背景:シンガポールの2型糖尿病(T2DM)の若年成人は、血糖コントロールと薬物順守が劣る傾向があり、糖尿病合併症のリスクが高い要因と相関しています。 後期段階糖尿病とその合併症は、ゆっくりと進行しており、すぐに生命を脅かすものではないと認識されています。 これらは、シンガポールの糖尿病の制御とリスク認識の状態を反映しています。 健康信念モデルを使用して、リスクコミュニケーション介入であるピア比較ベースのリスクコミュニケーション(PRICOM)が開発されました。 Pricomは、糖尿病合併症のリスク認識を高めることにより、健康行動を促進することを目指しています。
特定の目的:主な目的は、T2DM制御不良の成人のプライマリケア設定にプリコムを提供する可能性を評価することです - 募集、保持、プリコムの遵守、リスク認識、健康アクション、およびHBA1Cのスコアの変化の推定効果サイズの観点から変化。 二次的な目的は、リスク認識スケールの妥当性を評価し、患者のプリコムの視点を探求することです。
方法論:マルチサイトのランダム化比較試験は、2つの同時段階のプライマリケアクリニックで実施されます。 フェーズ1では、リスク認識スケールのコンテンツ検証とテスト再テストの信頼性が実行されます。 フェーズ2では、混合メソッドを使用して2腕のランダム化比較試験が実施され、プリコムの提供の実現可能性が評価されます。 合計40人の患者参加者が便利なサンプリングによって募集されます。 適格な患者は40〜79歳で、過去6か月以内に少なくとも1つのHBA1Cが8.0%以上読み取られ、T2DMコントロールが不十分でなければなりません。

患者参加の資格基準:
- 少なくとも12か月間、それぞれの研究サイトでのフォローアップのT2DM
- 40歳から79歳
- 過去6か月以内に少なくとも1つのHBA1Cリーディング≥8.0%
- 英語を読んで話すことができます
患者参加の除外基準:
- シンガポール市民や永住者ではありません
- 妊娠中
- 末期腎臓病または腎補充療法
- 既知の末期疾患
- 視覚および/または聴覚障害
- 認知障害または精神疾患

実験的:介入
介入グループは、調査チームが提供するリスクコミュニケーションセッションに参加し、糖尿病のパンフレットを受け取ります。
これは10〜15分かかると推定され、患者の日常的なクリニックの予約に連続しています。

健康信念モデルに基づいて、リスクコミュニケーション介入、ピア比較ベースのリスクコミュニケーション(PRICOM)が、血糖コントロールと発生する可能性のある合併症についてT2DMの患者に助言し、改善する方法を推奨するために、プライマリケア医師向けに開発されました。血糖コントロールと合併症(またはさらなる合併症)を防ぎます。
これは、AI(人工知能)ベースの糖尿病ツールからの情報によってサポートされます。
Pricomは、糖尿病合併症のリスク認識を高めることにより、T2DM患者の健康行動を促進することを目指しています。
