葉酸受容体α陽性の進行性卵巣がんおよびその他の固形腫瘍を有する成人参加者におけるTAK-853の研究
葉酸受容体α陽性進行卵巣がんおよびその他の固形腫瘍を有する日本人患者を対象としたミルベツキシマブ ソラフタンシン(TAK-853)の安全性、忍容性、有効性および薬物動態を評価するための第1/2相非盲検試験
この研究の主な目的は、TAK-853による副作用の有無を確認すること、参加者が副作用を発症せずにTAK-853の摂取量を確認すること、TAK-853が症状をどの程度抑制しているかを確認すること、TAK-853の量を確認することです。時間が経っても血液中に残ります。
研究はフェーズ 1 パートとフェーズ 2 パートの 2 つのフェーズに分けて実施されます。 フェーズ 1 パートでは、参加者はいくつかの検査のため、および治療による副作用を確認するために、最初の注射後少なくとも 3 日間は病院に滞在します。 フェーズ 2 パートでは、参加者は研究対象の病院を複数回訪問します。 どちらのフェーズでも、参加者は 3 週間の各サイクルの最初の日に TAK-853 を投与されます。
参加者はフェーズ 1 パートで約 9 か月間、フェーズ 2 パートで約 24 か月間研究に参加します。 研究担当医師は、研究治療による副作用がないか確認します。

フェーズ 1 の部分:
診断、許容される以前の治療法、および疾患の測定可能性の要件:
すべての参加者は、葉酸受容体アルファ (FR アルファ) を発現することが知られている進行性固形腫瘍の後に、標準治療に抵抗性または難治性であり、標準治療が利用できない、または参加者が標準治療を拒否していることが病理学的に証明されている必要があります。
- 卵巣がん
- 子宮内膜癌
- 非小細胞肺がん (NSCLC)
- トリプルネガティブ乳がん (TNBC)
- 胆管がん
- 結腸直腸がん (CRC)
- 胃食道腺癌 注: 上記以外の固形腫瘍タイプの参加者は、腫瘍 FR α 発現に関する事前の文書がある限り参加資格があります。
- 免疫組織化学 (IHC) による腫瘍 FR α 発現の事前文書のないすべての参加者は、アーカイブ腫瘍組織ブロックまたはスライドを提供するか、IHC で FR α を確認するための低リスクの医学的に日常的な手順を使用して新しい生検を取得する手順を受けなければなりません。フェーズ 1 パートに入るために、生存腫瘍細胞の 1% 以上が陽性であり、膜染色強度が 1+ 以上である
- 参加者が以前に受けた可能性のある細胞毒性または標的療法の数に上限はありません。 参加者は、葉酸受容体を標的とする治験化合物による以前の治療を受けている可能性があります。
- 参加者は、固形腫瘍における反応評価基準(RECIST)1.1に従って、測定可能な疾患または測定不可能な疾患(正確に測定できない大きな腹部腫瘤など)を患っていなければなりません。
- 参加者は、Eastern Cooperative Oncology Group のパフォーマンス ステータス (ECOG PS) が 0 または 1 である必要があります。
以前の治療からの経過時間:
- 全身抗腫瘍療法: 5 半減期または 4 週間のいずれか短い方 (以前のニトロソウレアまたはマイトマイシン C の場合は 6 週間)
- FR アルファ標的療法: 5 半減期または 4 週間のいずれか長い方
- 放射線療法: 広視野放射線療法 (例: 治験薬開始前に少なくとも4週間完了した、または少なくとも2週間完了した局所放射線照射
- 参加者は、以前のすべての治療関連毒性から安定化または回復(グレード 1 またはベースライン)していなければなりません
- 大手術は、TAK-853の初回投与の4週間前に完了する必要があります。 参加者は研究治療前に副作用から回復または安定していなければなりません。

フェーズ 2 の部分:
- 参加者は高悪性度漿液性上皮性卵巣がん、原発性腹膜がん、または卵管がんの確定診断を受けている必要があります。
参加者はプラチナ耐性疾患を患っている必要があります。
- プラチナベースの治療を1ラインのみ受けた参加者は、少なくとも4サイクルのプラチナ治療を受けていなければならず、奏効(完全奏効[CR]または部分奏効[PR])を示し、その後3か月以上から<プラチナの最終投与日から6か月後
- 2 または 3 ラインのプラチナ療法を受けた参加者は、プラチナ療法の最後の投与日から 6 か月以内に進行していなければなりません 注: 進行は、プラチナ療法の最後の投与日からプラチナ療法の投与日まで計算する必要があります。進行を示す X 線画像 注: 第一線治療中に原発性プラチナ不応症となった参加者は除外されます (除外基準を参照)
- 参加者は、最新の治療中または治療後にX線検査で進行が確認されている必要があります
- 参加者は、アーカイブ腫瘍組織ブロックまたはスライドを提供するか、Ventana FOLR1 で定義されている FR α 発現 (「陽性」と報告される) の IHC 確認のための低リスクの医学的に日常的な手順を使用して新しい生検を取得する手順を受ける意欲がなければなりません。アッセイ。 フェーズ 2 に入るには、生存腫瘍細胞の 75% 以上が FR α 陽性であり、膜染色強度が 2+ 以上であることによって腫瘍が FR α 高であることを確認する必要があります。

