DSフェーズ3(CareFit-DS/AD)向けにRDADを適応するにあたり、20組のDS成人/介護者ペアを対象に、12週間の単一アームパイロット試験介入を実施します。 この介入には、DS成人とその介護者向けの遠隔機能運動クラス、および介護者向けの遠隔認知症介護トレーニングセッションが含まれます。 DS参加者は、ビデオ通話を通じて12週間のグループ運動セッション(週3回、各約30分、介護者の参加は任意)を完了し、介護者は週1回約40分の認知症介護者トレーニング/サポートグループのグループビデオ通話セッションも受けます。
スタッフは、ベースライン時および12週間後に参加者の自宅を訪問し、身体機能の評価を実施し、Actigraph加速度計を設置します。 介護者用唾液採取キットは事前に郵送され、家庭訪問時に回収されます。 さらに、ベースライン時にスタッフは介入実施用の機器を配達します。
12種類の事前録画された運動クラスセッション(持久力、筋力、バランス、柔軟性)が研究全体を通じて各3回実施されます(合計36セッション)。 運動セッションの内容と頻度は、日常生活活動のための機能的フィットネスを改善し、参加者に軽度および/または中程度の強度レベルで挑戦するように設計されています。 セッション中、すべての参加者はビデオ通話に参加し、事前録画された運動ビデオにグループ形式で従います。 対象集団との連携訓練を受けたスタッフが通話に参加し、参加者の安全を監視しながら励ましとフィードバックを提供します。 介護者は運動セッションに参加できますが、参加を推奨も抑制もしません。
介護者は、3回の運動セッションのうち1回の直後に、ビデオ通話を介した週1回のグループ行動管理トレーニングセッションに参加するよう求められます。 これらの会議は、認知症と診断されたDS患者を介護するための情報とリソースで構成されます。 グループは対象集団との連携訓練を受けたスタッフが進行し、提示された情報とグループディスカッションの時間を含みます。 トピックには、ダウン症候群におけるアルツハイマー病の概要、行動管理戦略、期待の変化などが含まれます。

対象者 - ダウン症候群の成人:
- 年齢 >35歳。自己申告または介護者による申告によるダウン症候群の診断。この年齢範囲は、ダウン症候群の成人においてアルツハイマー病に関連する認知機能の低下が約31歳で観察されるため選択されました。
- 指示を理解し、話し言葉を通じて好み、欲求、必要性を伝える十分な機能的能力。
- 研究パートナーとして同意する親/介護者と自宅または支援付き居住環境に居住していること。
- 身体活動に参加できる自己申告能力。
除外基準 - ダウン症候群の成人:
- 中程度から激しい身体活動に参加できない。
- 自己申告による心血管、代謝、または腎臓疾患および/または兆候や症状。
対象者 - 介護者
- 年齢 ≥18歳。
- ダウン症候群の成人の主な介護者であると申告していること。
- 英語を話すこと。
- 身体活動に参加できる自己申告能力。
除外基準 - 介護者
- 中程度の身体活動(例:速歩き)に参加できない。
- 自己申告による心血管、代謝、または腎臓疾患および/または兆候や症状。

単群試験:遠隔運動と介護者教育セッションのための積極的介入
|
参加者は、ダウン症候群の高齢者向けに設計された事前録画されたビデオを使用して、週3回の運動セッションに参加します。
参加者はビデオ通話に参加し、ビデオに合わせて運動を行います。
この集団に精通したスタッフが通話に参加し、運動の調整、励ましを提供し、参加者の安全を確保します。
介護者は、訓練を受けたスタッフが主導するビデオ通話による週1回の教育セッションに参加します。
教育内容には、アルツハイマー病に関連する症状を経験している可能性のあるダウン症候群の高齢者の困難な行動をサポートし、管理する方法が含まれます。

Each stance is attempted to be held for 10 seconds.
Higher scores indicate improved physical function.

Responses are on a 5 point scale ranging from 'Without any difficulty' scoring 5, to 'Unable to do' scoring 1.
Higher scores indicate improved physical function.
