幼い小児がんサバイバーのための生活と学業の準備 (PLAY)
十分なサービスを受けられていない地域の小児がん生存者の神経発達成果を改善するためのデジタル医療介入
この臨床試験の目的は、若年小児がん生存者の介護者向けのオンライン プログラム「Preparing for Life and Academics for Young Survivors (PLAY)」と呼ばれるプログラムをテストし、改善することです。 PLAY プログラムは、がんを患う幼児の介護者と医療従事者のグループとともに作成されました。 最終的には、このプログラムが前向きな子育て行動を改善し、介護者のストレスを軽減し、幼い子どもたちの学校への準備に役立つかどうかを確認したいと考えています。 この臨床試験の準備段階では、次の質問に答えることを目指しています。
- 小児がん生存者の介護者は、PLAY プログラムに参加し、プログラムの前後に評価を完了する意欲がありますか?
- 小児がん生存者の介護者は、PLAY プログラムが使いやすいと評価していますか?
- 小児がん生存者の介護者はPLAYプログラムに満足していますか?
- PLAY プログラムは今後どのように改善できるでしょうか?
小児が維持期治療に移行する場合、またはがん治療を中止する場合、介護者は研究の開始時に参加して調査に回答するよう招待され、また、ビデオ撮影された小児との対話課題にも参加します。 子供たちはプログラムの開始時に簡単な発達検査を受けます。
その後、介護者は PLAY プログラムを完了します。これには、毎週または隔週で 7 つのオンライン モジュールを完了し、訓練を受けたコーチとビデオ会議で 1 時間のセッションを 3 か月間で最大 8 回行うことが含まれます。 セッションでは、お子様の学校への準備を支援し、家族がストレスに適応して対処できるよう支援することに焦点を当てます。
介護者は、PLAY プログラムが完了した 3 か月後、再度調査とビデオ撮影された対話タスクを繰り返します。 また、プログラムでの経験について学ぶための面接にも参加するよう招待されます。

介護者は子供の法的保護者です(母親、父親、祖父母、養親など) 介護者は 50% 以上の時間子供と一緒に住んでいます 介護者の主言語は英語です 介護者はナショナル小児病院から 250 マイル以内に住んでいます 小児がん経験者が治療を受けていますがんの治療(化学療法、外科的切除、放射線療法など)を行っている小児がんサバイバーががん治療を完了したか、証拠なしで維持療法のみを受けている小児がんサバイバーは現在3歳から6歳の間であるが、研究登録時に6.5歳以下である
除外基準:
小児がんサバイバーはがん診断前に神経発達障害を記録している(神経線維腫症、結節性硬化症、ダウン症候群、自閉症など) 小児がんサバイバーはいかなる治療も受けていない(化学療法、外科的切除、放射線など) 小児がんサバイバーは適切な治療を受けている良性腫瘍

実験的:若年生存者のための生活と学業準備プログラム(PLAY)
PLAYは、小児がん生存児(3〜6歳)の養育者を対象としたデジタルヘルス介入プログラムで、7つの自己管理型ウェブモジュールと、週1回から隔週で実施される最大10回のバーチャルコーチングセッション(7回のペアセッションと最大3回のブースターセッションを含む)で構成されています。
モジュールの内容は心理教育とポジティブな育児スキルに焦点を当てており、コーチングではポジティブな育児スキルの実践に重点を置いています。

PLAY プログラムは、小児がん生存者の介護者のために作成された Web ベースのプログラムです。
このプログラムは次のトピックを持つ 7 つのモジュールで構成されています: 1) プログラムの紹介 2) セルフケア 3) 人間関係 4) 学校の準備 5) 一緒に話したり、読んだり、数を数えたり 6) 社会的および感情的な発達 7) 復習モジュール。
このプログラムは約 3 か月かけて完了する予定で、プログラムに参加する介護者は毎週または隔週でコーチと会い、モジュールで学んだこととそれを日常生活でどのように実践するかについて話し合います。

介護者は、観察された肯定的な養育行動を測定するために、少なくとも10分間子供と遊んでいる様子を記録し、それは時間点について盲検化された訓練を受けた評価者によって標準化されたプレイ・アンド・ラーニング・ストラテジーズ(Play and Learning Strategies)コーディング方式で評価されます。
このコーディング方式は、肯定的な養育行動(すなわち、温かさ、状況に応じた応答性、維持、言語的足場掛け、実演・身体的指導)について1から5のリッカート尺度評価を含みます。
