상세 설명
인공 수정체는 이제 나이가 많은 소아의 백내장에 일반적으로 허용되는 치료법이며 어린 소아와 유아에서 점점 더 많이 사용됩니다. 안내 렌즈는 수정체의 광학을 보다 밀접하게 복제하고 매일 지속적으로 관리할 필요가 없으며 항상 적어도 부분적인 광학 교정을 보장한다는 점에서 콘택트 렌즈보다 우수합니다. 인공 수정체 교정의 단순성과 개선된 시각적 결과는 편측 선천성 백내장이 있는 어린이를 돌보는 것이 부모에게 덜 스트레스가 될 수 있습니다. 그러나 콘택트렌즈는 안내 렌즈의 장기적인 안전성에 대한 우려와 눈이 성장함에 따라 큰 근시 변화가 발생할 가능성 때문에 1세 미만의 어린이에게 허용되는 치료법으로 남아 있습니다. 콘택트렌즈는 양측 선천성 백내장 치료를 받은 영아에게 탁월한 시력 결과를 제공합니다. 그러나 편측성 선천성 백내장을 위해 콘택트렌즈로 치료받은 영아의 2/3는 무수정체 눈이 법적으로 실명 상태로 남아 있습니다. 이러한 열악한 시각적 결과는 일반적으로 건전한 눈과의 경쟁 및 패칭 및 콘택트 렌즈 착용 요법에 대한 열악한 순응도에 기인합니다. 파일럿 연구 및 문헌의 데이터에 따르면 유아기에 편측 무수정체를 교정하기 위해 안내 렌즈를 사용하면 우수한 시각적 결과를 얻을 수 있지만 이러한 눈은 더 많은 합병증을 경험할 수 있습니다. 인공 수정체는 우리가 이 실험을 수행하는지 여부에 관계없이 유아에게 점점 더 많이 이식될 것입니다. 이 임상 시험을 수행함으로써 안내 렌즈의 높은 합병증 비율이 시각적 결과 개선과 육아 스트레스 감소로 상쇄되는지 확인할 수 있습니다.
IATS(Infant Aphakia Treatment Study)는 단안 실어증에 대한 안내 렌즈와 콘택트 렌즈 교정을 비교하는 다기관 무작위 임상 시험입니다. 유아는 4년 동안 등록됩니다. 한쪽 눈에 시각적으로 유의미한 백내장이 있는 생후 28~210일의 유아가 자격이 있습니다. 백내장 수술은 연구 인증을 받은 외과의사에 의해 표준화된 방식으로 수행됩니다. 수술은 수정체 절제술, 후낭 절개술, 전방 유리체 절제술로 구성됩니다. 영아는 수술 시 두 치료 그룹 중 하나로 무작위 배정됩니다. 인공 수정체 그룹으로 무작위 배정된 영아는 인공 수정체를 수정체 낭에 이식하게 됩니다. 안경은 이후 잔여 굴절 이상을 교정하는 데 사용됩니다. 콘택트렌즈 그룹으로 무작위 배정된 영아는 수술 직후 콘택트렌즈를 착용하게 됩니다. 두 그룹 모두 동일한 패칭 요법과 후속 조치를 받게 됩니다. 모든 어린이는 표준 프로토콜을 사용하여 고정된 간격으로 조사관에 의해 검사되며 주요 종점은 12개월에 이동 시력 검사관에 의해 평가됩니다.
우리는 현재 10.5세에 녹내장 또는 녹내장이 의심되는 환자를 평가하기 위해 이 프로젝트(5년 이상)를 계속하고 있습니다. 우리의 목표는 유아기에 일측성 선천성 백내장 수술 후 어떤 유형의 초기 광학 교정, IOL 또는 CL이 장기적으로 최상의 시력 결과를 가져오는지 이해하는 것입니다. 우리의 중심 가설은 기본 IOL 이식이 더 나은 시각적 결과를 가져올 것이라는 것입니다. 이 제안의 근거는 최종 시력이 5세까지 결정될 수 없으며 조기 치료에 대한 권장 사항은 이 독특한 코호트에서 적절한 장기 평가에 의해서만 입증될 수 있다는 것입니다. 보다 정확한 시력 평가를 제공하고 대부분의 녹내장 사례를 진단할 수 있기 때문에 10.5세까지의 후속 조치를 선택했습니다.

