トラウマ生存者のためのピアサポートプログラムのパイロット実現可能性試験
自動車事故、転倒、火災などの出来事による外傷は、障害の主要な原因の一つです。 生存者は、身体的課題だけでなく、長期にわたる痛み、メンタルヘルスの課題、仕事や家庭生活への復帰の困難にも直面することがよくあります。 医療ケアは身体的回復に焦点を当てていますが、生存者は感情的・社会的ニーズが見落とされ、満たされていないと頻繁に報告しています。
同様の外傷を経験した人々からのピアサポート(指導)は、苦痛を軽減し、対処能力を向上させることができます。 しかし、カナダでは現在、病院からリハビリテーション、地域生活までの回復の旅全体を通じて、外傷生存者にピアサポートを提供するプログラムは存在しません。
研究者たちは、外傷生存者、介護者、医療提供者と協力して、これらの段階全体を通じてサポートを提供するピアサポートプログラム(PSP)を共同で設計しました。 このパイロット研究では、プログラムの実現可能性と受容性をテストし、回復への初期影響を探ります。 結果は、将来のより大規模な試験を導き、外傷ケアにおけるピアサポートの拡大に役立てられます。

参加基準:
- サニーブルック病院で急性期治療を受け、その後セント・ジョンズ・リハビリテーションに退院する予定の、偶発的な外傷による入院を経験した成人患者(18歳以上)。
- 英語でのコミュニケーションが可能であること。
除外基準:
- 以下の経験がある患者:a) 意図的な自傷行為(自傷行為は、トラウマ以前の精神保健上の問題の既往を示唆し、特定の精神科的支援を必要とするため)。
- 以下の経験がある患者:親密なパートナーによる傷害(これらの傷害は通常、家庭内暴力への慢性的な曝露に基づいており、患者の安全を促進し暴力のパターンを断ち切る介入を必要とするため)。
- 以下の経験がある患者:暴力的に生じた傷害(この患者集団は、サニーブルック病院の「共感による暴力の連鎖を断ち切る(BRAVE)」プログラムを通じて、既に特定の臨床的・社会的支援を受けているため)。
- 著しい認知障害および薬物乱用の既往があり、医療従事者がこれらの状態のいずれかがインフォームド・コンセントの提供および/または試験への安全な参加を妨げると判断した患者。

実験的:単群パイロット実現可能性試験
将来の無作為化比較試験(RCT)の前に実施状況と予備的影響を評価する。
|
訓練を受けたトラウマサバイバーのピアサポーター(n=4)は、研究チームメンバーによってサポートされます。ピアサポーターは、トラウマに配慮したコミュニケーション、境界線の設定、危機的状況の沈静化、セルフケア、二次的トラウマをカバーする5回のトレーニングを修了します。
トラウマサバイバーは急性期医療で募集され、入院リハビリテーションへの転院前に、訓練を受けたピアサポーターと一度会うオプションがあります。
入院リハビリテーションでは、訓練を受けたピアサポーターがファシリテートする1時間のピアサポートグループセッションに週2回、合計3週間参加します。
退院して地域社会に戻った後は、訓練を受けたピアサポーターがファシリテートする1時間のピアサポートグループセッションに週1回、合計12週間参加します。

これら8つの次元は、身体的および精神的健康の要約スコアにまとめられます。
