암성 질환의 완화 단계에서 반란군의 통증 치료에서 높은 오피오이드와 관련하여 낮은 진통제 용량으로 케타민의 효율성에 대한 연구
장기 아편유사제 요법은 중증 암 통증 관리를 위해 일반적으로 시행됩니다.
진통 효과가 나타나거나 참을 수 없는 부작용이 발생할 때까지 오피오이드의 용량을 증가시키면 효과에 대해 적정됩니다.
N-메틸-D-아스파르테이트(NMDA) 수용체 길항제인 케타민은 고용량의 아편유사제를 투여받은 조절되지 않는 통증 환자의 진통 효과를 개선하는 것으로 보고되었습니다.

오피오이드와 관련된 4일 동안 정맥 내 주입 시 케타민의 낮은 진통 용량이 케타민이 없는 오피오이드보다 불응성 암 통증을 더 잘 완화한다는 것을 보여주기 위해.
이 연구는 3단계로 구성된 다기관(11개 센터)의 전향적 연구입니다.
- 각각 38명의 환자로 구성된 2개의 병렬 그룹으로 구성된 5일의 무작위 통제 이중 맹검 단계: 케타민(고농도 아편유사제와 관련하여 4일 동안 정맥 주사) 대 위약(고농도 아편유사제와 관련하여 4일 동안 정맥 주사) ;
- 최대 4일의 공개 라벨 단계(이 기간 동안 여전히 조절되지 않는 통증이 지속되거나 재발하는 입원 환자에게 케타민 판파마(등록상표)가 정맥내 주입으로 투여됨);
관찰 단계: 환자가 퇴원할 때 시작하여 최대 6개월. 포함 기간은 18개월이며 총 연구 기간은 2년입니다.
76명의 환자가 예상됩니다: 38명은 오피오이드와 케타민으로 치료될 것입니다. 38명은 오피오이드와 위약으로 치료할 것입니다.
성공은 4일 후 일일 통증 점수의 50% 감소로 정의됩니다. 위약 그룹의 예상 성공률은 10%인 반면 케타민 그룹의 예상 성공률은 30%입니다.
1차 결과(11점 수치 척도의 통증 점수) 및 2차 결과(환자 및 임상의 전반적인 변화에 대한 인상, 오피오이드 소비, 부작용, 통증 완화에 대한 환자 만족도, 수면 방해 점수)를 매일 평가합니다.
바이탈 파라미터(심장 빈도, 호흡 빈도 및 동맥 혈압)는 매일, 하루에 여러 번 확인됩니다. 용량 변경 후 2시간 동안 매시간).
