참가자는 다음과 같은 과정을 거칩니다:

- 만성폐쇄성폐질환(COPD)은 전 세계적으로 320만 명 이상의 사망자를 기록하며 세 번째로 흔한 사망 원인으로 간주되며 잠재적으로 치명적인 질병입니다.
- 폐 구조의 이상으로 인해 COPD는 다양한 호흡기 증상이 특징입니다.
- 인지 장애(CI)는 호흡기계를 넘어 COPD로 인해 발생하는 문제 중 하나입니다.
- COPD 환자의 삶의 질, 치료 순응도 및 상태 악화는 모두 CI로 인해 부정적인 영향을 받았습니다.
- 현재까지 COPD 환자의 CI를 개선하기 위해 약속된 중재는 없습니다.
목적:
새로운 기술인 비침습적 뇌 자극(NIBS)이 COPD 환자의 인지 기능에 미치는 효과를 알아보기 위함
방법:
- 참가자와 결과 평가자 모두 이중맹검 무작위 대조군 시험(RCT)에 참여합니다.
- 킹 파하드 대학 병원에서 전적으로 자발적인 참가자를 모집할 것입니다.
- 이맘 압둘라흐만 빈 파이살 기관의 IRB가 연구 프로토콜을 승인했습니다.
- 몬트리올 인지 평가 검사(MoCA)를 사용한 평가 후, 참가자는 GOLD 기준에 따라 경증 GOLD 1, 중등도 GOLD 2, 중증 GOLD 3, 극심증 GOLD 4의 네 그룹으로 나뉩니다.
- 두 그룹 모두 3주 동안 주당 5회의 NIBS 세션에 참여합니다. 대조군은 가짜 뇌 자극을 받는 동안 실험군은 실제 뇌 자극을 받습니다.

• VO2 최대치를 계산하기 위한 표준 운동 검사는 심폐 운동 검사(CPET)입니다.
• 이 검사는 COPD 환자의 기능적 능력과 VO2max를 평가하는 데 사용될 것입니다.
각 피험자는 최대 노력으로 점진적 CPET를 사용하여 평가될 것입니다.
• 수정된 Bruce 프로토콜이 트레드밀에서 검사를 수행하는 데 사용될 것입니다.
• 호흡별 분석이 다음과 같은 가스 교환 및 환기 측정치를 평가하는 데 사용될 것입니다: VO2, 이산화탄소 및 산소의 말단 호기압(PETCO2 및 PETO2), 이산화탄소에 대한 환기 등가물(VE/VCO2), 분당 환기량(VE).

- 경두개 도플러(TCD) 초음파는 뇌 기저동맥 전체에 걸쳐 뇌혈관 혈역학 및 혈류 매개변수를 실시간으로 모니터링할 수 있습니다.
- 이완기 최저 혈류 속도와 수축기 최고 혈류 속도를 모두 고려하여 평균 혈류 속도(MFV)를 계산하며, 측두골 창을 사용하여 몇 가지 중요한 두개내 동맥을 평가합니다.
- 자격을 갖춘 신경과 전문의 및 혈관 기술자가 검사를 수행합니다.
