트라우마 생존자를 위한 동료 지원 프로그램의 파일럿 타당성 시험
자동차 사고, 낙상, 화재와 같은 사건으로 인한 외상성 손상은 장애의 주요 원인입니다. 생존자들은 종종 신체적 어려움뿐만 아니라 지속적인 통증, 정신 건강 문제, 직장 및 가정 생활로의 복귀 어려움에 직면합니다. 의료 서비스가 신체적 회복에 초점을 맞추고 있음에도 불구하고, 생존자들은 자신들의 정서적 및 사회적 요구가 간과되고 충족되지 않는다고 자주 보고합니다.
비슷한 손상을 겪은 사람들로부터의 동료 지원-지도는 고통을 줄이고 대처 능력을 향상시킬 수 있습니다. 그러나 현재 캐나다에는 외상 생존자들에게 병원에서 재활, 그리고 지역 사회 생활에 이르기까지 회복 과정 전반에 걸쳐 동료 지원을 제공하는 프로그램이 없습니다.

- 의도하지 않은 외상성 손상으로 인해 Sunnybrook에서 입원 치료를 받았으며 급성 치료 후 St. John's Rehab으로 입원 재활 치료를 위해 퇴원할 성인 환자(18세 이상).
- 영어로 의사소통이 가능해야 함.
제외 기준:
- 다음과 같은 환자: a) 의도적인 자해 손상(자해는 외상 전 정신 건강 문제의 병력이 있음을 시사하며 특정 정신과적 지원이 필요하기 때문)
- 다음과 같은 환자: 친밀한 파트너에 의해 발생한 손상(이러한 손상은 일반적으로 가정 폭력에 대한 만성적 노출에 기반하며 환자 안전을 증진하고 폭력 패턴을 중단시키는 중재가 필요하기 때문)
- 다음과 같은 환자: 폭력적으로 입은 손상(이 환자 집단은 이미 Sunnybrook의 BReAking the Cycle of Violence with Empathy (BRAVE) 프로그램을 통해 특정 임상 및 사회적 지원을 받고 있기 때문).
- 중대한 인지 장애 및 약물 남용 병력이 있는 환자로서 의료진이 해당 상태가 정보에 입각한 동의를 제공하거나/또는 시험에 안전하게 참여하는 능력을 저해할 것이라고 판단하는 경우

실험적: 단일 군 파일럿 타당성 연구
향후 무작위 대조 시험(RCT) 이전에 시행 및 예비 효과를 평가하기 위함입니다.
|
훈련받은 외상 생존자 동료 지원자(n=4)는 연구팀 구성원의 지원을 받으며, 동료들은 외상 인식 커뮤니케이션, 경계 설정, 위기 완화, 자기 관리 및 이차 외상을 다루는 5회기 훈련을 완료합니다.
외상 생존자들은 급성 치료에서 모집되며, 입원 재활 전에 훈련받은 동료 지원자와 한 번 만날 수 있는 선택권을 갖습니다.
입원 재활 기간 동안, 그들은 훈련받은 동료 지원자들이 진행하는 1시간 동료 지원 그룹 세션에 주 2회, 총 3주 동안 참석합니다.
퇴원 후 지역사회에서는, 훈련받은 동료 지원자들이 진행하는 1시간 동료 지원 그룹 세션에 주 1회, 총 12주 동안 참석합니다.

각 항목에 대해 응답자들은 외상의 다양한 결과에 대처하는 지각된 효능감을 7점 척도로 평가합니다(예: '정상적인 생활로 복귀하기'; '사건 이후 낙관적이기').
점수는 20점(낮은 자기효능감)에서 140점(높은 자기효능감)까지 범위를 가집니다.
이 척도의 내적 일관성은 지난 2년 동안 외상적 사건을 경험한 사람들의 3개 패널 웨이브 데이터에서 우수한 것으로 나타났습니다.

Short-Form 36(SF-36)은 신경근골격계 손상을 포함한 가장 널리 사용되는 일반 건강관련 삶의 질 측정 도구로, 신체 기능, 사회적 기능, 신체적 및 정서적 문제로 인한 역할 제한, 정신 건강, 에너지/활력, 통증, 일반 건강 인식 등 건강의 8가지 다항목 차원을 측정합니다.
각 영역의 점수는 0에서 100까지이며, 낮은 점수는 더 나쁜 기능을 나타냅니다.
이 8가지 차원은 신체 건강과 정신 건강 요약 점수로 요약됩니다.
SF-36은 시간에 따른 기능 변화에 민감하며, 외상 환자에서 임상적으로 의미 있는(비록 소폭이지만) 변화를 보여주었습니다.
