この臨床試験の目的は、非侵襲的脳刺激(NIBS)が慢性閉塞性肺疾患(COPD)患者の認知機能を向上させることができるかどうかを検証することです。

研究者は、NIBSがCOPD患者の認知機能改善に有効かどうかを確認するため、NIBSと偽NIBSを比較します。
参加者は以下の手順を行います:
NIBSまたは偽NIBSを、週5回、3週間にわたり、各セッション20分間受けます。
初回にクリニックを訪問してベースライン評価を受け、3週間の介入終了後に再評価を受けます。

- 慢性閉塞性肺疾患(COPD)は、全世界で320万人以上の死亡例があり、3番目に多い死亡原因とされ、潜在的に致命的な疾患とみなされています。
- 肺構造の異常により、COPDはさまざまな呼吸器症状を特徴としています。
- 認知機能障害(CI)は、呼吸器系を超えてCOPDが引き起こす問題の一つです。
- CIは、COPD患者の生活の質、治療遵守、状態の悪化すべてに悪影響を及ぼしました。
- 現在のところ、COPD患者のCIを改善するための介入は確約されていません。
目的:
新しい技術である非侵襲的脳刺激(NIBS)が、COPD患者の認知機能にどの程度効果的であるかを明らかにすること
方法:
- 参加者と結果評価者の両方が二重盲検RCTに参加します。
- キング・ファハド大学病院から完全に自発的な参加者を募集します。
- 研究プロトコルは、イマーム・アブドゥルラフマン・ビン・ファイサル機関のIRBによって承認されました。
- モントリオール認知評価テスト(MoCA)による評価後、参加者はGOLDに基づいて4つのグループに分けられます:軽度GOLD 1、中等度GOLD 2、重度GOLD 3、非常に重度GOLD 4。
- 3週間、両グループは週5回のNIBSセッションに参加します。実験群はアクティブな脳刺激を受け、対照群は偽の脳刺激を受けます。
- 両グループは治療セッション前後に評価を受けます。
- 主要なアウトカム指標はバッテリー認知テストです。二次的指標には、ABGs、超音波検査、心肺運動負荷試験が含まれます。
- 両グループ間の差を評価するために独立したt検定が適用されます。
- 研究では、介入の有効性を判断するために二因子反復測定分散分析(ANOVA)を使用します。
- 認知機能の改善と特定の要因との関連は、ピアソン相関係数によって確認されます。
- 有意水準はp値が0.05未満に設定されます。

- 実験群には40Hzで2mAのガンマ経頭蓋交流電流刺激(tACS)が与えられます。

• 評価される2つの主要な領域は記憶と注意力です。
• Flanker課題は、一つの刺激に焦点を当てながら同時に別の刺激の検出を防ぐことで、実行制御ネットワークがどの程度調整できるかを調べることで注意力領域をテストします。
• Memory Automaticity課題は作業記憶を評価するために使用され、参加者は自動性を達成するためにSternberg式の記憶検索を完了する必要があります。記憶セット内の特定の文字の存在を思い出し、正確に分類することがこの課題の前提条件です。

• VO2maxを算出するためのゴールドスタンダードの運動負荷試験は心肺運動負荷試験(CPET)です。
• COPD患者の機能的能力とVO2maxを評価するために使用されます。
被験者ごとに最大努力での漸増的CPETを用いて評価が行われます。
• 修正ブルースプロトコルを用いてトレッドミル上で試験が実施されます。
• ブレスバイブレス分析により、以下のガス交換および換気測定値が評価されます:VO2、二酸化炭素と酸素の呼気終末分圧(PETCO2およびPETO2)、二酸化炭素換気当量(VE/VCO2)、分時換気量(VE)。
- 事前に訓練された研究者が試験を実施します。

- 拡張期末期および収縮期ピークの血流速度の両方を考慮して、平均血流速度(MFV)が計算され、側頭骨窓を使用していくつかの重要な頭蓋内動脈が評価されます。
- 資格のある神経科医および血管技術者が検査を実施します。
