な SNM の基準を満たしており、永続的な SNM を計画しています。 UUI エピソードが 50% 以上減少した SNM のステージ I またはオフィス PNE 試験の成功)

除外基準:
- 18歳未満の患者

- 関連する神経疾患(多発性硬化症、パーキンソン病、重症筋無力症、シャルコー・マリー・トゥース病、完全脊髄損傷)