- 活動性または慢性の角膜障害、角膜移植の病歴、または制御不能な緑内障、硝子体内注射を必要とする滲出性加齢黄斑変性症、黄斑浮腫を伴う活動性糖尿病性網膜症、黄斑変性症、乳頭浮腫などの継続的な治療/モニタリングを必要とする活動性の眼疾患のある参加者、および/または単眼視。
以下を含むがこれらに限定されない、重篤な併発疾患:
臨床的に関連する活動性感染症:活動性B型肝炎またはC型肝炎感染(積極的な抗ウイルス療法を受けているかどうかに関係なく)
- ヒト免疫不全ウイルス (HIV) 感染症
- 活動性サイトメガロウイルス感染症
- 活動性の新型コロナウイルス感染症/SARS-CoV-2感染症。 SARS-CoV-2 検査は研究参加に必須ではありませんが、検査は地域の臨床診療ガイドラインと基準に従う必要があります。
-治験薬開始前2週間以内に抗生物質の静注を必要とする他の既知の併発感染症 注:肝炎のスクリーニング時の検査は必須ですが、上記の残りの感染症については、臨床的に示されない限り必要ありません。 B 型肝炎表面抗原血清陽性がわかっている参加者、および/または C 型肝炎ウイルス RNA が検出可能な参加者は除外されます。 B 型肝炎コア抗体および/または B 型肝炎表面抗体が陽性の参加者も登録できますが、血清 B 型肝炎ウイルス DNA が検出されない必要があります。 C 型肝炎ウイルス抗体が陽性の参加者は、C 型肝炎ウイルス RNA 血清レベルが検出不能でなければなりません。 参加者は、免疫抑制療法または化学療法によるB型肝炎ウイルス感染の再活性化の予防に関するガイドライン(日本肝臓学会2022年)に従ってモニタリングおよび管理されます。
臨床的に重大な心疾患を患っている参加者。以下のいずれかが含まれますが、これらに限定されません。
- 心筋梗塞 = 研究薬の初回投与前6か月未満
- 不安定狭心症
- 制御不能なうっ血性心不全 (ニューヨーク心臓協会 > クラス II)
- コントロール不良 >= グレード 3 高血圧 (NCI CTCAE v5.0 による)
- 制御不能な不整脈
- 重度の大動脈弁狭窄症
- >= 以前の化学療法後のグレード3の心臓毒性
- -多発性硬化症または他の脱髄疾患の病歴、ランバート・イートン症候群(腫瘍随伴症候群)、過去6か月以内の出血性脳卒中または虚血性脳卒中の病歴、またはアルコール性肝疾患。
- -肺炎を含む間質性肺疾患(ILD)の以前の臨床診断。
- 免疫療法、生物療法、放射線療法、化学療法、調査療法、または高用量ステロイドなどの他の併用抗がん治療。ただし、前立腺がんの参加者には、低用量のステロイドおよび黄体形成ホルモン放出ホルモン (LHRH) の用量が 14 日以上安定している場合は許可されます。

フェーズ 2 の部分:
- 類内膜、明細胞、粘液性または肉腫性の組織型、上記の組織型のいずれかを含む混合腫瘍、または低悪性度または境界線の卵巣腫瘍を有する参加者
- 原発性プラチナ不応性疾患を患っている参加者。第一選択のプラチナ含有化学療法の最後の投与から3か月以内に反応しなかった疾患(CRまたはPR)、または進行した疾患として定義されます。
- 過去に骨髄の少なくとも20%に影響を与える広視野放射線治療を受けた参加者
- NCI CTCAE v5.0によるグレード1以上の末梢神経障害のある参加者

ORR was defined as the percentage of participants who achieved a confirmed Partial Response (PR) or confirmed Complete Response (CR) during the study using RECIST 1.1.
Complete response (CR): Disappearance of all target lesions.
All pathological lymph nodes (whether target or non-target) must have reduction in short axis to < 10 millimeters (mm).
Partial response (PR): At least a 30% decrease in the sum of diameters (SoD of target lesions, taking as reference the baseline SoD).

Blood samples were collected to measure the presence of TAK-853 antibodies (ADA).
Seronegative was defined as a participant with negative ADA at baseline and negative ADA at post-treatment.
Treatment-emergent ADA was defined as a participant with negative ADA at baseline and positive ADA at post-treatment.
Treatment-unaffected ADA was defined as a participant with positive ADA at baseline and post-dose titer increase that was less than or equal to 4-fold compared to baseline.
Treatment-enhanced ADA was defined as a participant with positive ADA at baseline and post-dose titer increase that was greater than 4-fold compared to baseline.
