Pilot haalbaarheidsonderzoek naar een lotgenotenondersteuningsprogramma voor traumaslachtoffers
Pilotstudie naar de haalbaarheid van een peer support programma voor traumaslachtoffers
Traumatische verwondingen door gebeurtenissen zoals auto-ongelukken, vallen of branden zijn een belangrijke oorzaak van invaliditeit. Overlevenden kampen vaak niet alleen met fysieke uitdagingen, maar ook met langdurige pijn, geestelijke gezondheidsproblemen en moeilijkheden om terug te keren naar werk en gezinsleven. Hoewel medische zorg zich richt op fysiek herstel, geven overlevenden vaak aan dat hun emotionele en sociale behoeften over het hoofd worden gezien en onvervuld blijven.
Lotgenotenondersteuning - begeleiding van mensen die soortgelijke verwondingen hebben meegemaakt - kan leed verminderen en het omgaan ermee verbeteren. Er zijn echter momenteel geen programma's in Canada die lotgenotenondersteuning bieden aan traumaslachtoffers gedurende hun gehele herstelproces, van ziekenhuis tot revalidatie tot leven in de gemeenschap.
De onderzoekers hebben samengewerkt met traumaslachtoffers, mantelzorgers en zorgverleners om gezamenlijk een Lotgenotenondersteuningsprogramma (PSP) te ontwerpen dat ondersteuning biedt tijdens al deze fasen. Deze pilotstudie zal testen of het programma haalbaar en aanvaardbaar is, en de vroege impact op herstel onderzoeken. De resultaten zullen een toekomstige grotere studie begeleiden om lotgenotenondersteuning in traumazorg uit te breiden.

Inclusiecriteria:
- volwassen patiënten (18 jaar en ouder) die een onopzettelijk traumatisch letsel hebben opgelopen dat heeft geleid tot opname in het ziekenhuis Sunnybrook en die na acute zorg worden ontslagen naar klinische revalidatie in St. John's Rehab.
- moeten in staat zijn om in het Engels te communiceren
Exclusiecriteria:
- patiënten die: a) een opzettelijk zelfbeschadigend letsel hebben opgelopen (omdat zelfbeschadiging wijst op een voorgeschiedenis van psychische problemen vóór het trauma, waarvoor specifieke psychiatrische ondersteuning nodig is)
- patiënten die: een letsel hebben opgelopen dat is veroorzaakt door een intieme partner (omdat deze letsels doorgaans worden veroorzaakt door chronische blootstelling aan huiselijk geweld, wat interventies vereist die de veiligheid van de patiënt bevorderen en het patroon van geweld doorbreken)
- patiënten die: een gewelddadig opgelopen letsel hebben (aangezien deze patiëntengroep al specifieke klinische en sociale ondersteuning ontvangt via het BRAVE-programma (BReAking the Cycle of Violence with Empathy) van Sunnybrook).
- patiënten met aanzienlijke cognitieve beperkingen en een voorgeschiedenis van middelenmisbruik als hun zorgverleners aangeven dat een van beide aandoeningen hun vermogen om geïnformeerde toestemming te geven en/of veilig deel te nemen aan de studie zal belemmeren

Experimenteel: Enkelarmige pilot haalbaarheidsstudie
Om implementatie en voorlopige impact te evalueren vóór een toekomstige RCT.
|
Getrainde lotgenotenondersteuners voor traumaslachtoffers (n=4), ondersteund door een lid van het onderzoeksteam; lotgenoten volgen een 5-sessies training die trauma-informele communicatie, grenzen, crisisde-escalatie, zelfzorg en secundair trauma behandelt.
Traumaslachtoffers worden geworven in acute zorg en hebben de optie om één keer voor ontslag naar klinische revalidatie een getrainde lotgenotenondersteuner te ontmoeten.
In klinische revalidatie zullen ze tweemaal per week 1 uur durende lotgenotengroepsessies bijwonen, gefaciliteerd door de getrainde lotgenotenondersteuners, gedurende in totaal 3 weken.
Na ontslag naar de gemeenschap zullen ze eenmaal per week 1 uur durende lotgenotengroepsessies bijwonen, gefaciliteerd door de getrainde lotgenotenondersteuners, gedurende in totaal 12 weken.

