IA1기(LVSI+) 및 IA2-IB1기(≤ 2CM) 자궁경부암에 대한 비근치적 수술 요법(추가 근막 자궁적출술 또는 골반 림프절 절제술을 통한 추체 생검) 전후의 신체 기능 및 삶의 질(QOL) 평가
이번 임상시험은 자궁경부암 1기 환자의 수술 전후 신체 기능과 삶의 질을 연구한다.
자궁경부암 수술을 받는 환자의 삶의 질을 연구하면 수술의 중기 및 장기 효과를 판단하는 데 도움이 될 수 있습니다.

기본 목표:
I. IA1기(림프관 침습 양성[LVSI+])에 대한 방광, 장 및 성기능의 기능적 결과에 대한 비근치적 외과적 치료(단순 자궁절제술 또는 원뿔 생검[가임력 보존] 및 골반 림프절 절제술) 전후의 변화를 조사하기 위함 ) 및 IA2-IB1(=< 2 cm) 자궁경부의 암종.
II. 자궁경부의 IA1기(LVSI+) 및 IA2-IB1기(=< 2cm) 암종에 대한 비근치적 수술(단순 자궁적출술 또는 원뿔 생검[가임력 보존] 및 골반 림프절 절제술) 후 림프부종의 발생률 및 중증도를 평가합니다.
2차 목표:
I. 근치적 수술(근치적 자궁절제술 또는 근치 자궁적출술)에 대한 과거 데이터와 비교하여 비근치적 수술(단순 자궁적출술 또는 골반 림프절 절제술을 통한 원뿔 생검[가임력 보존])이 더 큰 신체 기능과 더 적은 독성을 나타내는지 여부를 조사합니다.
II. 전체 코호트에서 치료 유형별로 수술 합병증을 포함한 치료 관련 부작용의 발생률과 중증도를 평가합니다.
III. 삶의 질(QOL)(Functional Assessment of Cancer Therapy-Cervix[FACT-Cx]), 암 걱정(Impact of Events Scale[IES]) 및 성적(여성 성기능 지수[FSFI])/생식 문제의 변화를 평가하기 위해 (RCS) 전체 코호트 중 및 치료 유형별.
IV. 기능적 결과(즉, 방광 기능, 장 기능, 성기능), 부작용(수술 합병증 림프부종[부인과 암 림프부종 질문지(GCLQ)] 포함), 암 걱정(IES) 사이의 관계(상관관계, 상호작용, 독립성)를 탐색하기 위해 , 수술 합병증 및 전반적인 삶의 질(FACT-Cx).
V. IA1기(LVSI+) 및 IA2-IB1기(=< 2 cm) 자궁 경부의 암종.
VI. 자궁경부의 IA1기(LVSI+) 및 IA2-IB1기(=< 2cm) 암종에 대한 비근치적 수술(단순 자궁적출술 또는 원뿔 생검[가임력 보존] 및 골반 림프절 절제술)의 효능을 추정합니다.
개요:
환자는 원뿔 생검 및 골반 림프절 절제술 또는 단순 자궁 절제술 및 골반 림프절 절제술을 받습니다.
환자는 FACT-Cx, IES, FSFI, GCLQ 및 PROMIS(Patient Reported Outcomes Measurement Information System) 설문지를 기준선, 수술 후 4~6주 후, 그리고 3년 동안 6개월마다 작성합니다. 콘 생검 및 골반 림프절 절제술을 받는 환자도 ICF 및 RCS 설문지를 작성합니다.
연구 치료 완료 후, 환자는 4-6주, 1년 동안 3개월마다, 그 후 2년 동안 6개월마다 추적 관찰됩니다.

- 환자는 적절한 수술에 동의해야 합니다.
- 편평 세포 암종, 선암종 또는 자궁경부의 선편평 세포 암종, IA1기(림프-혈관 공간 침범[LVSI]+), IA2 및 IB1기(종양 크기[최대 가시적 또는 만져질 수 있는])의 조직학적 진단이 있는 환자 =< 2cm), 모든 등급
- 모든 환자는 콘 생검 또는 루프 전기 수술 절제 절차(LEEP)를 받아야 합니다. 침입 깊이는 =< 10mm여야 합니다.
- 환자는 양전자 방출 단층 촬영(PET) 스캔 또는 자기 공명 영상(MRI) 또는 컴퓨터 단층 촬영(CT) 스캔에서 전이의 증거가 없어야 합니다.
- 사전 입국 요건을 충족한 환자
- 환자는 승인된 동의서 및 개인 건강 정보 공개를 허용하는 권한에 서명해야 합니다.
- 환자는 부인과 종양학 그룹(GOG) 수행 상태가 0, 1 또는 2여야 합니다.
제외 기준:
- LVSI 음성인 IA1기 질환 환자
- 종양 크기(가시적 또는 만져질 수 있는 최대 크기) > 2cm인 IB1기 환자
- >= IB2기 질환이 있는 환자
- 명확한 세포 또는 신경 내분비 세포 유형을 가진 환자
- 첫 번째 원뿔 생검(또는 LEEP)에서 침습 깊이 > 10 mm인 환자
- 비흑색종 피부암을 제외하고 다른 침윤성 악성 종양의 병력이 있는 환자는 지난 5년 이내에 다른 악성 종양의 증거가 있는 경우 제외됩니다. 이전 암 치료가 이 프로토콜 요법을 금하는 경우 환자도 제외됩니다.