설명
포함 기준:
- 시각적으로 유의미한 편측 선천성 백내장(중앙 혼탁 크기가 3mm 이상).
- 백내장 수술은 환자가 생후 28~210일이고 임신 후 최소 41주 이상일 때 수행됩니다.
제외 기준:
- 백내장은 외상이나 방사선 치료, 약물 치료 등 산후에 시행되는 치료의 부작용으로 발생하는 것으로 알려져 있다.
- 캘리퍼스를 사용하여 수평 자오선에서 측정한 각막 직경이 9mm 미만인 경우.
- 영향을 받은 눈에서 Perkins 안압계, tonopen 또는 기압계로 측정한 안압이 25mmHg 이상입니다.
- 모양체 과정의 스트레칭 또는 견인 망막 박리를 유발하는 지속적인 태아 혈관계(PFV).
- 활동성 포도막염 또는 후유착 또는 케라틱 침전물과 같은 포도막염의 이전 에피소드를 암시하는 징후.
- 아이는 조산(임신 36주 미만)의 산물입니다. 미숙아 선별검사는 임신 주수에 대한 임상의의 최선의 평가를 기반으로 합니다. 의사가 재태 연령에 대해 확신이 없는 경우 분만 시 재태 연령을 확인하기 위해 의료 기록을 검토하거나 소아과 의사 및/또는 산부인과 의사에게 연락해야 합니다. 아이가 36주 미만에 태어났는지 여부에 대해 임상의가 확신하지 않는 한, 의료 기록 검토를 통한 재태 연령 확인은 등록 후까지 지연될 수 있습니다.

- 아이가 생후 12개월 또는 4세에 시력을 얻는 능력을 제한하는 것으로 알려진 의학적 상태를 가지고 있습니다.
- 부모/법적 보호자가 정보에 입각한 동의서에 서명하거나 두 치료 그룹 중 하나로 무작위 배정되는 것을 거부합니다.
- 아동이 정기적인 후속 검사 및 결과 평가(예: 교통 문제, 재배치 등)를 위해 돌아올 수 없기 때문에 아동에 대한 후속 조치가 가능하지 않습니다.

시력은 생후 12개월에 표준 객관적 검사 절차에 의해 측정되었습니다.
단안 격자 시력은 Teller Acuity Cards를 사용하여 순회 심사관이 평가했습니다.
이 테스트는 너비가 다양한 흰색 선이 있는 카드를 사용하고 크기가 고정된 사각형에서 설정된 거리만큼 떨어져 있으므로 선이 얇을수록 지정된 카드(주기/cm)에 더 많이 표시됩니다.
가는 선을 보는 능력은 더 나은 시력을 나타냅니다.
선이 있는 카드는 회색 카드와 동시에 제시되며 아이의 시각적 주의가 주목됩니다.
아이는 줄무늬가 있는 카드가 더 재미있기 때문에 우선적으로 볼 것으로 추정됩니다.
선이 너무 가늘고 서로 가까워서 회색 카드와 구별할 수 없는 경우 우선적으로 보이는 것으로 표시되지 않습니다.
아이가 보게 될 가장 가는 선이 있는 카드가 logMAR 단위의 최고 시력으로 기록됩니다.

단안 시력은 EVAT에 표시된 서라운드 바가 있는 단일 문자 시표를 사용하여 평가되었습니다.
ATS 프로젝트의 계단식 절차는 이 연령대 그룹의 성공과 신뢰성을 문서화했기 때문에 따랐습니다.
HOTV 매칭 테스트에 대한 피험자들의 친화를 위해 현장 경험자에 의한 4.0년 방문과 4.25년 방문 시 이 테스트를 도입하였다.