Om de haalbaarheid van de implementatie van het Peer Support Programma (PSP) voor traumaslachtoffers te bepalen. We zullen de onderstaande criteria meten om de haalbaarheid vast te stellen. Deze haalbaarheidsdoelen zijn opgesteld met verwijzing naar een andere studie die onze groep heeft uitgevoerd, gericht op het beoordelen van de haalbaarheid van een nieuwe psychosociale groepinterventie voor geamputeerden.

De 11-item Reintegration to Normal Living Index (RNL) Index behandelt gebieden zoals betrokkenheid bij recreatieve en sociale activiteiten, de waargenomen mogelijkheid om zich binnen de gemeenschap te verplaatsen, en de mate van comfort die mensen ervaren in hun relaties.
Het is een valide en betrouwbaar instrument voor het beoordelen van participatie in verschillende populaties.
Scores variëren van 0 tot 110, waarbij een hogere score een hoger niveau van gemeenschapsre-integratie aangeeft.

De 20-item CSE-schaal66 is een coping zelfeffectiviteitsschaal met een robuuste factorstructuur voor verschillende soorten posttraumatische gebeurtenissen, waardoor deze bijzonder geschikt is voor gebruik bij diverse traumapopulaties.
Voor elk item beoordelen respondenten hun waargenomen effectiviteit bij het omgaan met verschillende gevolgen van trauma op een 7-puntsschaal (bijv.
'het hervatten van een normaal leven'; 'optimistisch zijn sinds de gebeurtenis').
Scores variëren van 20 (lage zelfeffectiviteit) tot 140 (hoge zelfeffectiviteit).
De interne consistentie van de schaal is uitstekend bevonden over drie panelgolven van gegevens bij personen die in de voorgaande twee jaar een traumatische gebeurtenis hadden meegemaakt

De Short-Form 36 (SF-36) is het meest gebruikte generieke gezondheidsgerelateerde QoL-instrument, inclusief neuro-musculoskeletale letsels, dat 8 multidimensionele gezondheidsaspecten meet: fysiek functioneren, sociaal functioneren, rolbeperkingen door fysieke en emotionele problemen, mentale gezondheid, energie/vitaliteit, pijn en algemene gezondheidsperceptie.
Scores variëren van 0 tot 100 op elk domein, waarbij lagere scores een slechtere functie aangeven.
Deze acht dimensies worden samengevat in een fysieke en mentale gezondheidssamenvattingsscore.
De SF-36 is gevoelig voor veranderingen in functioneren in de loop van de tijd en heeft klinisch betekenisvolle, zij het bescheiden, verandering aangetoond bij traumapatiënten.

De PTSD Checklist-Civilian Version (PCL-C) is een gestandaardiseerde zelfrapportage-schaal met 17 items voor PTSS die goed gevalideerd is in verschillende populaties.
Respondenten geven aan in welke mate ze de afgelopen maand door een symptoom zijn gehinderd op een 5-puntsschaal, waarbij hogere scores duiden op ernstigere niveaus van PTSS.

Items worden gescoord op een 6-puntsschaal en scores variëren van 0 tot 105, waarbij hogere scores een groter niveau van groei aangeven.
Het heeft goede niveaus van interne consistentiebetrouwbaarheid (α=.90) en 2-maanden test-hertest stabiliteit (r =.71).

De Medical Outcomes Study Social Support Survey (MOS-SS) is een 19-items meetinstrument voor ervaren sociale steun over vier subschalen: emotionele/informatie steun; tastbare steun; affectieve steun; en positieve sociale interactie.
Vragen worden beantwoord op een 5-puntsschaal variërend van "nooit" tot "altijd", waarbij hogere scores meer steun aangeven.
De MOS-SS heeft een zeer goede interne consistentie in verschillende populaties, waaronder chronische ziekten en chronische pijn.
