胃腸の健康を維持することは、腹膜透析を受けている人にとって最も重要なことですが、健康管理のより広い範囲内では気づかれないこともよくあります。 透析治療の有効性は、栄養摂取や生活の質などの側面とともに、胃腸障害によって悪影響を受ける可能性があります。 腹膜腔内の体液の量、透析に関連する薬剤、および透析自体の手順はすべて、胃腸の健康状態の調節に役割を果たすことが知られています。
慢性疾患を持つ人々の胃腸の健康を高める上での身体活動の役割は、時間の経過とともに認められるようになりました。 カスタマイズされた運動計画は、これらの患者の胃腸症状、栄養素吸収効率、および全体的な生活満足度を著しく改善することが示されています。 特に、腹膜透析を受けている人の胃腸機能に対する運動の具体的な影響はまだ調査されていません。

説明
包含基準:
- 患者は3か月以上腹膜透析を受けている必要があります。
- 年齢は18歳から65歳まで。
- スマートフォンを所有し、その使い方に習熟していること。
- 同意書に署名することにより、自発的にインフォームドコンセントを提供する意欲があること。
除外基準:
- 最近の最高血圧が180 mmHg以上、または最低血圧が110 mmHg以上、または過去1週間以内の最高血圧が90 mmHg以下、または最低血圧が60 mmHg以下である。
以下を含むがこれらに限定されない重度の心血管疾患の存在:
- ニューヨーク心臓協会 (NYHA) の機能グレード IV ~ V に分類される心不全。
- 第3度房室ブロック、洞不全症候群、発作性上室頻拍、心室頻拍などの重度の不整脈。
- 不安定狭心症。
- 肺動脈圧が25 mmHg以上の肺動脈性肺高血圧症。
- 重度の心嚢液貯留、弁狭窄、肥大型心筋症、または大動脈解離。
重度の肺疾患には以下が含まれますが、これらに限定されません。
- 重度の慢性閉塞性肺疾患(COPD)。
- 肺塞栓症。
- 肺癌。
- 重度の肺感染症。
- 深部静脈血栓症の存在。
- 運動の遵守を妨げる重度の神経疾患、筋肉疾患、骨疾患、または関節疾患。
- 他の並行運動プログラムへの参加。

GIQLIには、消化器症状(19項目)、精神状態(5項目)、身体機能(7項目)、社会機能(4項目)、医療上の不都合(1項目)の5つの領域が含まれています。
各項目のスコアは 0 ~ 4 で、合計スコアの範囲は 0 ~ 144 です。
スコアが高いほど生活の質が高いことを示します。

胃腸症状評価スケール (GSRS) は、上部および下部の胃腸症状の影響を評価することを目的とした 15 の質問で構成されています。
これには 5 つの側面 (胃食道逆流、下痢、便秘、腹痛、消化不良の症状) が含まれており、各質問は 0 (不快感なし) から 6 (非常に重度の不快感) までスコア付けされます。
GSRS スコア ≥1 は、胃腸症状の存在を定義します。スコアが高いほど、より重篤な胃腸症状を示します。

胃食道逆流症、下痢、便秘、腹痛、消化不良の症状を含む 5 つの側面から評価します。
各質問は 0 (不快感なし) から 6 (非常にひどい不快感) までスコア付けされます。
GSRS スコアが 1 以上は胃腸症状の存在を示し、スコアが高いほど症状が重篤であることを示します。

6-MWTの変化

6 分間歩行テスト (6-MWT) は、2 つの分岐点でマークされた 10 メートルの経路に沿って実施されます。
患者は道に沿って6分間往復するように指示され、合計の歩行距離が測定されます。
テスト期間はストップウォッチを使用して監視されます。
達成した歩行距離に基づいて、患者の心不全の重症度は次のように分類されます。150 メートル未満は重度の心不全を示します。 150〜425mは中等度の心不全を示します。 426〜550mは軽度の心不全を示します。
