선천성 심장 수술 중 심장 보호제인 Dexrazoxane의 사용: 파일럿 조사를 위한 제안
외과 의사가 심장 수술을 수행할 수 있도록 심장 수술 중 심폐 바이패스 및 심장 정지가 필요합니다. 그러나 이러한 과정은 수술 후 결과를 악화시킬 수 있는 심장 손상을 유발할 수 있습니다. 사실, 이러한 부상의 영향은 수술 후에도 계속될 수 있으며 심장 기능의 장기적인 감소로 이어질 수 있습니다. 신생아와 어린 영아는 특히 이런 일이 발생할 위험이 있습니다.
수술 중 심장을 보호할 수 있는 약물을 찾는 성인에 대해 많은 연구가 수행되었지만 신생아와 유아에 대한 연구는 거의 수행되지 않았습니다. 연구자들은 선천성 심장 수술 중 및 수술 후 심장 손상을 줄여 신생아 및 유아 인구의 결과를 개선할 수 있기를 바라며 소아암 환자에서 심장 보호 효과가 있는 것으로 입증된 Dexrazoxane을 테스트하고 있습니다.

상세 설명
심정지가 있는 심장 수술을 받는 신생아와 유아는 염증과 심근 허혈-재관류[IR] 손상을 모두 경험합니다. 이러한 과정은 심근 세포 사멸 및 산소 자유 라디칼 형성을 유발하여 심장 손상 및 기능 장애를 유발합니다. Dexrazoxane은 안트라사이클린 관련 심독성 예방용으로 승인된 EDTA의 유도체입니다. 그것은 독성 반응성 산소 종[ROS]의 감소와 세포 사멸의 억제를 통해 심장 보호를 제공합니다.

조사관은 선천성 심장 수술 동안 덱스라족산 대 위약의 무작위, 이중 맹검, 임상 시험이 뒤따르는 생후 0-6개월의 어린이를 대상으로 덱스라족산의 파일럿 약동학/안전성 시험을 제안합니다. 조사관은 장기 부전 해결까지의 수술 후 시간, 저심박출량 증후군의 발생, 심장 ICU 및 입원 기간, 심장 기능 장애의 심초음파 지표를 평가할 것입니다. 결과는 선천성 심장 수술 중 허혈-재관류 손상을 개선하는 데 있어 dexrazoxane의 안전성과 임상적 유용성을 확립할 수 있습니다.

시험 피험자는 수술 전에 저용량(200mg/m2/용량), 중간 용량(300mg/m2/용량) 또는 고용량(400mg/m2/용량)의 세 가지 용량 중 하나로 Dexrazoxane을 받도록 배정됩니다. 총 12명의 환자에 대해 각 투여 요법에 4명의 환자가 배정됩니다. 약물은 수술실에서 심폐우회 시작 15~30분 전(dose #1), 심폐우회 종료 후(dose #2), 수술 후 아침에 심장중환자실(dose #3)에서 투여한다. ). |
