尋常性痤瘡患者における局所用ダプソンゲルの臨床的有効性、皮脂産生および紅斑への影響に関する無作為化二重盲検プラセボ対照評価
この無作為化二重盲検プラセボ対照臨床試験の目的は、成人の尋常性ざ瘡治療における外用7.5%ダプソンゲルの有効性を検証することです。
主な研究課題は以下の通りです:
- ダプソンゲルはプラセボ(ベヒクル)ゲルと比較して、にきび病変の数と重症度を減少させるか?
- ダプソンゲルは、メキサメーターで測定した皮膚の発赤(紅斑)と色素沈着(メラニン)を改善し、セブメーターで測定した皮脂分泌を減少させるか?
- ダプソンゲル治療は患者の生活の質を改善するか?
研究者は、1日1回のダプソンゲルと外観が類似したプラセボゲル(有効成分なし)を比較します。
参加者が行うこと:
- ダプソンゲルまたはプラセボゲルを1日1回、8週間塗布する
- ベースライン(開始時)、4週目、8週目に来院する
- 各来院時ににきび病変を数え、重症度を評価する
- 予め設定された顔面部位(両頬と眉間)でメキサメーターによる紅斑とメラニンの測定、およびセブメーターによる皮脂測定を受ける
- ベースライン時と8週目に生活の質アンケートを記入する
- 各来院時に標準化された写真を撮影する

本研究は、尋常性ざ瘡の治療における局所7.5%ダプソンゲルの臨床的有効性を評価するために設計された前向き、無作為化、二重盲検、プラセボ対照臨床試験である。 本研究では、標準化された機器を用いた皮膚生物物理学的パラメータへの影響も調査する:メキサメーターによる紅斑とメラニン、およびセブメーターによる皮脂産生。
2つの並行群が含まれる:1群は7.5%ダプソンゲルを1日1回塗布し、もう1群は外観と包装が同一であるが有効成分を含まないビヒクル(プラセボ)ゲルを塗布する。 無作為化はコンピューター生成され、研究者と参加者の両方が治療割り付けについて盲検化された状態を維持する。
参加者は、ベースライン(0日目)、4週目、および8週時に診察を受ける。 各診察時には、標準化されたデジタル写真が撮影され、病変数が計数される。 病変評価には、Plewig & Kligmanに基づく面皰性および丘疹膿疱性病変数、グローバルざ瘡評価システム(GAGS)、および研究者全体評価(IGA)が含まれる。 各診察時には、標準化された順応期間後に、3つの事前定義された顔面部位(右頬、左頬、眉間/額)でメキサメーター測定値(紅斑とメラニン)およびセブメーター測定値(皮脂)が三重測定で取得される。 ざ瘡生活の質指数は、ベースラインと8週時に実施される。

説明
包含基準:
- 尋常性ざ瘡の臨床診断を受けた18歳から50歳の成人。
- 過去1ヶ月(30日)以内に全身的または局所的なざ瘡治療を受けていない。
- 書面によるインフォームドコンセントを提供する能力と意思がある。
除外基準:
- 妊娠中または授乳中。
- 過去6ヶ月以内の全身性レチノイドまたは全身性抗生物質の使用。
- 研究参加または評価に干渉する可能性のある活動性皮膚疾患または全身疾患。
- ダプソンまたは研究製品のいずれかの成分に対する既知の過敏症。
