- CYSHCN スクリーナーで定義されている、特別な医療ニーズを持つ児童または青少年。身体的、感情的、行動的、発達的、またはその他の健康状態が持続している、または継続しているために、1 つ以上の機能制限またはサービス利用を経験している児童を特定します。少なくとも12か月続くと予想されます。

参加者は、通常は診療所内で提供されるケアと、医療リソースを備えた標準パッケージを受けます。
実用的な設計に沿って、ナビゲーションやケアの調整、医療や社会サービス、地域リソースへの紹介パターンに対して提供されるサポートにもバリエーションが生まれます。
ただし、サイト間で発生する可能性のある影響の潜在的な違いについてのコンテキストを提供するために、これらの違いを文書化します。

実験的:患者ナビゲーター
参加者は患者ナビゲーターとペアになります。
患者ナビゲーターは参加者に連絡します。 1) 無作為化から 7 日以内。 2) 少なくとも 3 か月に 1 回、最大 12 か月に 1 回。 3) 必要に応じて、以前の訪問で特定されたタスクをフォローアップします。 4) 参加者によって開始された場合。
接触は、クリニックへの直接来院時(可能な場合は初対面を優先)、またはビデオ、電話、SMS、および/または電子メールで行われます。
ナビゲーターは、参加者のニーズに応じてこれらの対話中に取り上げられるトピックを調整し、患者、家族、臨床医からの意見をもとにケアプランを作成します。
さらに、介入の忠実度を評価するために、患者ナビゲーターは、取り上げられたトピック、提供されたサービスと調整、提供された書面によるケアプラン、同時にアクセスされたリソース/介入(例:ソーシャルワーカー、資金提供/住宅申請、コミュニティ組織からのサポート)。

患者ナビゲーターは次の要件を満たします: 健康科学または社会科学の学士号、または同等の経験。地域の医療および社会サービスシステムに関する豊富な知識。トラウマに基づいたケアと文化的安全。強力な対人スキル、問題解決能力、自主性。移民コミュニティでの経験(実際の経験を含む)と医療現場での勤務経験がある。
ナビゲーターは、他のナビゲータートライアルと同様に、ケースディスカッションやロールプレイングを通じて、自分の役割に関連するトピックについてトレーニングを受けます。
トピックには、移民の健康に適用される先住民の文化的安全、システムのナビゲーション、ケアの調整、家族中心のケア、健康の社会的決定要因(健康保険、収入補助など)と不平等、高度なコミュニケーションと援助スキル(例:動機付け面接)などが含まれます。 )、倫理、プライバシー、機密保持、および共同設計プロセスを通じて決定されたその他の事項。

BCQ は、特別な医療ニーズを持つ子供や若者向けに検証されています。
